While cleaning P92A ore pass grizzly in P91 at around 21:25 of plug reversing in 1st gear, hit rib with left rear corner of loader. Upon contact with rib, low speed, felt sharp pain in front right shoulder.

Replacing return roller on locked out conveyor belt system when jack failed and belt pinned hand against roller. No broken bones, pinched muscles and loss of sensation to hand.
On 08/22/2025 at 1300 an employee was inside tub of Grade 1 Disc filter north retrieving piece of cpvc pipe that was dropping the drain valve. The employee moved their leg against an agitator fin which cut their upper thigh resulting in 3 sutures to close wound.
Mechanic was inspecting a bolter to replace the striker bar. The extractor and striker bar were resting on the rails when the mechanic placed their left hand on the extractor and right hand on the striker bar it rolled forward and off of the bolter resulting in a laceration and a tufts fracture to the distal phalanx on right ring finger.
Employee was checking froth quality at cleaner cell when finger became pinched between froth paddle and cell weir.
Employee assisting with caustic transfer when they smelled something funny triggering an asthma attack.

Employee was washing their glassware from lunch when it contacted the counter, shattering the dish, causing lacerations to both hands. One of which required a single stitch.
Employee attempted to re-secure bolt on service truck air brake system, ring clamp released causing part of system to impact individual in face causing laceration to eyebrow and mild concussion.
The injured Miner was moving 8'x10' 6 gauge welded wire mesh panels from the stockpile to the LHD bucket for transport. The miner stepped on panels that were previously placed in the bucket, slipped and fell to miner's side.

EE was going to loosen the valve as steam came out. EE moved arm quickly, which was over EE's head, and that's when EE heard a pop while EE was moving arm.

Employee was picking up case of water from the pallet, tried to step backwards and caught foot on plastic wrapping around pallet. Employee fell backwards and hit back of head on concrete floor.
Employee was installing 30lbs bolts into belt liner. EE's grip slipped and EE lost grip on the bolt. The bolt then came down on EE's left 3rd finger dealing damage. This resulted in 8 stitches on the 3rd finger. The employee was well rested and fit for duty.
The employee was struck in the nose with a 1"" spud when the 1"" valve handle contacted the sill and opened, which caused the air hose to swing. The contact caused a laceration requiring 4 sutures to close.
While cleaning up P77 drift the mucker bucket hit the rib, causing an abrupt stop - forcing the operator's face into the windshield. The operator received several laceration on EE's face.
Right front wheel fell off the Kubota causing vehicle to stop abruptly resulting in fractured ribs.
Employee was helping with repair bolting at Henderson Mine. Employee was walking and stepped on a rock approximately 3""w x 3""L x 1 1/2"" H causing EE's left ankle to roll. The employee was wearing pull on steel toed muck boots. The area had roughly 1 inch of water in the area. Minimal trip hazards were noted during investigation. Employee suffered a sprain to their left ankle.

Mill Liner rigging failed leading to Employee Injury
While responding to a troubleshooting event, an electrician tripped on a toe kick area guarding in the area and fell down. Electrician hit abdomen and left shoulder. They did not experience pain but could not lift/move their shoulder. Employee given lifting restrictions site was unable to accommodate.

Employee was preparing to take a shower in the EE's Dry. They attempted to step over a bench and when they brought down their right foot impacted index toe on wooden edge. Toe is fractured and employee was given restricted duties preventing from walking up stairs. Site not able to accommodate restrictions.
Employee was assisting with a mill liner change and while handling the bolter, EE's left knee locked up causing strain.
Employees were tramming 30"" HDPE pipe at 3 Dam crest raise with excavators when one of the excavators got stuck on a sand pile. The other excavators kept moving and the excavator that was stuck tipped over causing the operator to get contusions on right shoulder and elbow.

While mucking in P75, employee was cleaning off the muck from the grizzly and the bucket of the LHD hit a rock stuck in the grizzly. The loader stopped suddenly causing EE's left arm/shoulder to hit door pad. This impact caused pain in the left shoulder that got worse on Sunday morning (6/20/21).
One employee's hard hat bumped the side of the chute causing it to fall (~8 feet) off EE's head and landed on another employee's left thumb.
Employee was shoveling snow (after applying salt to the area) and slipped on the ice outside of the Loci shop. The fall caused bruising on EE's back.

