Employee claims he slipped on loose debris. He says he fell and landed on the outside of his left thigh. The resulting contusion caused swelling in the left knee and lower leg. Employee reported this injury on 6 July 2011.

EE was holding slide gate in place while other mechanic attempted to knock it in while using a sledge hammer. The other mechanic swung the sledge, and it slipped off the slide gate and made contact with the EE causing an abrasion to the left hand. EE was taken for medical evaluation and given modified duty, antibiotic and pain meds.
EE was assisting in the cleaning of our mud tank. The mud tank consist of kiln dust and water mixed together resulting in a slurry type material. The alkali from the kiln dust caused the employee's feet to burn. The employee did not wear his rubber boots. Instead he wore his regular work boot to complete this task.
While EE was using vacuum truck he was attempting to remove some vacuum hose. The EE was using a razor knife to cut the hose when the razor knife slipped and cut the EE left forefinger.
Employee complained of chest pains during lunch and asked for medical responders to be called. Employee was transported to local hospital for evaluation and passed away early the next morning. Employee has history of heart conditons.
EE was carrying a guard and had it perched on his shoulder while walking on a platform when the guard caught on another existing guard and caused the employee to jerk his to his right and feeling a strain to his neck.
EE reported back pain while performing assigned task in dust control. EE was evaluated by Dr and prescribed prednisone and started physical therapy. EE was not given modified duty and has continued to work his regular assigned duties and shift.

EE was replacing bags in dust collector while bending over pulling the bag from the cage he felt a sharp pain in his lower back and hip.
EE was lifting a battery to put on shelf when the battery slipped from his hand and smashed his right middle finger causing a laceration that needed 9 stitches to close.
EE was taking measurement from silos and walking down stairs. When EE reached bottom steps he twisted his right ankle. EE was taken to local clinic for medical evaluation.
EE was lifting a rail car vibrator while unloading cement railcars and felt a strain in his groin. The vibrators weigh approximately 45 lbs. EE reported the injury immediately and was taken to the local clinic for medical evaluation.
EE states as he was lowering the boatsman chair to the next level, he felt pain in his lower right side of his back.

While moving a 4' ladder, the ladder folded pinching the right thumb causing bleeding from under the thumb nail and a tufts fracture.
Employee was adjusting a steel cable rigging on a ball mill. The cable tightened on the mill pinching his finger. This resulted in a closed fracture of the left index finger distal phalanx.
Employee was moving a tarp to cover a clinker pile. He claims he felt a sharp pain in his lower right abdomen. He was diagnosed with a hernia. He chose to have surgery after the job had ended on 3/28/08.
Employee was using a come along, lost his footing and rolled on his right foot causing a sprained foot.

EE was operating a forklift traveling westbound in plant. Service vehicle traveling eastbound in plant was temporarily blinded by sun and dirt on windshield. Resulted in head on collision. Service truck operator no injuries, and forklift operator slight whiplash. Sore in leftside neck and back.
Employee was removing a thin layer of cement build up from a trunion adjustment screw. The improvised scraper slipped and jammed against the trunion support. The edge of scraper cut the employee through his gloves. Stitches were required to close the wound.
Coal mill got choked up and the residual heat was drafted into the main dust collector and began smoldering. Shortly after that an explosion occurred at the main dust collector.
Employee claims to have received a small puncture wound while cutting belt material with a razor knife. Employee did not care properly for the wound. Employee did not report the incident or seek medical treatment until 7 days later. The result was a staph infection.
The victim cut through two chain linked fences and made entry into a Southern California Edison electrical substation, in attempt to extract copper. The victim climbed a 20ft 12kv tower in attempt to cut more copper ground wires. An arc occurred striking the victim in the top left part of his head. The victim was found deceased.
Employee was preparing to change belt at tailpulley. After locking out belt employee removed guard. The guard became wedged between the wall and the belt conveyoor framework. The employee tried to free up guard; once the guard broke loose it mashed his finger between the guard and wall causing a laceration and fracture to the employee's left index finger.

When opening a dust hopper manway door under waste heat boiler 2 a lining brick fell from the hopper manway striking a ball valve which opened the valve. This caused air to be blown into the hopper which expelled hot material on to Dennis Lanceta as he was moving away. Dennis received burns to the right side of his face and his right wrist.
THE EMPLOYEE STATES THAT THE INJURY IS A RESULT OF BEING JOSTLED AROUND WHILE OPERATING THE D10 CATERPILAR.

EMPLOYEE WAS KNEELING ON THE GROUND WORKING ON A PIECE OF EQUIPMENT. AS HE WENT TO STAND UP HE FELT PAIN IN HIS LEFT KNEE.
IT APPEARS THAT ONE OF THE #1 COAL MILL ROLLS LOST THE OIL THAT LUBRICATES IT. THIS ALLOWED THE ROLL TO BECOME VERY HOT WHICH THEN PROVIDED AN IGNITION SOURCE FOR THE GROUND COAL. THE IGNITION OF COAL CAUSED THE RUPTURE DISCS IN THE COAL MILL DUST COLLECTOR TO BLOW OUT AS THEY WERE DESIGNED TO DO.

