On 1/5/11 employee was taken to the company clinic and was placed on restricted duty. The employee attempted to remove the right outside tooth from the bucket of a loader with a 4 lb. hammer when a metal fragment chipped off and entered the R-knee. The employee was diagnosed with a foreign body to the knee.

Employee slipped while coming from the top of sweeper which had a ruptured hydraulic line. The employee had hydraulic fluid on the bottom of his boot and the slip resulted in a laceration to the right shin and the employee being placed on modified duty by the company doctor on 2/25/08.

Worker was sitting on beam 14 feet above ground helping another worker torque down 3/4"" bolts on dump hoppers. He had torque gun with him on the beam so the other worker in scissor lift could reposition the lift. Once he had the lift repositioned he had the worker hand back the gun and that is when he fell from the beam.

The employee went to see the company doctor on 10/31/2005 and was placed on restricted duty. While tightening a connection to the C-2 electrical box, a piece of steel thread from the piping became embedded in the employee's R-palm (hand)
The employee went to see the company doctor on 8/2/2005 and was placed on restricted duty. The employee's R-index finger was caught between the bowl frame of the hydraulic crusher while re-assembling the locking post, causing a laceration that required sutures.
Employee went to see doctor on 2/21/05 and stitches were applied. Employee was operating a loader and struck a berm, causing employee to strike head against light in cab of vehicle, resulting in a laceration to scalp.

WHILE INSTALLING A THIMBLE IN THE WASH CONCRETE SAND BUNKER, THE THIMBLE WAS BIGGER THAN THE OPENING AND IT SHIFTED, PINNING EMPLOYEE'S SHIN.