On November 3, 2016 at approximately 10:15 PM employee was sharpening powder knife on the 7700 level at 625 Day Mag. The knife slipped from the sharpening tool and cut right middle and index fingers. Employee was transported to the hospital and received 6 stitches.
Employee rolled ankle while walking thru the mill.
Employee was installing wall liners in PC1/PC2 transfer chute when the liner slipped from hand. The liner pinched left ring fingertip causing a laceration requiring 5 stitches.
On 11/23/2016 EE informed the supervisor that EE strained shoulder while performing ropes training on 10/11/2016. EE could not identify a specific event that cause the strain.
Employee was checking the hours and fire extinguisher on a Kamatsu track hoe. As EE was dismounting the piece of equipment EE's foot slid off of the step and EE fell backwards injuring back.
Employee was operating a skid-steer loader when hit a bump and jarred back.
While beginning to change liners of Mill #2 a steel ball about the size of a cue ball fell from above and landed on employees left foot hitting just behind the steel toe protection.
On 3/23/16 employee noticed EE's right wrist starting to hurt while changing belly liners, using the thunderbolt to hammer the liners out. The injured employee expressed a desire to keep working. It started feeling better so they let it go. Then on Monday 4/11/16 the wrist started hurting again. Clinic gave employee a splint and placed EE on restricted duty.

While shoveling a ditch in the crusher lube room, the employee went to lift the loaded shovel and twist. He felt a small pain in his lower back resulting in a lumbar strain
While attempting to descend a piece of equipment, the employee tripped or slipped, falling approximately 4' to the ground. The fall caused a right 1st rib fracture and a separated AC joint.
The employee was installing a roller frame on a PC2 Table. When He lift the frame, he twisted to maneuver it into place and heard/felt a pop in his hip and pain in his back.
The employee stepped off the boom to inspect the alignment of the drill steel when a small piece of rock fell off the face and landed on the boom startling the employee. He stumbled backwards and landed on a rock causing a laceration to his back.
The employee was using a chipping hammer bit and hammer to chisel large pieces of concrete off the slick line. In doing so, the employee missed the head of the chisel and struck his left middle finger causing multiple lacerations.
The employee was washing down the transfer station when he tripped over a 4"" tall concrete pad. He fell forward landing on his right shoulder and sustained a dislocation and fracture.

The employee was rolling a plate/cover for the bottom of a large pump when the bottom of the cover slipped sideways causing the top to make contact with the employees finger against a pedestal.
While pulling the loader door closed on a concrete transport, the employee lost his grip, the door swung back and smashed his right middle and ring fingers between the drum and the door. The employee lost his fingernail and it required a stitch.
Working with old cabling that could not be handled normally resulting in a laceration to hand.
While removing a piece of old grating on the rollers, a 1"" X 4"" piece of steel became lodged in the grating. Two employees propped the grating up and knocked the steel piece loose. The force of knocking the steel piece loose caused it to ricochet off the boom and struck another employee in the ribs on his right side.
The employee was investigating a foul odor in the mezzanine above the false ceiling in the pond shop lunchroom. He removed two 8x4 plywood flooring sheets to expose the ceiling tiles. While kneeling down, the employee lost his balance and fell forward through the ceiling tiles to the floor of the lunch room.

The employee stepped off a loci flat onto uneven ground and rolled his ankle, causing a sprain and distal fibular fracture
Employee was breaking casing using pipe wrench and breakout wrench, sustained a laceration to his right index finger while he was working. Employee received 9 stitches.
While cutting down dead trees, the employee was using a smaller tree (pole) to push a standing tree in the appropriate direction. The pole slipped causing the employee to fall forward into the line of fire of the chainsaw. The operator of the chainsaw shut the power down prior to the employee making contact, however the employee still sustained a laceration requiring stitches.
The employee was using a hand held grinder to remove a broken nut. He was wearing gloves and his left gloved index finger was caught in the grinder disk. He pulled his hand out of the way and received a laceration from the guard which required stitches.
While guiding a detonation line through an emulsion hose, the feeder hose became dislodged and fell under its own weight. The whipping hazard was recognized and the employee ran away from the line, tweaking his lower back in the process.
The employee was lifting rollers into a bin for disposal when he felt pain in his left shoulder. The employee received conservative treatment from the time of the incident to present. It was later determined the employee required surgery.
The employee twisted her right knee while removing her stuck boot from the mud.
On 3/21/13 the EE was physically pulling electrical cable and felt pain in his knee. He was transported to the medical clinic for evaluation. The EE was able to continue working at his regular duties. The EE saw the Dr again on 5/9/13 & an MRI was taken. It indicated a medial meniscus tear. This tear will require surgery to repair. (Turning this 1st aid into a lost time injury).
The Employee was cleaning a regrind chute to prep it for painting. In the process, he turned to come out of the working area and strained his back by aggravating a preexisting condition.