AFTER THE EE WAS FINISHED GREASING THE COOLER FAN BEARING HE STEPPED DOWN FROM THE PLATFORM ONTO A PIECE OF ELECTRICAL CONDUIT THAT WAS STICKING UP FROM THE GROUND CAUSING HIM TO SPRAI N HIS RIGHT ANKLE

WHILE TIGHTENING BOLTS IN A BARELY ACCESSABLE LOCATION THE WRENCH SLIPPED FROM THE BOLT AND THE EMPLOYEE STRUCK HIS NOSE WITH THE WRENCH. HE WAS EXAMINED (X-RAYED); POSSIBLE HAIRLINE FRACTURE. NO MEDICAL TREATMENT WAS PROVIDED; NO WORK RESTRICTIONS WERE IMPOSED.
EMPLOYEE WAS DRIVING A LOADED 16 CU YD DUMP TRUCK INTO THE QUARRY TO DUMP CLEANUP MATERIAL FOR RE-PROCESSING WHEN HE LOST CONTROL OF THE VEHICLE CAUSING IT TO TIP ONTO THE DRIVERS SIDE.
EMPLOYEE WAS WALKING UP STAIRS WHEN HE FELT PAININ HIS RIGHT CALF.

EE WAS LIFTING A SCREW CONVEYOR TO UNHOOK IT FROM THE CHAIN. THE SCREW BECAME UNSTABLE AND FELL LANDING ON THE EE'S RT. FOOT.

EE WAS CLIMBING A FIXED RUNG TYPE LADDER HOLDING A PIPE WRENCH IN HIS OTHER HAND. EE MISSED THE LADDER HAND RAIL UPON REACHING THE TOP OF THE LADDER CAUSING HIS BODY TO TURN WHICH CAUSED HIM T O LOSE HIS FOOTING.
EE HAS FILED A CLAIM FOR HEARING LOSS BASED ON EVALUATION BY AN AUDIOLOGIST.
EE WAS REPLACING BELTS ON BLOWER PULLEY WHEN HE CAUGHT HIS HAND BETWEEN THE BELTS AND PULLEY. INJURY RESULTED IN 7 SUTURES TO RIGHT LITTLE FINGER.

EE WAS GRINDING THREADS OF BOLTS IN A CLOSE AREAWHEN THE GRINDER JUMPED BACK UNDER HIS FACE SHIELD AND CUT HIS CHIN. THE CUT WAS ONE INCH IN LENGTH AND TOOK EIGHT STITCHES TO CLOSE.

A 1:15PM THE EE'S WIFE CAME TO PLANT & SAID EE PERSONAL DR SAID HE SHOULD FILE A STRESS CLAIM WHICH HAS AFFECTED HIS ANGINA. THE COMPANY IS DISPUTING THE CLAIM THRU INS. CARRIER. HIS DR TOOK HIM OFF WORK DUE TO ANGINA
LOWERING FEEDER ONTO COAL MILL WITH CHAIN FALL. CHAIN FALL JUMPED DROPPING FEEDER ONTO COAL MILLPINCHING FINGER IN BETWEEN.
EE WAS IN A HURRY AND DID NOT DON THE APPROPRIATE PPE AND DID NOT ISOLATE. THE VALVE PRIOR TO DISCONNECTING THE AIR HOSE. WHEN SPEAKING WITH THE EE HE KNEW THAT HE SOULD HAVE SLOWED DOWN AND T HOUGHT ABOUT THE TASK AT HAND, THE PROPER PPE AND THE PROCEDURE OF ISOLATING THE VALVE.

EMPLOYEE WAS REMOVING A FOREIGN OBJECT FROM THE CLINKER FEEDER WHEN DUST FLUSHED AND GOT INTO HIS EYES.
FOLLOWING OUR ANNUAL AUDIOMETRIC TESTS, THE EMPLOYEE WAS NOTIFIED OF AN OSHA SHIFT - STS & IT WAS SUGGESTED HE SEE A DOCTOR. JUNE 16 WE RECEIVEDNOTICE FROM HIS ATTORNEY THAT ON JUNE 6 HE FILED A DWC-1 FORM. WE ARE REPORTING TO MSHA AS THE HEARING LOSS MAY BE RELATED TO HIS JOB, BUT EVALUATION IS IN PROCESS.
EE WAS ADJUSTING HIS POSITION STRADLING A SCAFFOLD PLANK. INJURED PLACED HIS HAND ON A 9"" GRINDER CAUSING IT TO START. HIS LEFT HAND WAS IN A POSITION TO KEEP HIS BALANCE. IT CAME INTO CONTACT WITH THE GRINDING DISC CAUSING A LACERATION AND8 STITCHES.
