EE flip a manual gate to chute EE opened the metal door and secured the safety chain; EE bumped the metal door causing the chain that secures the door to come loose, the door fell back down and smashed the employee's finger The EE was wearing safety gloves when incident occurred Employee's finger index nail was smashed and employee sustained a fraction finger

Employee was operating the Komatsu WA900 wheel loader when employee applied the service brakes, employee felt a sharp pain in lower back.

Employee was moving a 40' container with a boom forklift, during the process the forklift over turned causing the employee to hit the right side of their head on the window. Employee sustained a small laceration, receiving five staples.

Employee had completed his lunch and was returning to the water truck when he attempted to step over one of the indents designated as equipment ""ready line"" at which point he lost his footing, slipped and fell spraining his right knee.

The miner was performing the routine task of cleaning around a screen support structure when he inadvertently stepped off of the concrete foundation to the surrounding grade; twisting his knee.

The miner was utilizing a utility knife to notch out a piece of skirt board rubber. The miner was first using a dull blade. He changed the blade, began cutting towards his left hand while using his right. The knife jumped and cut his index finger and palm of his left hand needing 4 stitches in said index finger. Gloves were not being worn.

As employee was removing a wear plate from the bucket of the Komatsu WA900 loader with a sledge hammer a piece of metal chipped off and embedded into his left inner thigh.

Fall from headframe, derrick or tower

Inappropriate personal fall protection, he tied off in a unsafe area, caused employee to fall while taking down a surge bunker off the side of a wet tower. Injuries were a small laceration to the back of his head and a broken right forearm.

As EE was operating his loader, he drove over some rough terrain, and injured his back.

The laborer was trying to secure a large rock inside the bucket of a skid steer when the operator of the skid steer accidentally dropped the bucket on the laborers foot. The extent of the injury is not known at this time.

Employee was in a chute taking measurements and cut his right hand on a piece of sharp metal protruding inside the chute.

Struck by falling object

Employee was trying to put back a steel cover over a conveyor and the cover got stuck. While trying to push the cover down, he got his right thumb in the way; the cover dropped down and smashed his thumb. Employee fractured his right thumb and also received 15 stitches.

Employee was walking down a catwalk,tripped and fell injuring his right wrist. *Employee did not report this injury until 2/13/06 which is when it became reportable.*

EMPLOYEE WAS HELPING SPLICE A BELT WHEN HE SPRAINED HIS RT. WRIST. EE WAS PULLING ON THE BELT WITH BOTH HANDS & JUST SPRAINED HIS WRIST.

The empoyee was cutting through a waterpump hose with a razor knife in a tunnel and cut his left thumb in the process. The employee went to the clinic and was given five stitches in his left thumb.

OUR EMPLOYEE WAS DRIVING HIS LOADER TRUCK TO OUR MECHANIC SHOP ON OUR FIELD AND WHEN HE STEPPED OUT OF HIS TRUCK HE STEPPED ONTO UNEVEN GROUND AND AND HIS FOOT TWISTED, HE THEN REACHED HIS LEFT ARM OUT TO CATCH HIMSELF AND HURT HIS LEFT SHOULDER IN THE PROCESS.

EE AND OTHER EE WERE SHOVELING IN THE #3 ROCK TUNNEL WHEN EE SLIPPED AND FELL HITTING HEAD AGAINST THE CONVEYOR STRUCTURE. EE HAD A CUT ON NOSE AND NOSE WAS BLOODY. EE WAS DISORIENTED, AND 911 WAS CALLED.

EE MISSED THE SECOND STEP COMING DOWN HIS LOADER AND FELL OFF TO THE GROUND, ONTO HIS BACK.

HE WAS CLIMBING UP THE LADDER AND HIS FOOT SLIPPED OFF THE STEP AND HE LOST HIS GRIP AND FELL TO THE GROUND APPROX. 7' AND LANDED ON HIS RIGHT FOOT.

ee was helping in maintenance to remove a worn out pulley. ee tood the bolts out & when he went to push the pulley out, it slid down & smashed his left index finger.

Ee was operating the 777 & hit a pot hole and the seat suspension was not operating properly and hurt his back. EE told us about this on the 30th of Oct that he hurt his back not on the 18th.

CLIMBING DOWN OFF THE 992 LOADER BEHIND MINE OFFICE TO USE RESTROOM, EMPLOYEE STEPPED ON A ROCK AND TWISTED HIS ANKLE.