The employee was adjusting a spray nozzle for dust control, to do this, the tie-wrap needed to be cut and replaced. While cutting the tie-wrap, the hose and nozzle started to fall. The employee, using his free hand attempted to grab the hose as he cut through the tie-wrap. The momentum of his hand coming down resulted in a laceration to his left hand.
The employee scraped the face with an LHD bucket preparing the face for drilling. He exited the LHD to analyze the face for further scraping when a rock fell off the face striking his right foot beyond his boot metatarsal guard causing a fracture.
The employee looked up to monitor the crane he was using. In the process a dust particle landed in his left eye and became embedded. The EE had to have the object removed from his eye.
While mucking the employee struck the steel set causing him to jolt forward in the cab of the loader. Due to the abrupt stop, the employee struck his face causing a nasal fracture, concussion, and a contusion on his knee.
The employee moved approximately 20 bags of gel when he thought he felt something odd in his back muscles. He stood straight up to stretch his back and that is when he felt discomfort in his abdomen and notice it bulging.
The employee was inspecting the wheels and breaks on a train flat at the invert in the haulage tunnel. He stepped onto the parallel tracks to check the opposite side of the flat as an approaching train with a flat clipped and twisted the employee, causing his foot to be run over by the flat wheel.
EE was cleaning steel beams through a floor grating with a ""makeshift"" L-shape bar. He was knocking scale from a piece of plywood when the bar went through the plywood and the bar hit the grating pinching his fingers between the bar and grating.EE lost his left thumb nail and broke the first joint on his left middle finger.
Contractor was removing a 12"" vacuum line used in the process to clean HVAC unit and ducts. His right hand was pinched between the vacuum line and HVAC unit access door. The suction from the vacuum line caused his hand to be pinched against the latch on the HVAC unit access door resulting in 3 laceration on back of contractor's hand. Lacerations required stitches at hospital.
The employee was removing a metal band that was holding friction/split bolts together. As he removed the band it flipped around and a sharp edge hit him in the right wrist resulting in a 1"" laceration that required stitches.
The employee was removing grating using an overhead remote control 15 ton capacity crane. The employee was distracted while lowering the crane hook striking his left wrist resulting in a sprain. Employee was required to wear a wrist splint and was given modified duty for lifting, pushing, pulling and carrying a maximum weight of 5lbs.
Employee was leaving the LOCI shop building through the north mandoor. After exiting the mandoor she slipped and fell. Employee was carrying trash bags full of trash from the LOCI shop building. Employee had on rubber steel toed slip-ons over her shoes. Another employee witnessed the incident.
The employee was attempting to remove a large piece of liner steel from the belt to drop it into the steel bin next to the belt. In the process of trying to maneuver the steel by hand, the st eel rolled crushing the tips of three fingers on his right hand. This resulted in an amputation of the tips of his three fingers.

The employee was descending a set of stairs and caught his foot in a hose that was rolled up and hanging on the outside of the steps at the bottom. He lost his balance and in the process of trying to regain it, he twisted his right knee which resulted in a sprain.
The employee was hammering set concrete on the grating of a hopper. The hammer head slipped through the grating, pinching the employee's small right finger in between the hammer handle and the grating. This resulted in a hairline fracture and small laceration requiring stitches.
Employee was loading a face round as he reached up and out, he slipped and fell forward catching himself with his left hand, this resulted in dislocation to his left shoulder.
Roof fall 7065 Truck loop BC and 92C. Corrosion of split sets is likely a major factor. Rust stains were observed on rocks near bolts. Area was reported to be intermittently wet since mining and installation of initial ground support in late 2005 / early 2006. K order was issued and a repair plan was put together and approved through for mitigation.
Employee was trying to engage parking break and put his finger in a pinch point near the service break causing a laceration to the right middle finger.
Blasting activities in the undercut level precipitated a ground fall at the 7210-P85/PEHW intersection. The fall was bounded on the hanging wall side of the intersection by a molybdenum slip fault running slightly askew to the PEHW drift, and daylighted on the hanging wall side. A repair plan for remediation has been developed and implemented.
Employee descending challenger powder buggy, holding on to the handrails, when employee stepped down with left foot he stepped on uneven ground causing his left ankle to roll outward resulting in a pop and strain to the ankle.
Employee had climbed down off a kamatsu excavator after filling it with oil as he turned he felt pain in his lower back.

