It appears that an employee experienced a personal medical event while at work as they were found unconscious leaning into a parked and shut off service truck. Employee was transported to a hospital after EMS reviewed EKG results.

Struck by flying object

Employee reported eye irritation. Stated they had washed it out with saline solution, but the following day was still experiencing discomfort. They requested to be seen at the clinic. Employee was seen by clinic. Instructed to return to work the next day. Prescribed antibiotics. The employee's medical visit concluded after the end of work shift on 9/15.

Employee was working under conveyor, replacing a return roller. The roller slipped from the bracket and struck the employee in the face.

Employee was walking up a flight of stairs at the C-106 conveyor to replace a bearing when ee felt a pain in lower right leg. The injury was reported to the Plant Manager on 9-13-17 and the Injured employee was taking to an Occupational Medical Clinic were ee was diagnosed with a strained muscle/tendon to lower right leg.

Employee was trimming a newly installed grease line for a bearing installed on a tail pulley. When employee cut through the grease line ee cut left index finger. EE was seen at the occupational on 06/30/2017 where ee received butterfly bandages. EE was released to restricted duty on 06/30/2017 with a follow up appt. on 07/06/2017.

Fall onto or against objects

Pulling mining belt onto conveyor with excavator; almost completed belt pulling. The excavator veered right, resulting in taking belt out of alignment causing belt to come off the top structure and subsequently forcing workers into hand railing on catwalk adjacent to structure.

Caught in, under or between a moving and a stationary object

A five-employee crew was stringing a new conveyor belt using a 350 excavator, forklift, and pry bars. As the belt was being pulled into place, it lifted out of the troughing rolls, swinging toward three employees standing on the catwalk. The miner was caught between the belt and the handrail, causing a strain to neck, jaw and shoulder.

Struck by falling object

Employee was removing worn paddles from material washer by lifting paddles to edge of tub and tipping them over the edge. Employee lifted paddle to edge and didn't clear lip of tub causing the paddle(40lbs) to fall approx. 10in. smashing employees right middle finger. Resulting in fracture to tip of right middle finger. Management notified of incident on 4/25/2016 @ 8:30am.

Employee was operating a Cat TH83 forklift to remove a 6"" steel pipe from a walkway. While moving the pipe, a second employee came over to direct the operator. The 6"" pipe was lowered to the ground when it pinched against the mast of the forklift, causing pipe to be launched into the air, causing a fracture to the left foot.

Employee reported that he was removing an old rubber grease line to install a new longer one. He stabbed his left wrist with his pocket knife when his pocket knife slipped while cutting the old grease line.

Employee was climbing into the bottom deck of screen #1 and hit his right knee on the sidewall support that was being installed.

Employee was helping a heavy duty repairman install a (2) section belt clamp on a conveyor belt. EE did not realized his right middle finger was between the belt clamps and when the repairman tightened the side tension clamps with an impact guy, his finger was caught between the upper and lower belt clamps.

Employee was performing normal duties of a weighmaster which included tearing ticket tags for customers, when she developed swelling and pain in both wrists. When reported to the company, the employee was given modified duties that were accommodated until 2/17/12. Her condition did not improve so modified work was ceased to allow for additional healing.

Employee was doing repair work in sandscrew. The employee was finishing the job and throwing a 3/8"" lifting chain out of the tub, over the side, to the ground. As he released the chain, the grab hook caught on his right glove and pulled his arm and strained his right shoulder.

Employee was moving coarse material washer shoes throughout his shift from pallet lifted by forklift and handing to another repairman before they were bolted into place on flights of shafts. Shoes weigh 31 lbs. each. At the end of the work shift the employee's right shoulder was stiff and painful.

EE WAS TRYING TO ALIGN THE THREADS ON A CRUSHER CAP NUT. HE WAS PULLING AND ANOTHER EMPLOYEE WAS PUSHING ON A PRY BAR. THE PRY BAR SLIPPED OFF WHEN THE CRUSHER CAP MOVED. HE FINALLY HAD AN MRI DONE ON KNEE AND A LIGAMENT TE AR WAS FOUND. HE WORK UNTIL 6-11-01 WHEN HE WAS PUT ON A NO-WORK STATUS BY HIS DOCTOR. EE WAS GREASING DOZER AT THE SHOP.
