Fall from machine

On 4/19/17, employee requested medical treatment, was placed on restricted duty and was prescribed physical therapy. The employee did not use safe means of access on a screen and fell 3 feet. The physician diagnosed a L-hip sprain and contusion in addition to lumbar radiculopathy.

On 8/2/16, the employee requested medical treatment, was taken to the company clinic, diagnosed with a L-ankle sprain, was returned to full duty and was prescribed physical therapy. The employee alleged that they rolled their left ankle while walking towards the back of the quality control lab.

On 7/13/15, the employee was treated at an emergency room, was diagnosed with a 2nd degree burn to the left wrist and was placed on restricted duty which is being accommodated. The employee lost control of a torch and subsequently burned his left wrist.

This is on injury that developed after a non-reportable fall 11-16-04 where the person caught his right foot in a tail pulley handle guard when he slipped and fell and at that time did not have injury and went back to work. his right foot toe dislocated now.

PUSHED THE CRACKED CERAMIC RESERVOIR TOILET TOP WITH THE BACK OF HIS LEFT HAND & HIS MIDDLE FINGER WAS CUT BY THE CERAMIC.

WORKER WAS PUTTING THE LEFT FRONT FENDER ON A 980-G CAT LOADER. HE PROCEEDED TO CLIMB DOWN FORM THE FENDER ON THE INSIDE OF THE TIRE AND CAUGHT HIS RIGHT FOOT BETWEEN THE TIRE & THE LOADER FRAME. HE LOST HIS FOOTING AND TURNED SIDEWAYS KNOCKING HIS HARD HAT OFF AND FELL HEAD FIRST INTO THE CORNER OF THE BACK OF THE LOADER BUCKET PUTTING A CUT ON TOP OF HIS HEAD.
