During preparation for dismantling of motor and gear box on slide gate. Employee didn't follow given instructions and removed all bolts from gearbox. That cause released of stored energy on slide gate and motor with gear box start spinning. Employee got caught by the equipment and started spinning with it. During spinning their head hit the handrail which caused a fatal injury.

Miner working on emission analysis equipment located in the third stage of the pyro processing tower, Miner was cleaning the sample probe, the sample probe was still hot when the miner accidentally touched it with EE's hands. Miner show minor burns in palm of EE's hands.

On final day of EE's job, employee was descending a 15' fixed ladder with a cage when EE's foot slipped and EE fell approximately 10'. Employee broke EE's left fibula.

EE was performing preventative maintenance on an MCC bucket. While checking the A phase line side termination EE lifted EE's arc flash hood to align EE's screw driver correctly, as EE lowered EE's hood the screw drive made contact with the frame of the bucket causing an arc flash. EE sustained 2nd degree burns to face.
Electrician had inserted a new breaker bucket to a 480 volt panel and attempted to re-energize the panel. Upon doing so the breaker shorted causing the miner to sustain burns.
SHOVELING DEBRIS IN & AROUND CHUTE & SMASHED FINGER
Employee was working on the pre-heater tower. As EE was coming down a stairway, EE slipped/tripped on the last step. EE fell onto the mezzanine straining their back.
Employee was water blasting in the tower. As EE was holding the gun, EE pulled the hose causing the safety pedal to be pushed down. This in turn pressurized the gun causing it to strike the miner above the left eye.
EE got dust in eye. The next day EE reported irritation in the eye and was sent to the clinic.
Miner claims to have injured (R) knee climbing stairs, while fetching a fire extinguisher to extinguish a spot fire, while on watch.
EE was walking down some stairs and EE's hand slipped off the handrail. This caused EE to stumble and loose EE's balance. When trying to recover from the slip EE stepped on a curb and rolled EE's ankle.

EE was removing a thermocouple from the tower using a pipe wrench. The pipe wrench slipped off the thermocouple and pinched left pinky between the pipe wrench and the handrail.

Employee tested positive for covid on 12/1/20. I was notified by employee that was admitted to the hospital on 12/5/2020. I reported it to MSHA to err on the side of caution. Employee is not aware of how or when they got the virus.
While EE was welding a plate inside the crusher, a rock that was lodged in the rollers (overhead) became dislodged and struck EE in the back of the head, near EE's ear. Medical treatment was provided and EE was diagnosed with a concussion.

Over the last 4 years ee noticed back was hurting when operating mobile equipment. Over time the pain became unmanageable and the employee was placed on light duty.
EE put left hand next to clay bailey to check the draft and hand was slightly pulled in the clay bailey. This caused EE to pinch finger in-between pin on screw and clay bailey.
EE was cleaning a chute and it appears EE stuck hand into a clay bailey while holding a flash light. As hand entered through the clay bailey and into the screw conveyor housing, a piece of round stock cut hand.
While striking a bar with a hammer a small piece of the bar splintered off of it going into ee left forearm
Employee was getting out of a piece of equipment and slipped. EE came down approximately 2' landing on right ankle.
EE was jackhammering a dirt pile. While lifting the jackhammer out of the pile, ee strained back.
Employee was holding a ladder for a co-worker while ee was installing a ballast cover for a light fixture. While the co-worker was adjusting the east side so a bulb would fit the west side came loose causing the cover to fall and strike and cut employee's left hand/wrist.

While installing liners in a MILL a liner was lowered and rested on mill balls after disconnecting rigging the liner slipped on mill balls and fell over on a mans foot.

