Elevator got stuck with EE inside at 5:41. Service contractor notified. MSHA called at 6:21 ref. 04978221. Service contractor arrived at 7:05. Stuck EE out at 7:33. MSHA notified. Service contractor cleaned dirty ground floor switch.

EE was lifting a mill bearing cap and the chain hoist hook stretched causing the cap to fall and graze EE's hard had and right cheek bone.

EE was attempting to clear a dust collector vent line and in the process of doing so they got a fragment of material in eye. Although they were wearing safety glasses the material got into left eye. The material was removed at the clinic and the EE was given an eye patch and placed on restricted duty for 2 days.

During manual concrete sounding inspection of a storage bam wall, the inspector impacted the left thumb with a 20 oz hammer, causing a fracture of the digit.

Contractor was taking the elevator down to the ground with an oversized ladder and it shifted in the elevator causing the elevator car to rock which locked the governor. This is a safety feature of the elevator car.

Worker was placing hand brake on rail car. Worker felt a strain in shoulder while doing this but did not think any medical attention was needed. Worker did not seek medical attention until 07/17/23, Dr modified worker's duty on this date of no overhead work.

Elevator went into earthquake mode due to excessive movement in elevator. One employee in the elevator stated They tripped over their broom causing the excessive movement which tripped a safety feature on the elevator engaging elevator brakes.

Temp Laborer was cleaning material off Group 3 scale and while attempting to step off the scale EE rolled EE's ankle. The step off the scale is approximately 8"". The temp. laborer was taken for medical eval. and given restrictions due to sprained ankle.

EE was removing plywood form material with a pry bar. When pulling down to separate the materials, the bar slipped pinching the tip of the EE's right ring causing a laceration.

An EE was standing on a step ladder using a hammer to unchoke an air slide at the Plant. For unknown reasons they slumped over and slid down the ladder landing on their left side. EE was non responsive and workers provided first aid including use of an AED and CPR until EMS arrived and took over. EE was transported to the hospital, did not recover consciousness before they died.

EE was attempting to open a clinker rail car hopper door to dump the railcar using a prybar. The EE put their leg under the hopper door and when the door opened the clinker flushed onto EE's leg causing a burn to their right leg. The EE was taken for medical evaluation and placed on modified duty. The burn was cleaned and bandaged.

Fall down stairs

EE was walking down steps to access mens shower room when EE lost balance. EE landed flat footed on right foot and felt a sharp pain. EE had fractured the same foot 4 months prior. X-ray on 9-30-22 showed foot had been fractured once again in same area. EE was given restrictions and walking boot.

Train mechanic was using a sledge hammer on locomotive when they hit themselves in the lip with the hammer, causing a laceration requiring one stitch to close. Employee was given modified duty.

Employee had just arrived and clocked in for work when EE went to get into EE's service truck. As EE was attempting to open the door with exterior door handle EE stated EE felt a pain in EE's middle finger and finger and hand immediately swelled. EE was diagnosed with a sprained finger from this action.

An employee was attempting to clear a plugged air slide. When EE opened the inspection cover while it was running, material shot out of the air slide causing material to get in EE's eyes. EE was taken for medical evaluation and placed on modified duty.

EE was performing routine maintenance of a bucket elevator when a speck of sand blasting material that was used on the separator got in eye. The EE flushed the eye and felt confident the material had come out but later that evening felt discomfort in the eye. Taken for exam and object was removed by optometrist. EE given antibiotics to fight possible infection.

There was an abrupt motion inside the car caused by one of the miners while traveling down that activated motion switch that caused the elevator to stop therefore causing the miners to be trapped. Elevator service co. responded and miners were released within 58 min. The elev. was inspected and returned to service.

EE was clearing debris using a hammer and chisel. EE felt something hit leg and felt a small bit of blood on EE's leg. EE reported it to us on 12/16/19, EE was taken to the Dr. who ordered x-rays. The x-ray was read on 12/19/19 and it was found that there was a .3cm fragment. They attempted to remove it and did not find it. This caused EE to receive a single stitch on 12/19/19.

