Struck against a moving object

Employee was operating a Haul truck when EE went over a mound of material at approximately 30mph. EE received friction burns and a lower back strain.

Over-exertion in wielding or throwing objects

Employee was using a bar to clear a sand hopper. Five days later, employee said elbow was hurting. Employee reported the injury on 12/20/2016.

Absorption of radiations, caustics, toxic and noxious substances

Injured miner received bee stings when a nearby dozer ran over a box containing a bee hive. The hive swarmed around the injured miner and stung EE on head and neck. EE was treated at the company clinic and released to full duty.

Employee was removing bolts from crusher top shell with air gun and caught his right thumb between the air gun handle and top shell of the crusher.

Employee was installing rubber screens with a slide hammer in the middle deck of a screen. When he was hitting one screen in the bottom of the hammer slipped off the panel and causing the whole slide hammer to fall suddenly and smash his finger between the handle of the hammer and the bucker bar rail on the top deck.

Employee was digging out the # 1 three inch feeder with a digging bar, a loader was digging out # 2 feeder when material on the northwest edge of feeder #1 sloughed off and caught his legs causing a right knee strain.

Employee was operating back hoe moving raw material in front of pit feeder for block wall placement when he felt tightness in his back from bouncing in the seat of the equipment while operating it.

Contact with hot objects or substances

Employee was overhead welding a cross brace in the wet screen when hot slag dripped into the gap of his long sleeve shirt where it buttons at the wrist and it gave him 2 small burns on his left elbow. The small burns developed into an infection which resulted in lost time because employee did not report the incident to his supervisor until 4/14/2009

While the employee was grinding slag off a piece of angle iron a gust of wind blew a small piece of grinding under the employee's face shield and safety glasses into his upper eye lid.

Employee was checking baghouse #2. When he stepped off the slab, he sank in the mud which appeared to be hard ground. His right leg sank about 14"". Employee started to twist and turn before he realized his footing was unstable.

While cleaning up a construction site the employee was lifting a 30 lbs piece of metal when he felt a strain in his low back.

While holding the ladder for a co-worker, a large slab of metal the co-worker was working with slipped and fell striking the injured employee.

Employee was removing sand stone that had lodged on a conveyor belt. When the conveyor belt rocked, a stone fell off the belt landing on the employee's left ankle.

The employee was repairing a hole in Chute C-9 while using an extendable ladder. The legs on the ladder he was working on slipped out causing the employee to fall. He struck his leg on the tailguard.

Over-exertion in lifting objects

The employee was moving screen panels on the deck of S-1. He was holding a flashlight in one hand; lifting one end of the screen with the other hand. His elbow made a popping sound, then became painful causing the employee to drop the screen panel he was carrying.

Struck against stationary object

While removing a rock from a conveyer belt with a shovel handle, the employee was struck on the left hand thumb with the other end of the shovel. The left thumb then struck the side of the conveyer.