Employee was removing a come along from a forklift and set of cribbing. Forklift shifted and employee's right middle finger got lacerated.
EE was lowering a blastline into a column when his left 4th and 5th fingers became caught between the pulley and the rope. This resulted in lacerations to these fingers. The employee's wounds were immediately treated on site and he was then taken to a clinic to be examined. He was diagnosed with a nail avulsion, given 3 stitches, and a tetanus shot. He was then released to work.
EE was jackhammering in the clinker cooler and turned to his right straining his back.
Miners was walking down a dirt slope and fell to the ground
EE was running a vactor truck and the hose became plugged. In an effort to unplug the vactor hose he hit it against a wall. Dusty material on the wall fell down and got into the ee's eyes.
EE was struck by a fan while trouble shooting the starting system of the skytrac forklift.
Two employees were going up to the top of the shipping silo when the elevator stopped at aprox 8:05 pm. The elevator earthquake safety switch bar fell onto the emergency shut off causing the elevator to stop. The switch was reset and the gentleman were able to operate the elevator as designed at aprox 8:49 pm. They were in the elevator for aprox 44 min.
The EE was putting the pin gate in on 41WF06 with a hammer. He was in an improper position for the task and hurt his back.
EE was going up the elevator and it stopped in-between floors. The up normal and bottom final contacts were found to be dirty causing the elevator to stop.
Employee was trouble shooting a motor which he had locked out and pinched his finger between the pulley and drive belt.
Diagnosis from the DR. said to be an Acute reaction from lack of vitamins, sleep, and dehydration caused his body to go into convulsions. the employee apparently was going through alcohol withdrawals while he was sustaining his attempt for sobriety .

EE was swinging a hammer at a return roller on a belt conveyor and missed it. This caused his hand to in between the roller and conveyor belt.
EE was prying on a door and the pry bar slipped. When the bar slipped it pinched his left index finger in between the pry bar and the kiln hood causing a cut to the tip of his finger.
Employee was stung by a bee on 8/9/12 and reported the incident the following morning after some swelling had occurred overnight. He was taken to the doctor and then placed on light duty activity on 8/10/2012.
Miner was installing a motor when he hurt his back at aprox. 10:30 am on Thursday the 6/7/12. He did not report it until Monday 6/11/12. He thought it would just go away. He was sent to the Dr. on the 11th after reporting the injury. He was released to full duty work on that day. 7/3/12 he asked to go back to the Dr. as his back was still hurting him and he was given light duty.
EE was driving a pin into the drive track of the drill. After striking the pin with a hammer a small sliver of metal from the pin entered EE's left hand.

EE was working on a dust collector and dust blew into his eyes causing minor irratation. EE was given some eye drops for his eye.

