Injured employee was using a grinder to remove bolts in a unit. The grinder handle was preventing EE's reach from bolt and EE decided to remove the handle. Without a strong securement handle, the grinder kicked back and made contact with the employee's left middle finger, cutting through a glove. This resulted in a laceration requiring 3 stitches.

Operating light vehicle and struck a bollard resulting in a broken right radius.

Injured employee alleges knee pain when stepping on to grating that was on the ground in a laydown yard. The grating was placed on the ground to prevent slip hazard in muddy conditions.

Operator used air pressure to clean a line and opened valve before bleeding pressure. Air pressure burst a catch trap and oxalic acid with debris hit operator's face knocking safety glasses off.

EE was stepping over a small containment wall. They stepped over with left foot and stepped on a grading that was to large for the hole it was covering. It caused the grading to flip and the EE fell into the hole and ripped their pants and cut their right upper knee on the grading.

Employee entered a sump that had what they believed to be only water to install a pump. The sump had residual caustic in the water. The employee did not put on their chemical resistant boots and water topped their boots causing chemical burns to their feet.

Removing bolts from a differential using a Hytorc and wrench. The wrench slipped off the bolt and struck the worker in the safety glasses. The safety glasses broke and caused a laceration to the right eyebrow. Worker received 10 sutures.

IW was cleaning preheater tubes using a hydroblaster flex lance. As they pulled the flex lance out of the tube, they mistakenly held pressure on the bypass (foot pedal), causing the lance to continue spraying. The whip hit them in the bicep, resulting in an injury.

Employee was stepping from a concrete pad to a gravel surface, slipped and fell striking their knee on the ground.

EE walked behind the truck for setup. During setup, co-worker extended the rear hydraulic bumper striking EE and pinching EE between the bumper and concrete wall. EE did not feel as if had received any major injuries but pain increased when bearing weight on right leg. EE was then transported to a medical facility for further evaluation.

Crew members were walking to the lunch tent due to a weather stand-down due to high winds. While walking behind another person, the wind blew that person's hard hat off which struck the injured employee in the mouth resulting in a chipped tooth.

Employee was doing pre start inspection at the crusher on C10-CV01 tail pulley and guards. Employee stepped back and turned to their right when their ankle rolled and fell to the ground. Employee was assisted by co worker and lead person.

Employee was holding the threader against the conduit to start the threading process. When the employee let off the trigger to readjust how they were holding the threader, the threader kept running spinning around the conduit. Employee attempted to unplug threader when the threader hit them in the leg causing fall to the floor landing on shoulder.

Employee was conducting maintenance work. When EE moved hand away from the part, EE received a small cut to finger. Employee reported minor incident to the supervisor. On July 9, 2021, employee informed supervisor that the finger had become infected and employee was sent to occupational medical provider for treatment.

Employee was walking on elevated floor grading and did not see a portion was removed for maintenance. Employee fell approximately 2 feet to lower level before they caught themselves. When falling, their right foot struck a storage vessel.

Driller was lifting a drilling rod/steel into position when the top of the rod/steel contacted a brace on the drill mast which caused the bottom of the rod to swing outward striking the miner's finger causing a dislocation injury.

Struck by falling object

While removing rigging from a main I beam located above the CHP steam header, a 3T beam clamp weighing ~25 lbs. fell 10-15 feet and ricocheted off a cable tray into an employee's left forearm. This resulted in bruising and restricted mobility of employee's fingers.

Operator was heading out to do samples when stepping into some mud inside the containment of sump 3. This caused them to slip and twist right ankle and fall to the floor. This incident happened by pump 32 inside the sump 3 containment.

PER MSHA DEFINITION, FIRST AID INJURY ONLY UNTIL 07/01/2020. Employee was attempting to avoid falling 30lb fire extinguisher so jerked leg backwards. However, foot stayed planted and knee was hyperextended.

Prep Lab Technician was grinding blast holes and inadvertently placed a prepared ring and puck bowl on the upper ledge of the ventilation hood which was attached to the work bench. The ring and bowl fell from the shelf and impacted employee's left thumb.

