On 8/12 employee was descending tower stair case and slipped on step and twisted lower back. On 8/19 it was brought to our attention as they wanted to seek treatment .

Employee was injured while attempting to dislodge a rock from the portable crusher. While standing on the short stub conveyor and using a pry bar, they lost balance and fell approximately 4 feet onto the stacker conveyor below, resulting in an injury to right shoulder.

Changing drill hammer on drill rig putting on Jay wrench onto the hammer and wrench dropped on employee's left hand crushing the tips of EE's two middle fingers. EE needed stitches to close the cuts on both fingers.

Employee step down on two steps stair and twisted his ankle then fell to the ground. The ankle swelled and, so employee left to go home in Sparks Nevada. Employee went to a health clinic and the ankle is fracture. Employee has not missed any work days as he is an administrator and will be working from an office to do his work.

EE is a dump truck driver/owner operator working thru T407. As he was on standby work due to a blasting operation, he was staged outside the pit area and in the North/East area of the quarry. While standing outside of his truck on a pile of material, he proceeded to step off the pile and lost his footing, thereby falling onto the ground.
Injury occurred while pile butt was repairing handrail at the Quarry spring line. He cut the handrail away and lost footing resulting in a fall to the water below, where he struck a float in the water. This resulted in two broken ribs and a punctured lung. Employee will be off duty until he has rehabilitated. Lost time injury.

While removing a rubber lined wear plate, the plate came free and struck employee on the foot above the steel toe causing a laceration.

Transporting wheelbarrow when it began to tip over. Caused employee to be unbalanced and he incurred a strain to his lower back.

Employee was operating loader to place broken rock into haul trucks, while digging, the loader contacted unbroken rock which stopped the loader causing the employee to be jarred resulting in pain in lower back and pain radiating into left leg.
Walking on level but uneven surface and felt a twinge in lower right back. Pain became more severe and notified supervisor.

While cleaning out a rip-rap chute with pry bar a rock moved and the pry bar struck the employee on the right shoulder. Xray was negative. Slight bruise to right shoulder.
While loading a barge, operator was attempting to make slack in a hand line and the line struck him in the back of the right hand causing a break between the little and ring fingers.

HE SLIPPED AND FELL WHILE WALKING ON THE LOADING DOCK. HE STRUCK HIS LEFT HAND ON A METAL GRATING CAUSING A LACERATION. PROVIDED GLOVES BUT WAS NOT WEARING THEM.
Ee was clearing diester of rock that was blocking passage. He was using a sledge hammer, the handle broke, causing right hand to strike rock.
Employee was patching hole on the barge when hydraulic ram shifted unexpectedly causing multiple pieces of debris to hit right temple area.
While changing punch plates in diester at primary plant, he was in a hurry and not wearing gloves. Lacerated his fingers on punch plates.
