Improper placement of hands/tool usage. The pry bar being used slipped and pinned the operator's hand between the pry bar and the pump causing a laceration to the left thumb.

Mechanic was in the process of changing the teeth on an excavator bucket. The teeth were difficult to remove and required the use of a sledge hammer to work them loose. While hammering the one of the teeth loose, a small shard of metal broke away on impact and imbedded into employee's left leg below the knee.

EE was finishing a conveyor belt repair & was climbing off the conveyor when left foot got caught by the water hose that runs along the frame of the conveyor causing EE to trip & EE landed on right knee. EE reported the incident to management, first aid was administered and the employee was taken to clinic for evaluation. EE was placed on modified duty.

Employee was working in the tunnel around the surge feeder on the platform and had left the trap door open. EE was spraying down the feeder with water when EE had sprayed self in the face and stepped back into the access point falling less than 6' onto the dirt below. Employee was chipping out build up inside of cone crusher, accumlated debris fell on employee causing him to sprain neck.

One of the tail pulleys had buildup behind the guard that was covering the wing pulley. EE took a 36"" long rod with a hook at one end and inserted it in a 1"" space in the guard to sweep the material free from the tail pulley. In doing so the rod was caught by the wing pulley and ripped the rod from his hand which resulted in breaking the skin and his thumb.

Non-employee committed suicide on mine property

We were cutting down a 30' section of conveyor and IE was making what he thought was the final cut, but he over looked one 2"" metal water pipe on the catwalk so when he made the cut, the 2"" metal water pipe came up and hit IE from behind and injured his back and face.

Employee was washing out the main dozer trap feeder with a water hose. When he exited the feeder he slipped on the wet rock and cut his right hand on a sharp rock. He was not wearing gloves at the time of the accident. Went to doctor and received three stitches.

Emp. was checking PC-6 head section drive belts, 14"" from ground. He climbed onto belt, got on his knees, leaned out over the box, he grabbed a piece of angle iron that was not connected and slipped away. He lost his balance and fell into head box (5 1/2"" deep). He reached up with his left hand & grabbed side of head box to break his fall, dislocating his left shoulder.

EMPLOYEE WAS REPAIRING LACINGS ON CONV. #4 AT THE SCRUBBER, USING AN AIR HAMMER AND HIS LEFT HAND HOLDING THE FASTENER PUSHING IT UP UNDER THE BELT. THE BIT SLIPPED HITTING HIS LEFT HAND. HE WAS GIVEN FIRST AID AT THE SHOP AND TAKEN TO AN EMERGENCY ROOM FOR TREATMENT.

Employee was using pressure hose around the old rinsing area cleaning tail pulleys on 10-15-09. On 10-16-09, 8:30 a.m. he reported to superintendent that his back and leg pain was worse than yesterday (did not go away overnight). Employee was sent to medical clinic. Diagnosis was ls strain with modified duty.

Emp was starting load out system, slipped on concrete pad falling into 3 feet of soft material, he managed to free himself. He went to mmedical clinic and received a return to work on 9/5/09. He received pains and anti-inflamatory medications. Upon follow up 9/8/09 he was given physical therapy for 2 weeks 3 times a week. He returned to full time work with no loss time.

On 8-6-07 employee complained about sore knee from slamming on D9L Dozer brakes. Did not want medical attention. On 9-4-07 complained about his knee being swollen. Sent him to doctor and he was cleared for work. Medication: Oxyproxin more than 1 dose (anti-inflammatory).

EE was cutting a piece of wear rubber. He was cutting towards himself & cut right thigh. Required 3 stitches.

West Coast Sand and Gravel truck driver slipped & fell at self loading bunker #6, he was stepping over a space of 32"" wide & 34"" tall; hitting his head, side of chest, right hand & elbow abrasion. Weather was drizzling & damp. After the accident a catwalk & handrail was installed.

While shoveling, employee was bent over and twisting left with the shovel. He overextended, pulling lower back.

Employee was found @ 7:40am collapsed in tunnel walkway. Emergency procedures were admininstrated to no avail.Employee was pronounced dead at the site preliminary findings were by natural causes.Awaiting final determination.

After cutting a steel plate, it was being turned over to de-slag the cut edge; when it slipped out of his hands and caught his finger between the two pieces of steel.

EE WAS WORKING AROUND CLASSIFIER WHEN SOMETHING BLEW IN EE'S EYE RESULTING IN DISCOMFORT & MEDICAL TREATMENT FLUSHING OUT, ECT

EE WAS CHECKING RADIATOR FLUID, WHEN HE TURNED THE CAP IT BLEW OFF CAUSING FLUID TO SHOOT OUT OF TOP OF RADIATOR, CAUSING BURNS TO HIS HANDS & FACE.

