AB plant fixing the Universal crusher. Turning the blow bar by hand. The only way to move it with the power locked out is to put your weight into. While EE was turning it, EE's glove got caught on the blow bar. This caused EE's left hand to be caught between the blow bar and the wall of the universal crusher causing contusion.

Climbing onto the loader and put hand on a bee that had landed on railing and got stung.

Employee was carrying a bag of ANFO to load a hole and twisted left ankle when employee stepped on a loose rock. Employee was given an x-ray that was negative and placed on restricted duty.

EE has incurred a reportable hearing loss in his right ear. First 2012 annual audiogram test was on 10/31/12 and retest was given on above date. The retest confirmed a right ear shift. EE is sch. to see an ear specialist. EE was diag. by his personal Dr. to have an ear infection 11/30/12.

He compromised the guarding in order to knock dust off the conveyor frame. The hammer he was using was grabbed by the conveyor belt pulling it and right hand between the troughing roller and underside of the belt. Hammer got wedged between the skirtboard and roller causing belt to stall. His right hand, thumb and first two fingers were pinched between the belt and troughing roll.

Employee was replacing screen cloth sections on the second deck of the Telesmith screen at the AB Plant when he felt pain in his right shoulder. The employee was taken to a local health care facility and returned to work on restricted duty.

Struck by flying object

Employee was assisting with the assembly of the upper frame of a Pegson cone crusher. A driving maul tool and 20 pound sledge were being used to help drive the concave wedges in place as a metal chip struck the employee in the left hand. The employee was taken to a local healthcare facility and stitches were required to treat lacerations in his left index & middle fingers.

Employee was assisting with the installation of a new rubber skirt on the oversize chute at the sand plant. When he reached for and pulled on the bottom right corner of the rubber piece he felt something pop in his right arm.

Employee was clearing a rock plug from the blue rock plant's C1 conveyor discharge chute and slipped when dismounting the work area. He fell onto the hand pulley metal skirting and lacerated his left buttock. Medical treatment was adminsitered at a local emergency room and the employee was placed on modified duty.

EE was adjusting dust control water valve at the blue rock jaw crusher as a loader dumped rock into the plants feed hopper, a small flying rock struck and scratched his left eye. EE's prescription glaases did not come with side shields.

Employee was cleaning and rearranging screen storage room when a yellow jacket bee was encountered. He tried to swat the bee and accidentally hit a wooden shelf with his left hand and jammed his long finger. Injury was diagnosed as left long finger avulsion fracture on 5/7/08 and employee was placed on modified duty.

Employee was dismounting loader and missed step. Injury was diagnosed as a lower back strain and employee was placed on restricted duty.

EE was removing hammers on the impeller at the AB Plant when the wedge preventing the impeller from rotation broke loose. The impeller rotated throwing the EE against the backwall(curtain). EE attempted to break the impact with his arm and struck his left elbow. He worked the rest of the week but pain and inflamation worsened and he went for medical treatment over the weekend.

Employee was operating a 988B cat loader sorting riprap. A rock fell from the loader bucket and was driven over. Employee shook fines from clamp bucket and backed over the same rock. Seat bottomed out and jarred operator's left hip. The injury was diagnosed as a left hip jamming contusion on 11/17/06 and employee was placed on restricted duty.

Employee was mixing cement mortor to use for the new installation of the outbound truck seale for the napa quarry. a square end margin trowel was being used as a mixing tool. While mixing the mortor with his left hand the employee felt pain in his left wrist that radiated up to his elbow. the injury was diagnosed a left wrist strain on 7/14/06 and ee was placed a modified duty

EE was completing repairs to a Lippmann swing jaw shaft assembly. Task at hand was to plug the threaded puller holes in the shaft bearing housing. When rotating housing to locate the holes his left 5th fingertip was caught between the housing & counterweight clamp bolts. Injury was diagnosed as a left 5th finger crush injury on 5/31/06 & he was placed on modified duty.

EE was adjusting the open setting of a 48"" gyradisc crusher to assist with the unplugging of the machine. When changing the position of the crusher's hydraulic ram his left long finger got caught between the cylinder and the ram head injury was diagnosed as a left long finger crush injury on 5/12/06. EE was placed on medical duty.

EE was placing a section of drill steel on his drill while performing his normal job duty. While placing the steel into position he felt a sudden pain and burning in his right groin area. Injry was diagnosed as right groin strain on 2/23/06 and EE was placed on modified duty.