EE was walking from the quarry break room to the quarry operator shack. The EE slipped and fell hurting his left knee and left wrist. On 12/29/2010 the miner was put on restricted duty.
Miner was performing maintenance on a motor grader at the shop and strained his neck.
Employee was checking the position of 41TV12 NOTAM and walked toward 41FG01 NOTAM. There was a rake laying on the catwalk, employee tripped over the rake and fell down.
Employee was working on electrial project. He was pulling on a large bundle of wiring when he felt a sharp pain in his lower back area. Subsequent diagnosis indicated that EE suffered lower lumbar strain.
Employee was operating D-9 Dozer in Quarry. He complained of soreness in the area of his tail-bone. Apparently employee developed a boil in tail bone area possibly due to bouncing of dozer in rough terrain. Investigation of seat in dozer indicated that the seat was in good condition.
Delivery truck driver driving for Kim Trucking, Victorville, CA was delivering guard rail material. He climbed onto the rear of his truck to remove edge protectors from the load when he fell backwards approximately 5 feet. He landed on the paved surface breaking his RT arm.
Maintenance shop mechanic was installing new handrail section on haul truck. As he was pushing and pulling hand rail into mounting holes, he felt a pain on his left side. He at first denied medical treatment, but reported that injury was not any better on 11/5/09. Further diagnosis indicated that EE had torn some rib cartledge. EE is on modified duty.
Employee was cutting skirt rubber with a utility knife. As he was cutting the rubber, he cut through the rubber faster than he anticipated and the knife slipped and cut his lower right thigh just above the knee.
A small fire occurred in the NTDF feed system located on the mezzanene level of the pre-heater tower, just below the 4th floor. There was minimal damage and no injuries involved with this incident. MSHA was notified and inspector conducted an investigation.
About 12 noon Injury,other ee and another ee were adjusting the tarps on top of the outside clinker pile. Other ee and Injury were working together trying sand bags to the tarps to weight them. As other ee step back a gust of wind came up lifting the tarp throwing injury up in the air about 8 feet. Other ee dove pulling injury free of the tarp.
A fire occurred on 54-BC-01 and associated transition chute to shacker conveyor (alternate fuel tire fluff feed system). Maintenance was modifiying a guard that the MSHA inspector had issued a citation on earlier in the day. It appears that some hot slag or sparks ignited some of the fluff which remained in the chute area. Fire was put out by Kern County Fire Dept.
Around 12:45 AM on January 23, 2009, a kiln push occurred. Hot clinker came out of the cooler and at 1:13 AM a fire was reported on 62 belt stacker 02 that required off site fire services to be called to the plant. The resulting fire burned the conveyor belt, skirting, rubber impact idlers and dust cover.
Unplanned brush fire, lasting longer than 30 mins, ignited by overhead powerline.
Employee bent over to pick up a light fixture and could not stand up. He was diagnosed with a sprain lumbar region.
Employee was using a chipping hammer to clean a screw conveyor. He was working overhead and slipped on some loose material causing him to twist and sprain the lower back.
Employee was adjusting the tires on the clinker reclaimer. As he walked around the reclaimer employee slipped on the clinker and twisted the left ankle causing a sprain.
Employee was pulling on a coupling with a porta-power and was hitting the coupling with a sledge hammer to jar it loose when a piece of the coupling broke loose and struck the employee in the face. The employee suffered some contusions on the left side of the face.
Employee was using a keyboard and the right hand became swollen with numb fingers and a burning sensation. Employee was given prescription medication and instructed to wear a wrist brace and returned to work.
Employee removed a safety lock and turned on the air on, an air blaster that had the discharge end disconnected. The blast of air struck the left arm and caused an open wound.
Employees instructed to use JLG lift to install and tighten bolts on expansion joint. Employee untied safety harness, climbed out of JLG basket & climbed over onto an unsecure platform to allegedly look for an air hose. Employee did not tie off and fell through a floor opening. He fell approx 3-5 feet onto ""X"" bracing and slid down to top of classifier another 4-5 feet.
Accident happened at 42BE02 while removeable take up support bar was in the process of being removed. His finger got pinched between bar support holes in elevator housing. EE received medical treatment, returned to work the next day.
Employee was assisting with the retorque of a 1 1/2"" Cap screw and re-agrivated and old hernial injury.
Employee was drilling a blast hole in the quarry and saw something fall. He put on his hard hat and exited the cab of the drill where he bumped into a hydraulic hose and jambed his neck.
Employee passed out in the change room. As he fell to the ground he hit his head on the bench and strained his back.
Employee was lying in the bottom of a motor and due to the awkward position he was in he strained his back. Employee did not report the accident until 2/19/08.
Employee strained the lower back using a 60 lb bar to strike the door to #3 ball mill to break the seal so the door could be opened.
Employee was vactoring out a plugged chute in the clinker shed and the hose came out of the clay and attached to the right knee. The employee moved away from the hose and fell striking the knee on conduit lines.
Employee was racking out motor switch gear for fan # 71FA07 at MCC J. He checked with the control room and asked if it was okay to lock it out, but failed to check the guages to verify the switch gear was not under power. The employee disengaged a 4160 volt switch under load causing an arc flash and destroying the breaker. The employee was not injured.
Employee was getting down form D-10 bulldozer and stepped from the track onto the arm of the blade and the foot slipped causing the employee to fall between the track and arm. Employee received a left leg contusion, and a neck sprain.
Hot clinker ignited conveyor 62BC11 at 6:15 AM. Operators responded and reported the fire out at 6:30 AM. The conveyor flared up again at 6:45 AM and was under control at 7:00 AM. Kern County Fire Department responded and released the area by 8:30 AM.
Employee was installing a hydraulic portable jack below the socket in the roller mill when the ram slipped and fell on the finger tip causing a fracture.
The employee was shortening the rigging on a 400 lb. metal drum. It came loose from the forklift and fell, striking the employee's right knee, sliding down both shins, and stopping on the steel toe boots.
Employee was removing exhaust manifold off the engine of 85 ton Wabco haul truck. The turbo slipped and caught the left middle finger between the manifold and steering arm causing a fracture of the finger tip.
EE's were working with 2 chainfalls to remove a damper gate. One chain hung up and the EE put his hand where the gate would come down. When the gate came down, it pinched the right index finger.
Employee was carrying vactor hose up steps. As the employee was going through a door, the hose flipped causing the employee to twist his back.
Employee was opening a welding rod container and received a laceration on the left wrist from the top of the container requiring suture repair.
Employee stepped over a wall by the secondary crusher and rolled the right ankle on a rock.