Employee smashed right middle finger in door while lifting door to reinstall on vehicle.

Supervisor checked on progress of a welding project. After speaking with welders for about ten minutes and assuming the metal was cooled, the supervisor placed hand on the metal and received second degree burns to left fingers and palms.

Employee used hammer to free impeller motor from a vertical pump. Hitting the impeller, a piece of metal broke off & entered employee's hand. Employee treated onsite by contract medical. On 2.25.19, employee notified employer that wound not healed correctly. Employee referred to offsite medical facility where a metal fragment was found embedded under the skin.

Fall down stairs

Employee exited the office trailer going down the stairs onto the concrete pad. Due to icy conditions, employee lost footing and fell. Employee suffered a scraped knee.

Employee was removing tail stock from the lathe which required strenuous pushing and pulling. Employee felt a twinge of pain in groin area but did not report until 09.22.18. On 9.22 employee declined need for medical care. On 10.22.2018 employee advised that they were still experiencing pain and was referred for professional medical care.

On 9/15/18 employee reported blurred vision and eye irritation. Employee was packaging Mill Concentrate when some dust got into EE's eye.

On 09/07/2018 employee reported that they stepped on pillar to avoid getting wet while testing eyewash station. Employee slipped off the pillar and hit their chin on the eyewash bowl resulting in a laceration to the chin which required 4 sutures.

Employee was cleaning liquid floc from floor by using hose to push the liquid flock into the floor sump when ee slipped and fell on left shoulder. The floc is both wet and slick.

Employee #1 standing on platform pushing material through hopper for packaging. Employee #1 encountered a rock and tossed it over the edge of the hopper. Employee #2 was rounding the corner near the hopper and was struck in the head by the rock.

Komatsu grader parked on ready line in a ditch with door opened completely to the left. Employee grabbed door handle with left hand and braced right hand at the door frame. Employee had to use force to shut the door because of the angle. Door impacted right index and right middle fingers.

Employee was on top of the pla removed cloth & walked backwards tripped on a head metal brackets & fell backwards into the gap left from removed plates. Employee fell aprox 6 feet & restrained by EE's fall restraint system from contacting floor. Medical treatment not initially sought. On 4.20.18, employee requested & rec'd medical

At the P7 packaging area, the employee was coming down the ladder (facing away from the ladder) as they descended they twisted around facing the ladder missing the bottom step causing them to trip and stumble twisting an ankle.

On 2/14/2018 employee reported that on 2/10/2018 employee was operating a skidsteer while packaging mill concentrate and a gust of wind blew the concentrate in left eye. Employee was wearing a half-mask respirator and safety glasses. Employee then switched to a full-face mask and completed shift. Upon return to work, employee eye was red and irritated.

Employee was attempting to remove a 2"" nipple that was frozen into a valve with two pipe wrenches. He was using great force and felt pain in his abdomen. Was not a reportable incident at the time but now is scheduled for surgery to repair umbilical hernia.

EE was pulling a 3 conductor number 6 cable with eight other miners, the pull was 450 feet. Men were spaced about 50 feet apart in cable tray. The pull was on 12/8/2012 and the injury was reported on 12/10/12. The injury was to the left shoulder.

A plant maintenance inspector was inspecting the wear liners in a ball mill when his foot sunk into a soft spot of mud and he was unable to get it unstuck without assistance. No injury or exposure occurred to the individual however this was called in out of an abundance of caution.

Employee using a tool called (Klever Kutter) to remove electrical tape that was used to hold together 3 wires being pulled through a conduit. Employee was cutting the tape towards his left hand and the cutter slipped and caught his glove, cut the glove and consequently cut the top of his hand between the left forefinger and thumb area.

Employee was cleaning up at end of shift, moved gangbox out of way, felt a 'pop' in right arm and chose not to seek medical treatment. Worker returned to work 8/14/12 and requested medical treatment and was diagnosed with right bicep rupture.

Injured Ironworker started to finish tying column dowels to rebar mat. Ironworker cut rebar loose and accidentally dropped one and it fell hitting injured in the back causing a 1 1/2"" laceration to the right rear flank.
