Quality control technician was adjusting the forks on a Gradall forklift & placed right hand between the back of the fork and the back of the attachment, resulting in a laceration to the right ring finger and small finger.

On 11/7/19 the employee received stitches to close a laceration. While hosing the catwalk near the feeder, the feeder became clogged, and a rock fell out striking the employee on EE's hand resulting in a laceration to the right index finger.

The employee's pant was caught on a fence post that was leaning against a truck while walking backward carrying a fence with a co-worker. This caused EE to fall backward; EE used arm to break the fall.