A fire of unknown origin was noticed on the fluff pile and reported to the control room. The Kern County Fire Department responded and quickly got the fire under control. Damage was limited to minor consumption of the fluff pile and there were no injuries or structures damaged.
Employee was painting railing on catwalk next to conveyor and slipped on some spilled material, spraining his back.
Employee was tracking and training a conveyor belt by tapping the return roller hangers with a hammer. The hammer head was grabbed by the roller and pulled the employee's hand to the roller. The right thumb was pinched between the roller nad the belt. The right thumb was lacerated requiring suture repair.
Employee was using an air lance to clean cement silo #1. Employee did not think ear protection was necessary and injured the ear drum.
Employee was cleaning glass from a broken upper window out of a crane cab. The wind blew a piece of glass into the right eye. The employee was wearing safety glasses with side shields at the time.
Employee was stepping down from a platform after taking temperature readings on a trunion bearing and the right knee gave out, causing him to fall and bump the left knee.
Employee had inflamed knees from climbing stairs. During normal duties, employee was climbing stairs and felt discomfort in the knees.
Employee was being assisted in egressing a ball mill by two co-workers. The co-workers were helping the employee by pulling on his arms. This action caused the employee to sprain his left shoulder.
Employee was moving spare parts for plant equipment. One spare part fell and employee moved to avoid the part from falling on him. Icy conditions caused his foot to contact a wooden pallet, jamming his toe and tearing his toenail.
Employee was climbing a quarry haul truck when he slipped and sprained his wrist while holding on to the ladder handrail. Employee initially declined first aid and medical treatment. On 2/20/07, employee requested medical evaluation. He was released with no duty limitations.

EE was climbing a ladder at 41RM01 and reached with his R hand to open the access gate at the top. When his weight shifted, he strained his L wrist. No Lost-time, No specified work restrictions--Physician issued a rigid wrist brace.
EE was attempting to loosen a bolt for the drive mechanism on 34BC08 and strained his right shoulder while pulling on a wrench.
At about 8:22 a.m. a fire was discovered on belt conveyor 62BC11. The fire was confined to the 62BC11 area and was extinguished by 9:15 a.m. The fire is believed to have been ignited by hot clinker fines contacting the rubber parts of the conveyor. This unexpected release of fines may have been the result of work in the precal system to remove restrictions.
EE lost balance while jack hammering on 62BC11 and fell against a support beam about 3-4' away from his position, bruised right hip
EE reports that he was closing a hatch door to #4 cyclone and lost control of the door. It struck his left hand, fracturing L#4 finger.