On 5/31/2017 Employee reported an injury that happened while loading mill balls. Employee got hit on the lip with a piece of metal. EE was released back to work full duty same day. On 6/17/2017 employee felt something in EE's lip. We took EE to the clinic. The doctor sent EE for a new x-ray that showed something in EE's lip. On 6/22/2017 they cut it out and put in 1 stitch.

EE was cleaning a bearing cover in the solvent tank, it slipped and he pinched his right middle finger between the cover and the edge of the tank. Superficial laceration requiring a steri-strip. Medical treatment was not documented on the medical work status report. The clinic was called today for clarification of medical treatment (12-28-12), and this is reportable per definition.

Employee was changing a roller on a conveyor belt. He was removing a clip that retains the cover, when some clinker dust was dislodged and blown into the employee's left eye by the wind. The employee was wearing safety glasses at the time of incident.

Employee's hearing was tested on 12/14/2011 which revealed a mild hearing loss. He was scheduled for a re-test but became ill and could not be re-tested until he recovered. Hearing loss from re-test showed a 25 decibel shift in the right ear at the 3000 Hz level. This became reportable at the time the company received the final re-test report on 2/2/2012.

Employee had a mild stroke and was transported via ambulance to the hospital. He did not have any accident or property or any type of incident on the mine property. This was determined to be non-work related.

Employee was diagnosed in 1990 with bilateral carpal tunnel syndrome. She only chose to have surgery on one hand. On 11-11-11, she requested to have the surgery on her left hand. She had surgery on 4-26-12 at which time this became reportable by definition. She returned to work on her next scheduled workday 5-01-12 on full regular work duty.

Employee was diagnosed with carpal tunnel syndrome in 1994 but did not elect to have surgery. He filed a new claim on 11-12-10. He has been doing full work duty without any restrictions unti l the date of his surgery on 5-24-11, at which time this case became reportable.

Employee alleges he aggravated a pre-existing hernia while using a 16 lb. sledge hammer to loosen bolts from the #11 Mill Bull Gear. Employee alleges he felt pain and a tearing sensation to h is right side.

Employee was cutting a piece of hose to use as a patch for another hose on #12 Mill. His razor knife quickly went through the hose and lacerated his right thigh, which required seven sutures. He returned to full work status the same day.

Employee walked into an airslide, hit his head which caused him to jam his neck. He has been under medical observation only, but on 6-11-2010 the doctor felt he needed physical therapy, which makes this accident reportable on this date 6-11-2010.

While closing one of the double doors on the compressor room when a steel bar came down and struck him on the left hand ring finger. Hand was on the door knob when the bar came into contact with his finger.

Employee alleges he strained his right groin area as he slipped while sweeping clinker and did a half spllit. He failed to report this accident until 12-15-09 at 8:50 AM. He was sent to the Industrial Clinic and was placed on restricted work duty. This alleged incident is being investigated as to whether or not it is work related or an incident that may have happened at home.

Employee was using a pry bar to hit an air activated rail car door that did not open. He was crouched down so he could pry it open, but there wasn't anywhere he could pry it. As he was crouched with the pry bar in his hand, the door automatically opened, striking the pry bar which hit the employee on the forehead.

EE was trying to fix a stuck pallet in the pallatizer when he got his arm stuck between the rollers and the pallet came loose and arm was caught. The result was crushing of his forearm and wrist resulting in damage to muscles and possible ligaments.

EE was cleaning north side of tower with an airlance when hot feed blew onto his jacket on right side burning a hole through to his right forearm and wrist causing 2nd degree burns.

EE was on BB1 conveyor belt looking at blockage when a second EE turned on the belt causing first EE's body to be projected towards the chute. His left leg went under the belt when left leg wa s caught by the head pulley severing the left leg, freeing the employee.

Gas Fire on North side of Kiln 3 preheater tower. Gas was disabled immediately. The fire caused damage to cannons, lighting and air hoses. The connection fitting from the riser to the hard pipe failed, causing natural gas and oxygen to come into contact with ignition source and ignite. No injuries took place and MSHA was notified locally.

EE was on 416 locomotive when backing up on the mainline couple with the clinker train. The 416 coupled hard causing a jerk of the train, which caused the employee to be jerked hard. EE did feel any injury at the time however days later experienced pain in his lower back.