The employee went to get a section of vent bag and as he was loading it onto the buggie, he pulled on the vent bag and it got caught. When the employee pulled on the vent bag again, his back popped right below his shoulder blade.
While removing a bearing from a floatation agitator with a dead blow hammer, the bearing broke free and struck the employee in the back of his right hand. It caused a laceration, approximately 1/2 inch in length. It was cleaned and bandaged, however, it became infected. The employee was sent to the clinic, had the wound cleaned, which lead to the discovery of a damaged tendon.
Silicosis of the lungs due to dust exposure.
Employee was checking the level of emulsions in an emulsion pump, the pump has 2 split doors that lay on top of flat on top of the pump. Both doors open to a 90% angle to the pump. One of the doors came down and pinched the employee's small finger of his left hand resulting in a laceration to the tip of his small finger. The laceration required 6 stitches.
Miner was walking to equipment shop and slipped on ice and sprained his right knee.

Employee was removing wire matting from the belt of a stopped conveyor when he pulled on the matting that was wedged in the muck, it came free and he lost his balance and fell on his right side resulting in a fracture to his 10th rib.

While installing a 48"" diameter fiberglass 'Y' for a ventilation pipe, an employee was pressed against the railing of a personnel lift and suffered a hairline fracture to the #9 rib on the right side.
Mechanic was grinding and cleaning a weld in a float cell tank, at some point during this activity he realized that he had gotten somthing in his eye.
While removing failed starter from 510917, employee lifted nose of starter using the first three fingers on his right hand. Employee noticed a popping sound/feeling in his ring finger.
Employee was removing an assay from oven and brushed left arm against door to oven.
Employee suffered fractures and lacerations when operating a tugger while handling the cable.
After completing investigation and receiving diagnosis from doctor it was determined that the employee may have suffered a repetitive motion illness from working in a warehouse environment.

Employee was exiting the warehouse at the end of shift out the Northwest man door and down the steps. It is unkown if she had her hand on the handrail. When she took her last step to the ground her foot hit the seam edge of the asphalt surface which drops about 1"""" and rolled her left ankle.
Employee reached from cab of mobile equipment to shut off a water spray valve. The mobile equipment moved, and pinched operator's forearm between rib, door, and door frame. This resulted in fractures and lost workdays.
Employee slipped on ice during a snowstorm resulting in a fractured knee and lost time.
Employee was getting out of personal vehicle in employee parking lot prior to his shift. The parking lot was icy due to an early winter storm. As he stepped out, he slipped and fell on his tailbone. The following day (9/23) he was sore and was sent for medical evaluation. He was given work restrictions that resulted in temporary reassignment.
EE was removing a 6' conveyor belt clamp with another ee. The partner disconnected the jaws on his side first, and dropped the bottom plank. The injured ee's side was not disconnected, so as the bottom plank fell, the jaws on his side acted as a fulcrum. The planks extended past the jaws by about 1"", and the ee's thumb was between. His thumb was pinched.
Employee was attaching a choker to a mill liner, while a second employee tipped liner back, using mill bar to access bolt hole. Bar slipped, allowing liner to tip over, landing on employee's hand and causing a fracture.

An employee entered a surface elevator to get to the top floor of the SC1 Drive House. The elevator stopped functioning and the employee riding the elevator was entrapped inside. he was there for approximately 90 minutes from 9:00AM to 10:30AM.
Employee helping to trip out drill steel on raisebore drill, while clamping the break-out tool (clamp) to the steel string, employee got his R middle finger pinched between the 2 parts of the tool when the tool slipped out of position to set the bolt that holds the tool parts together. Resulting in section of the finger tip and finger nail being severed from the finger.
Employee was carrying a 50 lb object and stepped off a 7"" curb. When he stepped off, his foot was not in full view and he fell with his entire weight onto the right ankle, resulting in a fractured fibula. Employee was unable to work the next scheduled shift.
Employee was removing goggles, which had wet, sandy material on them. A small amount of material fell into employee's eye. On-site medics rinsed eye, but irritation persisted. Off-site physician diagnosed a scratched cornea and gave a prescription for eye medication.