EE was backing water truck & misdirected the truck into a berm at a sharp angle. Truck was full of water & with R-rear wheels up on berm rolled over to left & settled upside down on access road a few feet below. EE was using seatbelt. EE was evaluated by ER on 5/1 & by Occ Med MD on 5/2 and given full release; On 5/12, eval by Occ Med MD resulted in TTD.
EE was stepping into the limestone storage building & stepped off a landing and slipped, falling back onto the landing. Reported 4/25; First Treatment 5/4--full release RTW; Returned to MD during work hours 5/8; Missed next scheduled shift 5/12
EE was removing a strip of ""T"" rubber caught in the lift conveyor of the sweeper truck. When pulling, tension on the conveyor was released and one of the conveyor flights caught EE. Laceration to forehead, R-eye orbit fracture
EE reports that he was lifting a section of 1/2"" plate steel with 18"" hole cut out and clay bailey attached using a rope. Approx. weight 40#. The following morning, upon awakening, experienced distended hemorrhoid. Causal relationship being investigated.
EE reports feeling sharp pain on neck when positioned on side, vacuuming on 31BC01 conveyor. On 3/24, employee reported small welt on back of neck, sent for med eval. On 3/27, physician placed on TTD. First lost workday 3/25. Probable insect bite or sting.
EE reports that in February or March of 2006, he strained his neck and L shoulder while dislodging oversized material at the Tower A grizzley. EE reported the incident to management on July 27, 2006. the incident is being investigated for compensability.
EE reports tht on 2/1/06 he was engaged in strenuous pushing, pulling & lifting while working a preheater blockage over 12-hour period. Pain manifested 2-days later off duty while driving. EE filed for WC & notified management on 2/5/06. Case is under investigation to determine if work related
EE was walking down 5 steps from the toolshed to go to the restroom. She lost her footing on the last or second to the last step and fell forward. She caught herself with her left hand and injured her left wrist.
EE was manually adjusting a feed positioner (34 FP 02) and caught his R thumb between the rail and the chute. Initially only FA applied. Med evaluation on 11/24 for swelling, reduced by incision.
Fire discovered at 10:15 pm on 62BC11 conveyor belt inside the clinker storage building and extinguished by 11:15 pm.Probable cause: material from the clinker cooler broke apart on 62DP03 feeding 62BC11.Dust from heat exchanger may have covered the hot material and masked the heat from sensors that would have triggered water sprays.Physical damage to belt only--no injury.
V EXPRESS TRK DRIVER WITH FULL LOAD OF BULK CEMENT RAN OFF THE PAVED ROADWAY BEFORE REACHING THE ENTRANCE GATE. TRUCK TIPPED OVER ON L SIDE. DRIVER WAS EXAMINED & TREATED FOR HEAD LACERATION. DRIVER WAS DISCHARGED FROM EMPLOYMENT FOR DRIVING INFRACTION.
EE was operating boom truck, pulling and loading used conveyor belt onto the flat bed of the truck. EE stepped over to fold over a section of belt and slipped on the belting and fell. He landed on his R foot cuasing a sprain
A customer cement truck driver working for State Ready Mix Company slipped while opening a trailer hatch in preparation for loading. He fell against a guardrail, striking his R. lower back. Resulted in 2 fractured ribs. This is not a mine employee.
EE reported injury on 6/3/05. EE had propped a dust collector (81DC17) cover against the guardrail to exchange bags. A gust of wind caught the cover and it tipped over striking EE on left shoulder. Possible bone separation at tendon attachment to L. shoulder.
EE was preparing to pick 2 crusher hammers with boom truck. He lifted one on end and balanced it. While attempting to lift the second, the first tipped, pinching the tip of R #3 finger between the hammers. Laceration required stitches and there was a minor fracture to the distal phalange
EE had removed a cardox tube from the base socket attached to the meal pipe in the preheater tower. Cardox shot was successful in unplugging a blockage & the flow of material resumed shortly therafter. Hot dust discharged through the base socket, striking EE & causing burns to both forearms, wrists, R thumb, L little finger, R ear.
Details are unavailable. Under investigation. Doctor report received from EE's personal physician indicates EE stressed from both off-work and work environment. EE has not filed a report of injury or illness.