WHILE EMPLOYEE WAS CUTTING A NUT OFF A BOLT SOME HOT SLAG POPPED AND LANDED IN HIS LEFT EAR.

WHILE EE WAS CUTTING A CONVEYOR BELT THE RAZOR KNIFE HE WAS USING SLIPPED OFF BELT, CUTTING HIS LEFT MIDDLE FINGER.

WHILE EE WAS LAYING ON HIS BACK, CUTTING A WATER PIPE A PIECE OF HOT METAL SLAG FELL INTO HIS RIGHT EAR.

WHILE STEPPING OFF LOADER WHILE FUELING, EE FELT A PAIN IN THE LEFT LOWER BACK & LEFT LEG.

WHILE EE WAS REMOVING SOME PIPE FOR A JOB HE WAS WORKING ON SOME PIPE ROLLED OFF THE AND LANDED ON HIS RIGHT HAND INJURING HIS PINKY FINGER.

EE WAS PARKING DOZER AT THE END OF HIS SHIFT AS HE WAS STEPPING OFF THE DOZER, HE SLIPPED ON THE DOZER TRACK AND FELL TO THE GROUND BREAKING HIS RIGHT HIP.

GOT ON 50 TON HAUL TRUCK TO MOVE DUST. REACHED BEHIND SEAT TO RETRIEVE WINDOW CLEANER. FELT BUMP ON HAND. PULLED BACK AND NOTICED CUT ON FINGER.

EE WAS CLEANING TAIL PULLEY ON CONVEYOR 13. WHILE CLEANING PULLEY HE BEGAN TO FEEL BURNING PAIN IN THE GROIN AREA. WAS TOLD TO KEEP AN EYE ON IT AND IF IT GOT WORSE, EE WOULD NEED SURGERY. EE WENT IN FOR SURGERY ON 2-12-03.

INHALED OILY GASES, WHILE SHOVELING DUST UNDER CONVEYOR BELT. OIL HAD BEEN LEAKING FOR 3 WEEKS WAS TOLD IT WAS TO BE FIXED.

EE WAS GRINDING BURRS OUT OF HOLES WITH GRINDING WHEEL. SOME FRAGMENTS FLEW & HIT EE IN RIGHT EYE.

WHILE WALKING TO CONVEYOR 30 EE STEPPED IN SOME WATER WITH HIS RUBBER BOOTS & A NAIL WENT THRU THE BOOT PUNCTURING HIS LEFT FOOT.

TIGHTENING STATIONARY JAW PLATE CRUSHER BOLT SPRING LOADED. BOLT BROKE, STRIKING IN CHEST & STOMACH CAUSING VICTIM TO FALL ON BACK.

WHILE INSTALLING SCREEN MAT ON SCREEN #11 MY FINGER GOT CAUGHT BETWEEN SCREEN MAT AND SCREEN #11.

PIECE OF METAL SLIVER WENT THROUGH COVERALLS WHILE DOING REPAIR WORK AND STEPPING OFF TRACK IT DRAGLINE.

WAS WALKING BY SCRUBBER UNIT WHEN HE FELT BURNING SENSATION ON NECK.

INJURY HAPPENED ON 1/31/01. HAD REOCCURRING PROBLEM 3/30/01. HAD TO GO TO DOCTOR WHO REFERRED TOORTHOPEDIC. WAS INSTALLING TROUGHING ROLLER. ELBOW HIT METAL FRAME.

WHILE EE WAS PUSHING MATERIAL AT SURBE PILE MUD FLEW UP OFF OF TRACK HITTING HIM IN THE EYE.

WHILE OPERATING DRAGLINE EMPLOYEE EXPERIENCED SOME NUMBNESS ALONG LEFT SIDE OF FACE ARM AND LEG.

WHILE SLIDING STEEL PLATE OVER SIDE OF PLANT IT HUNG UP ON A BOLT CAUSING ME TO RUN INTO IT AND BREAKING #11 TOOTH AND HAVING IT EXTRACTED.

FOOT SLIPPED OFF STAIRWAY AND EE FRACTURED LEFT HAND AND ARM.

CHESTPAIN, EYE PAIN, ARM PAIN, HEART PAIN.

EE WAS REMOVING SANDSHOE FROM BELT, BELT CAUGHT EE'S SHIRT HE LOST BALANCE AND FELL ON LEFT KNEE.
