Contact with heat

Repairman reported experiencing symptoms of heat illness. Emergency Medical Services were dispatched to assess the employee. At the employee's request, the employee was transported to the emergency room for further evaluation where employee was placed off work for 1 day.

Over-exertion (Not Elsewhere Classified)

On 7/10/24, operator reported that turning the knobs on the loadout system caused a cyst on EE's right index finger over a span of two days. The medical evaluation diagnosed EE with a ganglion cyst of the right finger. The employee was cleared to full duty with no restrictions. However, during a recent appointment on 8/20/24, the employee was referred for physical therapy.

Symptoms experienced were dizziness and blurred vision. EMS was summoned for precautionary assessment due to personal health condition. EMS decided to transport employee to hospital for evaluation. EE remains off work. The MSHA Investigator requested an MSHA 7000-1 be completed indicating the illness was non-work related.

Contact with hot objects or substances

On August 4, 2020 the employee struck a welding rod on the top side of EE's right hand in the thumb area causing an arching burn. The employee was returned to work without restrictions. On November 9, 2020, the employee was prescribed work restrictions following surgery to repair nerve damage.

On 9/14/15 the employee was taken to the Company clinic, received stitches for a left eye orbit laceration and was released to regular work on 9/15/15. The employee was struck in the orbit of his left eye by a drag cable during an installation and sustained a laceration.

On 4/02/14, the employee requested medical treatment and was placed on modified duty which was accommodated. A conveyor belt flopped loose during a repair and made contact with the employee's hard hat. The Company physician diagnosed the employee with a head contusion and cervical spine strain.

The employee went to the company clinic on 8/1/11 and was placed on restricted duty. The employee reported that he lost his footing, fell and struck his R-knee cap on a protruding 5/8"" bolt that secured the dust pan.

The employee went to the company clinic on 11/14/09 and was placed on restricted duty. The employee was using a putty knife to remove a decal, when the knife tore through the decal and struck L-palm, causing a laceration that required sutures.
