Employee was working on replacing skirting on this particular conveyor. Employee was kneeling down when they stood up and turned, resulting in a knee strain. As of 8/15/25 employee has an MCL tear right knee.

Upper Pier on K4 Kiln welding retention blocks. Stepping down from ladder and stepped on things underneath the ladder causing employee to twist and strain back. As of 6/24/25 Modifications to desk duty by Medical professional.

Employee was trying to remove a metal door on the disentegrator. As the door came loose ee lost grip. The door fell and smashed left wrist.

Employee was cleaning out extruder. The chute that drops down to run out the plugs, slipped and smashed his right ring finger.

Employee was vacuuming debris from jack hammer when a piece of debris flew up under his safety glasses. The debris caused minimum irritation. Employee's condition worsened as time went on. Doctors put him on restricted duty to stay away from dusty environments.

Employee was attempting to throw a rope over a portable water tank that was loaded on a flat bed trailer. When he threw the rope he felt a sharp pain in his left arm. Doctor put employee on restricted duty.

Employee was helping replace a shaft on a gear box. They were trying to beat the opposite side with a hammer. While holding the opposite side the gear box, it slipped and hit his hand. A small peace of metal was stuck in his left hand.

EE was shoveling underneath a conveyor belt. He struck an object embedded in the ground with the shovel. This caused a contusion to his right wrist.

Employee was installing new V-belts on K-3 shuttle drive using a pipe wrench to roll on belts when the pipe wrench slipped smashing his left index finger between the wrench and motor base. Employee suffered an open wound and hematoma.

Co-worker was using a hammer to drive the pin out of the excavator tracks employee was on the other side when the pin came out and struck his arm. This caused a large cut on employee's right forearm. He returned to work the following day.

Employee was changing out extruder grid when he inadvertenty moved a protective cover over a section of catwalk that was under construction. Employee had been aware and warned of the covering over the catwalk. Employee lost his balance causing him to fall.

Walking around K-3 Kiln gust of wind blew dust and metal particals in face and eyes. a foreign piece of metal went into left eye.

Employee felt hot, went to get more water and felt faint. Safety Coordinator at the facility called paramedics as a precatuionary. Employee was taken to Hospital and later that evening was released.

Employee was in manbasket attempting to weld wear ring on rear of kiln when a gust of wind took it out of his grip and pinched right thumb between new wear ring and rear of hood.

Truck driver fell off from the back step approximately 3 ft. while tarping trailer and broke his right leg.

While tightening the nose cone bolt with his foot and holding the wrench with his left hand and keeping the bolt from turning with his left foot, the bolt turned catching his left index finger between the end of the wrench and nose cone. This smashed his left index finger.

EE WAS DESCENDING STEPS WHEN HE FELT SHARP PAIN IN JAW. EE HAD AN INFECTION IN HIS JAW. EE PULLED HANDS OFF OF HANDRAILS TO GRAB AT HIS FACE. EE LOST HIS BALANCE & FELL & BROKE RIGHT LEG BETWEEN KNEE & ANKLE.
