On 1/5/2026, management was notified of an alleged injury. The employee reported back pain as a result of operating the water truck.

Employee was operating a loader on the haul road when they drove over a 3"" pothole. Employee states this cause their teeth to jar and lose consciousness for a second. After experiencing nausea, emergency medical services were dispatched to the site and transported the employee to an emergency room, where they was diagnosed with a head contusion.

Employee alleges a lower back strain due to shoveling. Employee self procured medical treatment at a local hospital where EE was prescribed 600mg of ibuprofen.

Employee alleges experiencing pain to EE's lower back after operating a haul truck, employee claims to have struck a sinkhole on the haul truck road which EE believed to have contributed to EE's injury.

Clear sunny day. Field Tech was stepping down from the bed of the service truck and lost footing. Body was then shifted and impacted an umbrella holder mounted to the truck, they then fell to the ground on left side and impacted rib cage

Employee exited the excavator EE was operating. Upon exiting EE stepped onto an unstable piece of concrete which caused EE to lose balance, twisting ankle and falling over.

Employee was replacing a transmission when the transmission slipped off the jack, and pinched employee's right little finger between the jack and the transmission. The employee received stitches and was diagnosed with a partial traumatic transphalangeal amputation of right little finger.

Employee was installing ground engagement parts for the bucket of a 980H loader when a piece of metal punctured EE's lower left leg. The employee was treated at the clinic where EE received stitches.

On May 31, 2018, EE requested a medical evaluation to be done the following day, June 1, 2018, and was prescribed physical therapy. On 3/6/18 EE was standing by a doorway when another employee, opened the door and struck EE in the shoulder area to punch in to go to work. EE was diagnosed with impingement of right shoulder.

On June 15, 2017 received report from audiologist that the employee's right ear hearing impairment has been caused or contributed by workplace noise.

The employee requested medical treatment on 11/17/14, was taken to the company clinic and was placed on restricted duty which is being accommodated. The employee alleges pain to the lower back as a result of lifting a fabricated stair case (approx. 80 pounds) with a co-worker. The employee was diagnosed with a lumbar strain.

On 1-9-14 the employee was taken to the company clinic and received stitches for an open wound to R-Index finger which was pinched between a sheave and sheave housing during an inspection. Employee released to full duty on 1-10-14.

Past occupational noise exposures can not be ruled out as a possible cause for the noise induced hearing loss that has been recorded. Current occupational noise exposures are within safe levels.

On 03/27/2010, the employee was taken to the company clinic and had a foreign object removed from his right eye by a physician. The employee reported while welding, wind blew a foreign object into his right eye. The employee was returned to full duty.

The employee was taken to the company clinic on 4/16/09 and was placed on restricted duty. The employee is alleging a continuous trauma from 10/2/07 - 10/2/08 to his back and lower extremities from continuous and repetitive motion. Employee, on 2/9/09 originally reported his back injury as a non-industrial specific injury that occurred at home.

The employee went to the company clinic on 8/24/2007 and was placed on restricted duty. The employee was bitten on back of right knee by a spider while refilling the water truck or while in the cab of the water truck.

The employee went to see the company doctor on 7/5/2006 and was placed on restricted duty. Installing underground electrical conduits at the San Bernardino plant resulted in a rash to multiple body parts (ALLEGED)

The employee went to the company clinic on 5/1/2006 and was placed on restricted duty. The employee allegedly strained his lombar spine while shoveling materials, resulting in a herniated disc. (ALLEGED)

The employee went to see the company doctor on 9/6/2005 and was placed on restricted duty. The employee cut his R-forearm with a utility knife while cutting rubber, causing a laceration that required sutures.

Employee went to see company doctor on 8/17/04 and was placed on restricted duty. Employee opened hood of yard truck for a pre-trip inspection prior to operating vehicle. Hood went past normal stopping point because safety catch was broken; employee was attempting to hold hood with his left arm straining lower back and rib cage.

WHILE CUTTING OFF THE BEARING SUPPORT ARM WITH A TORCH, SUPPORT ARM FELL ONTO EE'S L-KNEE WHICH CAUSED A CONTUSION & LACERATION TO L-KNEE. EE WENT TO SEE COMPANY DR ON 3/12/02 & WAS PLACED ON RESTRICTED DUTY.

EE WENT TO SEE TO CO. DR. ON 7-12-01 AND WAS PLACED ON RESTRICTED DUTY. EE WAS CUTTING A LIVE RUBBER BELT INSIDE THE WET SIDE MAIN FEED CHUTE WITH A RAZOR UTILITY KNIFE CUTTING TOWARDS HIM, IN LIMITED SPACE, AND CUT HIS RT. KNEE.

EMPLOYEE WENT TO SEE A DOCTOR ON 11-16-00 AND WAS PLACED ON RESTRICTED DUTY. WHILE IN THE RSTROOM, THE EMPLOYEE WAS BITTEN BY AN INSECT IN THE LEFT THIGH.

EMPLOYEE WENT TO SEE THE DOCTOR ON 9-26-00 AND WAS PLACED ON RESTRICTED DUTY. EMPLOYEE WAS PUTTING WHEEL CHOCK BACK ON THE TRUCK WHEN IT SLIPPEDAND STRUCK HER IN THE RIGHT SHOULDER.

EE WENT TO SEE THE DR. ON 9/7/00 AND WAS PLACED ON TEMPORARY DISABILITY. WHILE LOADING TRUCKS, EE DROVE THROUGH TWO CONSECUTIVE POT HOLES AND INJURED LOWER BACK.

EMPLOYEE WENT TO SEE THE DOCTOR ON 8-31-00 WHO REMOVED A FOREIGN OBJECT EMBEDDED IN RIGHT EYE. WHILE WELDING, A CHIP OF HOT, DRIED PAINT ENTERED INTO EMPLOYEE'S RIGHT EYE UNDERNEATH HIS CUTTI NG GLASSES.

EE WAS SHOVELING MATERIAL FROM UNDER CONVEYOR BELT ONTO CONVEYOR BELT. HIS SHOVEL HT A ROLLER. THE SHOVEL KNICKED BACK AND HIT EE ON HIS RIGHT CHEEK. THE BLOW CUT HIS CHEEK. EE RECEIVED 19 STITCHES.

EE WENT TO SEE THE DR ON 4/12 AND WAS PLACED ON RESTRICTED WORK DUTY. WHILE GETTING OUT TO LOAD TRUCK, LEFT FOOT SLIPPED OFF BOTTOM STEP, EE FELL AND LANDED ON RIGHT THIGH AND RIGHT HAND.

EE WENT TO SEE THE DR ON 4/10 AND WAS PLACED ON RESTRICTED WORK DUTY. WHILE ATTEMPTING TO OPEN BUNKER, PUSHBAR JAMMED AND IT JARRED EE'S RIGHT WRIST.

EE WENT TO SEE DR. AND INITIALLY WAS RETURNED TO FULL DUTY. ON 7-18-00 EE STARTED TO LOSE TIME, CAME BACK ON RESTRICTED ON 11-30-00 AND IS STILL ON RESTRICTED. EMPLOYEE'S BOOT WAS CAUGHT BETWE EN CAB AND STEP OF TRUCK WHICH CAUSED STRAIN TO HER LEFT KNEE.
