On 11/7/19 Vulcan was informed by its third-party insurance administrator that an Agreed Medical Evaluator through the litigation process determined the employees alleged cumulative trauma injury to the low back to be work related.
On 1/5/11 employee was taken to the company clinic and was placed on restricted duty. The employee attempted to remove the right outside tooth from the bucket of a loader with a 4 lb. hammer when a metal fragment chipped off and entered the R-knee. The employee was diagnosed with a foreign body to the knee.
On 11/8/2010, received notification from the workers' compensation Third Party Administrator that the employee received therapy relating to the 3/17/2010 cumulative psychological claim.
Employee slipped while coming from the top of sweeper which had a ruptured hydraulic line. The employee had hydraulic fluid on the bottom of his boot and the slip resulted in a laceration to the right shin and the employee being placed on modified duty by the company doctor on 2/25/08.
Worker was sitting on beam 14 feet above ground helping another worker torque down 3/4"" bolts on dump hoppers. He had torque gun with him on the beam so the other worker in scissor lift could reposition the lift. Once he had the lift repositioned he had the worker hand back the gun and that is when he fell from the beam.
The employee went to see the company doctor on 10/31/2005 and was placed on restricted duty. While tightening a connection to the C-2 electrical box, a piece of steel thread from the piping became embedded in the employee's R-palm (hand)
The employee went to see the company doctor on 8/2/2005 and was placed on restricted duty. The employee's R-index finger was caught between the bowl frame of the hydraulic crusher while re-assembling the locking post, causing a laceration that required sutures.
Employee went to see doctor on 2/21/05 and stitches were applied. Employee was operating a loader and struck a berm, causing employee to strike head against light in cab of vehicle, resulting in a laceration to scalp.
EE WENT TO SEE CO DR ON 3-24-03 AND WAS PLACED ON RESTRICTED DUTY. EE TIED A 2"" COUPLER TO THE END OF A ROPE TO THROW TO OTHER EE WITH EXTENSION CORD AND HE SPRAINED HIS R-SHOULDER.
EE WENT TO SEE CO. DR. 1/22/03 & WAS PLACED ON RESTRICTED DUTY. AFTER DESCENDING LOADER, EE MADE CONTACT WITH GROUND & HIS L-KNEE POPPED OUT OFJOINT & THEN POPPED BACK IN WHICH CAUSED A SPRAI N TO THE L-KNEE (ALLEGED)
EE WENT TO SEE CO DR ON 8-30-02 AND WAS PLACED ON RESTRICTED DUTY. WHEN DRIVING FUEL TRUCK IN SOFT SAND, TRUCK BOUNCED AND EE STRAINED BACK.
EMPLOYEE WENT TO COMPANY DOCTOR ON 12-13-01, STITCHES WERE APPLIED AND EMPLOYEE WAS RETURNED TO FULL DUTY. EMPLOYEE WAS TAKING NUT OFF USING A RATCHET AND TORQUE MULTIPLIER WRENCH WITH A CHEAT ER PIPE. CHEATERPIPE CAME OFF THE RATCHET, SPRUNG UP AND HIT EMPLOYEE ON THE NOSE.
EE WAS ATTEMPTING TO REMOVE TWO MOUNTING BOLTS ON AN ARTICULATING TRUCK'S HYDRAULIC PUMP & STRAINED HIS LEFT SHOULDER & ARM. HE WENT TO COMPANY DOCTOR ON 8/6/01 & WAS PLACED ON RESTRICTED DUTY
WHILE INSTALLING A THIMBLE IN THE WASH CONCRETE SAND BUNKER, THE THIMBLE WAS BIGGER THAN THE OPENING AND IT SHIFTED, PINNING EMPLOYEE'S SHIN.
EE WENT TO SEE THE CO DR. ON 5-30-01 AND WAS PL ACED ON MODIFIED DUTY. EE SLIPPED ON A ROCK AND LOST HIS BALANCE WHICH MADE HIM PUT OUT HIS RIGHT HAND TO BREAK HIS FALL. HE JAMMED HIS R-PINKY FINGER ONTO A HANDRAIL.
EMPLOYEE WAS USING A SLEDGE HAMMER TO KNOCK OUT A STEEL WEDGE; A PIECE OF STEEL BROKE OFF WEDGE AND CAUSED LACERATIONS TO TWO MIDDLE FINGERS OF LEFT HAND. EMPLOYEE WENT TO DOCTOR 8/8/00. STITCHES WERE APPLIED.
EE WENT TO SEE DR ON 5-1-00 AND WAS PLACED ON LIGHT DUTY. WHILE EXITING FORKLIFT, EE STEPPED ON UNEVEN CONCRETE AND TWISTED HIS LEFT ANKLE.
EE WENT TO SEE DR. ON 2-7-00 AND WAS PLACED ON RESTRICTED WORK DUTY. EE SLIPPED OFF THE BACK OF A REPAIR TRUCK WHILE DISMOUNTING AND JAMMED RIGHT HAND ON THE GROUND WHILE TRYING TO BREAK HIS F ALL.