EE WAS EXITING THE KILN AT THE END OF HER SHIFT. A FORKLIFT WAS COMING INTO THE KILN AND HAD STOPPED TO LET HER GO BY. AS SHE WAS WALKING AROUND, THERE WAS DUST COVERING THE SURFACE OF THE KILN. SHE SLIPPED IN THIS, RESULTING IN HER FRACTURING HER ANKLE.
At about 10:16 a.m. a fire occurred on 62BC11 & 62BC12. Clinker cooler failed to sufficiently cool the clinker--the source of ingition. BC11 is the 1st rubber belt in the system that feeds BC12, a tripper conveyor that runs the length of the clinker building. Kern County FD & LA County FD responded. Fire was extinguished by 11:27 a.m.
EE was stepping from the driver door of a vacuum truck to the pier at the finish silos. He missed solid contact with the pier and twisted his right knee. He reported the incident on 3/1/05 but declined medical attention until 3/11/05
2/8, mgmt first notified by unsigned reports that EE experienced back/neck pain on 2/6 & was consulting personal physician. 2/10 EE reported by phone that condition was result of mental stress. 2/10 mgmt received diagnosis ""Alleged Stress Cl[aim]"". 2/11 EE reported condition related to concern over disciplinary issue and other interpersonal relations--under investigation
Employee was inspecting work being performed on cone section of finish cement silo #10 when cement dust fell into his left eye
EE WAS OPENING A HATCH ON ROTARY FEEDER FOR 53CM02 COKE MILL WHEN HE LOST HIS GRIP ON THE DOOR WITH HIS RIGHT HAND. THE DOOR (HINGED ON BOTTOM) DROPPED OPEN, PINCHING EE'S LEFT HAND AGAINST THE HOUSING. RESULTED IN HAIRLINE FRACTURE TO LEFT INDEX FINGER. (FINGER DID NOT HEAL; SURGICAL IMPLANT OF PIN REQUIRED.)
EE was carying a load of materials down a short flight of steps and slipped falling forward. EE reported incident on 1/5/05 and declined medical treatment. On 3/24/05 EE was evaluated by a physician but no treatment was prescribed and no restriction were imposed. On 4/19/05, NCC learned that physical therapy was ordered for L. knee & lumbar strain
Employee attempted to retrieve a large rock from the end of a 6-inch vacuum hose and caught his left arm in the hose. The power was shut off within 30-seconds by a helper nearby. Injury was limited to upper arm soft tissue injury/hematoma.
Employee was cleaning up material in the vicinity of the primary crusher most of the workday. At or near the end of the shift, the employee experienced lower back discomfort. Six (6) days later, he reported the incident and completed a report, but declined medical treatment. Two (2) more days passed before he elected to seek medical attention (Dec. 02).
Employee was operating a loader with a quick-connect bucket attachment. Unknown to the operator, the locking pins for the bucket were released. The employee was dumping material while in forward motion when the bucket dropped off and the right front wheel rode up on the bucket and dropped off. Employee reports using seatbelt, but received multiple injuries.
Employee was assisting to unload damaged conveyor belt in scrap yard. He stepped on the belt rubber and slipped on loose clinker falling (same level) on his back.
Employee attempted to lift vacuum hose while cleaning in the vicinity of 62BC10 conveyor inside the clinker building. Employee was standing on a sloped surface with some loose clinker on the surface. Employee lost balance and fell (same level) on his back.
Employee twisted left knee when making a turn around the front of his assigned pick-up truck while performing a pre-operation safety isnpection
Fuel flow failure in pre-cal tower resulted in an unexpected surge of hot raw material (dust) through kiln and clinker cooler. The material was conveyed to conveyor 62BC11 and ignited the belt conveyor before sensor could register the temperature spike. The conveyor fire was discovered at 1:11 p.m. and extinguished at 2:11 p.m.
Employee was opening the outer door on the man-material elevator when the bottom lip of the top section struck his right elbow. The outer door is in two sections that are mechanically connected so that they open together. Employee was pushing the bottom section down with his foot and the upper section struck his right elbow as it passed his arm braced on the open inside door.
While aligning the ball mill manway door, overhead hoist cable failed, causing the line, hook block assembly to strike the employee on the head/neck area.
Employee experienced pain in abdomen and groin after using ladder to access top surface of shipping silos. Treated as first-aid to-date, medial treatment likely.
