The employee hurt himself on 6/26/2007 but did not choose to go see a doctor until 7/13/2007.He was placed on restricted duty on 7/13/2007.The employee twisted R-knee while chasing water truck to get the operator's attention, which resulted in a contusion to R-knee/R-ankle.

EMPLOYEE WENT TO SEE THE COMPANY DOCTOR ON 7-2 AND WAS PLACED ON RESTRICTED DUTY AND STITCHES WERE APPLIED. WHILE REPAIRING A CONVEYER AND A BELT CLAMP, EE LOST FOOTING AND FELL.
WHILE ATTEMPTING TO CATCH A STRAY DOG ON THE PROPERTY, THE DOG BIT EE WHICH CAUSED A PUNCTURE WOUND IN THE RIGHT HAND. EE WENT TO COMPANY DOCTORON 5/10 & WAS PLACED ON RESTRICTED DUTY.
EE WENT TO SEE DR. ON 1-29-01 AND WAS PLACED ON RESTRICTED DUTY. EE WAS TAKING A DOOR OFF WITH ANOTHER WORKER AND WHEN SETTING IT DOWN ON THE CATWALK, EE FELL FIVE FEET TO ANOTHER CATWALK.
EE WENT TO SEE A DR ON 1/2/1 AND WAS PLACED ON RESTRICTED DUTY. WHEN EXITING LOADER, EE STEPPED ON A ROCK WHICH CAUSED A SPRAIN TO HIS LEFT ANKLE.
