Caught in, under or between a moving and a stationary object

the laborer was tasked to unclog the ¾' Tunnel bunker so that material could flow through since it was plugged with wet material. The laborer stood outside the bunker and used an air-lance to dislodge material from below, material hit the air-lance and trapped the laborer's right forearm between the bunker's bottom opening wall and air-lance, causing muscle sprain.

Fall to the walkway or working surface

employee was on lunch for 20 minutes with other employees and was laughing and started to cough / chock, went to door and passed out causing EE to fall to the ground approx. 24inches, EE awake before medics arrived and vitals were normal. cuts to face and chipped a tooth

Over-exertion in pulling or pushing objects

On 9/9/22 at 8:20am, employee was cleaning the walkway for C-16B because rocks had piled up after a spill. Employee was shoveling walkway and planted their left foot, pushing up against a guard with right leg to move it inward. Employee felt a shooting sensation of pain on right side lower back, groin, and down the leg.

The water truck tail light needed to be changed. The mechanic performed a JHA and was wearing rubber/latex gloves to handle the lightbulb of the tail light. As the mechanic went to turn the bulb to remove it off the truck, the bulb exploded, causing a laceration to the thumb and index finger of the mechanic's left hand. The employee was escorted to the nearest hospital.

On Wed, August 12th at 11:15am, a CAT 770 haul truck operator was being utilized at the wash plant as an extra hand. The employee was washing out underneath the wet plant screen with a water hose in a squatted position, and when we went to stand up EE felt a pop in EE's right knee.

Struck by falling object

A WEDGE ON A PIVOT VICE BROKE, WHICH WAS MOUNTED ON BED OF SERVICE TRUCK. WHEN EMPLOYEE WAS REMOVING THE AIR HOSE THE VICE FELL ON EMPLOYEES LEFT PINKY TOE. THIS RESULTED IN A BONE FRACTURE.

EE walked to backhoe , the water truck had just gone by making his boots muddy. He slipped while getting into the backhoe. He twisted his arm while holding onto the handrail. He did not report incident until 5/17/10, when bicep was severely discolored. He was taken to the company doctor and diagnosed as sprained right shoulder and torn right rotator cuff.

Fall from machine

Employee mounted his dump truck without using the handrail. He reached for the steering wheel to pull himself in. He missed the steering wheel, falling from the elevated position down to the ground. He landed on his rear braced for impact with his left hand. Employee suffered a sprained wrist on his left arm.

The employee had spent approximately 5 minutes in a squatting position repairing a 1 1/2"" wash down hose. When he stood up his knee (left) buckled backwards re-injuring his knee. Sprain of left knee.