The employee was using a Die grinder to widen holes in a truck bed liner floor plate. The employee was bringing the tool up out of the hole when the grinder slid across the liner plate and hit his left knee causing a small puncture wound. He was treated and initially returned to full duty. At the present time the employee is under observation of a physician for infection.
While performing an emissions test under a pipe truck, the engine was revving and it slipped into gear rolling forward and spinning him under the truck.

Electrician was pulling down on strap to close elevator door with his right hand. His left hand was on the bottom of the top half of the elevator cage door when the bottom half of the elevator door came up and hit his left hand. He received a broken middle finger on his left hand.
Employee was using a pry bar to help pry a pilot bit off of drill rod. He had one hand on the pry bar and the other on the hood handle of a tractor. He was doing this to try and gain more leverage. As he was prying a rock fell out of the back and hit his left hand. This caused a laceration to the 5th left digit. As a result the employee recieved six sutures to the finger.
Employee pinched finger between forklift tines and basket rest. Fracture to middle finger on right hand.
The employee was trying to install a retaining pin in a concrete form. When he tried to get the pin in, the form came loose and pinched his finger in between the form pieces. The intial assessment of the finger was that it was bruised and had a small cut on it. It has been later found that his finger tip has a small hairline fracture in it.
Employee was riding in buggy returning to the elevator shaft. Buggy pulled to the side to allow equipment to pass. Driver released the brake to pull forward, the buggy drifted backwards, smashing the employees hand between the rail of the buggy and the rib.
While driving the mucker to the ore pass the employee hit the rib. The mucker stopped suddenly forcing him to lean forward in his seat. He hit his head on a mounting bracket at the top of the door causing a contusion on top of his head.
EE were removing a 1600AMP breaker from a load center that had been shutdown for cleaning.One EE was on each side of the breaker and one EE in the front of the breaker.As the breaker was being removed it shifted and piched the injured employee's finger between the breaker and the breaker rail that the breaker rests on when racked into the load center.
Pulling a rope on a generator, the right forearm hit the bottom of a trailer and cut the forearm.
While trying to unplug the acid dilution tank, the hot water hose came loose from its fitting spraying the employee with water.
Mechanic was lying down. He started to stand and felt a pop in his right knee.

Two employees were changing worn skirt liners in the PC1/PC2 transfer station. As they were handing the liners down from one to the other, the lower employee had one and lowered it to his thigh. As he was lowering it to his thigh, he dropped the liner and it fell on his foot.
The employee was drilling face rounds in P71. After scaling the face, hand mucking, and painting the bootlegs, he started aligning lines for drilling. He moved to drill the north side. The employee was still under supported ground. While the employee was sighting down the boom with his back to 603, a loose rock fell out and hit the employee in the shoulder.

There were 2 employees working together to lower an 8 inch X 20 Foot pipe with a boom mucker. The injured employee was guiding the pipe as they were lowering it to the ground. The IE had his left hand on the pipe and the boom truck lowered the pipe at a rate faster than anticipated and it pinched his finger between the pipe and the clevis.
The employee had been sweeping up hydraulic fluid using floor dry. He was depositing the used floor dry in a cardboard box. He lifted the box to put it in the dumpster. As he was throwing the box away, he felt a pop in his left shoulder. The result of his injury was a shoulder strain.
EE was installing a collar pipe in lifter holes, when he bent over to install collar pipe to prevent filling lifter holes, he was struck upon the back of the head/neck/shoulders by a piece of loose rock. Drill Jumbo was being utilized at the time as the method of drilling. Resulted in a laceration to back of head, abrasions and contusions to neck and shoulders.
The employee was clearing a jam in the #1 ball feeder. She was removing 4"" balls to get to a 5"" ball that had been misfed when some of the 4"" balls rolled in to the feeder and struck her on the fingers. She received a crack fracture on the tip of her right ring finger. She was not given any restrictions.
The employee was using a slide hammer and caught his palm between the slide and the rod pinching his palm. The employee was not given restrictions but was given a splint.

The employee was closing the front loading door on 320275 transport truck. He used his left hand to grab the door and when the door came down it struck the pipe wrench that was in his right hand pinching his right middle finger between the wrench and the railing. The incident has broken his finger.
