Struck against a moving object

On 1/5/2026, management was notified of an alleged injury. The employee reported back pain as a result of operating the water truck.

Employee was operating a loader on the haul road when they drove over a 3"" pothole. Employee states this cause their teeth to jar and lose consciousness for a second. After experiencing nausea, emergency medical services were dispatched to the site and transported the employee to an emergency room, where they was diagnosed with a head contusion.

Over-exertion in lifting objects

Employee alleges a lower back strain due to shoveling. Employee self procured medical treatment at a local hospital where EE was prescribed 600mg of ibuprofen.

Employee alleges experiencing pain to EE's lower back after operating a haul truck, employee claims to have struck a sinkhole on the haul truck road which EE believed to have contributed to EE's injury.

Fall from machine

Clear sunny day. Field Tech was stepping down from the bed of the service truck and lost footing. Body was then shifted and impacted an umbrella holder mounted to the truck, they then fell to the ground on left side and impacted rib cage

Employee exited the excavator EE was operating. Upon exiting EE stepped onto an unstable piece of concrete which caused EE to lose balance, twisting ankle and falling over.

Employee was replacing a transmission when the transmission slipped off the jack, and pinched employee's right little finger between the jack and the transmission. The employee received stitches and was diagnosed with a partial traumatic transphalangeal amputation of right little finger.

Employee was installing ground engagement parts for the bucket of a 980H loader when a piece of metal punctured EE's lower left leg. The employee was treated at the clinic where EE received stitches.

On May 31, 2018, EE requested a medical evaluation to be done the following day, June 1, 2018, and was prescribed physical therapy. On 3/6/18 EE was standing by a doorway when another employee, opened the door and struck EE in the shoulder area to punch in to go to work. EE was diagnosed with impingement of right shoulder.

The employee requested medical treatment on 11/17/14, was taken to the company clinic and was placed on restricted duty which is being accommodated. The employee alleges pain to the lower back as a result of lifting a fabricated stair case (approx. 80 pounds) with a co-worker. The employee was diagnosed with a lumbar strain.

On 1-9-14 the employee was taken to the company clinic and received stitches for an open wound to R-Index finger which was pinched between a sheave and sheave housing during an inspection. Employee released to full duty on 1-10-14.

On 03/27/2010, the employee was taken to the company clinic and had a foreign object removed from his right eye by a physician. The employee reported while welding, wind blew a foreign object into his right eye. The employee was returned to full duty.