A truck driver working for V-Express was standing in the waiting room for silo #10 with his back to the hinge side of an outwardly opening door that was partially open. Another driver pushed the door closed pinching the driver's right third finger tip between the door and frame. The injured driver was not an employee of National Cement Company, Lebec Cement Plant.
EE alleges back pain after turning around to get off work stool in shipping silo operator's booth. EE was offered medical treatment after reporting the condition, but refused until she requested medical attention with her personal physician on 5/2/04. She received medical evaluation on 5/3/04 and began loosing time from work on that date.
Employee was standing on an angled support with the right knee braced against a concrete peir at ground level to tighten a fan bearing bolt. His left foot slipped on the angle, shifting stress to his right knee
EE was descending a short fixed ladder and shifted weight to right side as he reached the bottom landing. At that point, he experienced a pain in the palm of his right hand.
Individual was preparing to cut a bolt with a torch. He reached up to place his cutting goggles over his safety glasses. When he did debris fell into his eyes and face.
EE was buffing with a 4"" grinder and debris lodged in his eye.
Employee was pulling water hose for hydroblasting and reports straining his lower back.
Supervisor was standing next to a drive sheave pulley (about 2' diamater) for the clinker crusher that had been removed to replace shaft bearings. Another employee bumped the sheave which was resting in an upright position. The sheave tipped over striking the supervisor's left foot
CROW WAS WRAPPING WOODEN PALLETS WITH PLASTIC TARPS. INJURED EE WAS AT ON CORWON PUSHING TARP UNDER PALLET WHEN THE FORKLIFT OPERATOR LOWERED PALLET. OPERATOR THOUGHT ALL PERSONNEL CLEAR. PALL ET CAUSED A LACERATION TO LITTLE FINGER ON RIGHTHAND THAT REQUIRED STITCHES.
AFTER ADJUSTING THE COOLING WATER VALVE ON A LUBRICATING UNTI 71LU14 FOR FINISH MILL #3 71BM03, THE EE STEPPED ON THE EDGE OF A RAISED FOOTING OF THE STRUCTURE SUPPORTING THE MILL EQUIPMENT AN D TWISTED HIS LEFT ANKLE. THE INCIDENT RESULTED IN A LEFT ANKLE SPRAIN.
REPLACING TRUNBUCKLE AND CHANNEL ON CONVEYOR, HE REMOVED BENT STEEL ATTACHED TO MOTOR CONDUIT & GEAR BOX. ASSEMBLEY ROTATED STRIKING THE BACK OF HIS NECK. HE REQUESTED MEDICAL ATTENTION 10/14.
CARTAGE VEHICLE HAD LEFT PLANT AND ROLLED OVER ONTO ITS LEFT SIDE JUST BEYOND THE FIRST SHALLOW CURVE IN THE ACCESS ROAD ABOUT 0.7 MILE OUTSIDE THE PLANT ENTRANCE GATE. AT 3:00 AM IT WAS DARK BUT CLEAR AND DRY. NO KNOWN OBSTACLES WERE PRESENT ON THE ROADWAY. THE DRIVER'S SPEED IS UNKNOWN.
EE STRAINED HIS LEFT SHOULDER WHEN REACHING FOR A FIRE EXTINGUISHER.
EE WAS CLIMBING DOWN LADDER FROM WORKING ON WABCO WATER TRUCK. RIGHT FOOT GOT STUCK ON BOTTOM RUNG OF LADDER. EE TWISTED RIGHT ANKLE. ANKLE WAS STILL SORE THE FOLLOWING MORNING SO WENT TO DOCT OR.
2 EE'S WERE REMOVING AN AIRSLIDE FROM THE FINISH MILL CEMENT COOLERS, THE ONE END WAS TOO HEAVY TO HOLD SAFELY, THE EE DROPPED THE END HE WAS HOLDING, HE DID NOT MOVE HIS HAND OUT OF THE WAY, THE AIRSLIDE FELL ON RIGHT HAND CUTTING MIDDLE FINGER OF RIGHT HAND.
PRE-HEATER PLUGGED IN AREA OF CY-5. EE FULLY OUTFITTED IN HEAT PROTECTIVE WEAR DRILLED HOLE THROUGH SHELL WALL INTO CYCLONE. WHEN HOLE BREACHED BRICK INTO MATERIAL - HOT MATERIAL PUSHED OUT TH ROUGH HOLE - HOT MATERIAL PUSHED UNDER PROTECTIVE GLOVE AND BURNED THROUGH INNER PROTECTIVE ARM GUARD.
KILN & OR TOWER WAS PLUGGED. CREW REMOVED SIDE DOOR BY KILN HOOD TO LOOK FOR PLUG. SHIFT FOREMAN STOOD AT EDGE OF DOOR & LOOKED INTO KILN DISCHARGE AREA. AN AIR BLASTED DISCHARGE & KNOCKED HOT DUST ONTO FOREMAN'S HEAD. BURNING LEFT EAR &NECK.
EMPLOYEE, WORKING ON SHOP TABLE USING A TORCH TO BURN A DUCT OUT OF A REFRACTORY PAN FOR A COOLER, AFTER BURNING IT OFF TURNED THE PAN OVER. IT CAUSED THE DUCT TO SLIDE BACK DOWN AND CATCH HIS FINGER BETWEEN THE PAD AND THE DUCT.
LIFTING A MOBILE STAIRCASE IN 62DP03 DISCHARGE TUNNEL AND FELT DISCOMFORT IN GROIN AREA. SEVERAL MONTHS LATER WENT TO PERSONAL PHYSICIAN. NOTIFIED COMPANY ON 7/14/03. ALLEGED INCIDENT WAS INVE STIGATED AND ACCEPTED BY INSURANCE AS COMPENSABLE INJURY IN LATE AUGUST.

EMPLOYEE WAS INSTALLING A NEW DIVISION SCREEN IN ROLLER MILL. PEOPLE WORKING ABOVE KNOCKED SOME DUST ONTO HIS NECK, BACK& SHOULDERS. AS HE TRIED TO GET AWAY FROM DUST HE PULLED A BACK MUSCLE.
