Fall from scaffolds, walkways, platforms

Employee received a load of cement by the TXI scale person. After he received his load he pulled forward to a stationary rack designed for safe access to the top of each trailer. He closed all but 1 latch on the A trailer when he fell to the ground landing on his head. He was found at 1:30 pm lying on his back behind the second axle driver's side.

Fall from scaffolds, walkways, platforms

Employee was assessing work area at floor opening. Employee fell into floor opening. The ensuing fall resulted in fatal head injuries.

The employee was assigned to inspect the clinker weigh feeder due to clinker chunks that could plug the system. They lowered themselves through the safety cage to reach the clinker weigh feeder, dropping their hard hat as they moved. They hit the pins with their head as they moved up, causing a laceration.

The employee was cleaning a cement coupling with a drill grinder. The drill bit became dislodged from the drill and lacerated the employee's left index finger.

The employee was removing a rock from the grizzly head pulley at the 551BC101 conveyor belt when another rock fell and smashed employee's left index finger.

Fall down stairs

The employee was walking up the stairs on the front (west end) of the building and missed the top/last step. Employee fell face first and opened a 2"" gash above right eye.

The employee was working on belt 611FN360 when their finger reached a pinch between the belt and the sheave. The ring and pinky fingers of their right hand were lacerated.

EE was putting new drive belts on the tail pull. As EE rolled it over EE's finger became caught between the pully and the belt causing a pinch.

Fall onto or against objects

The employee was driving the quarry pick-up truck and stopped to speak with the luber. As the conversation continued, the employee began to walk backward to the pick-up and lost EE's footing. The employee stumbled backward, and EE's left leg hit the bottom corner of the driver's door, causing a laceration. The employee caught self on the door and did not fall.

The employee was working on pump #331PP102, rotating coupling. The employee pinched their left index finger between the locking collar and bearing housing.

Over-exertion in lifting objects

Employee lifted an elevator bucket from the ground to the back of a truck.

Employee was cleaning the hydrated lime system. They pulled the rubber hose connected to the inductor, and the fitting broke off. The employee fell backward and hurt their lower back. The incident occurred on February 8, 2023; the employee refused medical treatment. Employee decided they wanted to see a physician on February 13, 2023, where modified duties were placed.

Two employees were working on the blending limestone way-feeder, removing pins. One of the employees was pulling pins when the other employee swung a four-pound sledgehammer and smashed left index finger, causing a fracture.

Struck by falling object

The employee was walking under the under-screen conveyor without a hard hat. When the conveyor started to move, approximately a three-inch rock struck the employee on the head, causing a laceration.

The employee used a pallet jack to push pallets to get them closer to the edge of a trailer, where a forklift would pick them up and transfer them to another location.

Four employees were stowing the jib on a Link-Belt RTC 8090 crane and were on the platform of the crane. The jib was not adequately stowed correctly; the crane operator telescoped the boom out, and the fly-tip head sheave was pushed towards and bent the front fly storage bracket. This caused the jib to swing out violently, striking two of the four employees.

Four employees were stowing the jib on a Link-Belt RTC 8090 crane and were on the platform of the crane. Driving on a Taylor Dunn cart to a freight elevator onsite. Starting to feel pain while walking the stairs.

The loader operator was loading a haul truck. The loader operator had loaded the last load and blasted the horn, haul truck operator started to drive forward when a loud and hard impact was felt on the driver side of the haul truck. The loader bucket hit the haul truck on top of the bed. This impact whiplashed the haul truck operator, causing a neck strain.

Fall from ladders

The employee was removing the filter housing, to change the fuel CNG filter on a truck while standing on the first step of the stepladder. As EE put force on the pry bar, it caused the socket to slip off the nut, causing EE to fall from the ladder. EE landed on EE's left leg, where EE thought EE pulled a muscle.

Fall down stairs

While going up the stairs to the coal mill, the employee was carrying a bag of gypsum with both hands, and lost EE's balance and fell. The employee stated that carrying the bag, prevented EE from catching the handrail. EE's left ankle was twisted from the fall.

Over-exertion in lifting objects

The crew was installing new parts in the clinker cooler. When EE felt sharp pain in my lower back. The following night shift there was tightness/tingling in Right leg and hip. After no improvement on night shift on 2/12/2022, EE reported it to safety at 5:30am, 2/13/2022. The object the employee was carrying was approximately 18 lbs.

Employee was cleaning sprayers on a downcomer and when putting in the long sprayer it slipped and crushed employees left pinky.

Opening fine coal bin, closed the bin. The explosion door blew open and flames came out burning the employees.

Investigating how to open the coal bin to clean it out. The explosion door was open then closed. Hot gases came out and burned the employee.

Fall from machine

EE was cleaning off the back of the welding pickup truck (flat bed). When finished, EE ""jumped"" off the back of the flatbed onto concrete ground. EE stated did not feel any pain or discomfort at this time to classify an injury. EE reported the event the following morning as EE began to feel pain and noticed swelling, but did not request medical treatment until 10-06-2021.

Employee hit a bearing race with a hammer. the bearing race shattered and a piece of metal struck employee in neck.

Employee torched to remove a bad bearing and slag build-up. Employee used a grinder to remove the slag when a small piece of steel flanged from the grinder into employee's right underarm.

The employee was walking down the tower stairway. In the process of turning to the next flight, EE sprained EE's left knee.

Employee was working on cleaning a mixer at the conclusion of a job. The equipment was secured and properly deenergized, the employee then started to clean the tub and mixing blades of the mixer. As the employee was manually turning the mixing blades and removing material, EE's finger was pinched between the mixing blade and the tub causing a laceration.

Fall from machine

Customer driver fell from the top of truck while opening the lids to be loaded with cement. Driver did not utilize the adjustable chains to ensure the safety cage was all the way down to close gaps posing fall hazard. The driver also was not properly aligned leaving gaps in which worsen the potential fall hazard.

Employee was loosening base bolts on the #1 bearing assembly drive end of FM1 when the #2 cylinder valve broke off and struck the employee on the inner side of the left leg.

Fall onto or against objects

Employee conducting maintenance on broken screw. Applying tension w/pipe wrench & come-a-long, during prep for welding - the wrench slipped causing employee to fall backwards, hitting elbow on guardrail. Injury non severe so employee continued to work. Employee realized the next morning that the pain had not gotten any better and pursued further evaluation.

Employee was using a cardox tube to shoot and clear a plug in a port on the preheater tower. When employee removed the tube from the port minimal material was released and worked it's way under employees PPE (Aluminized Coat/Aluminized Gloves) causing minor burns to employees left wrist.

Over-exertion in lifting objects

Mechanic was removing a rear bumper and spare tire from a pickup truck on a hydraulic lift. Employee utilized co-workers for help removing both parts. Employee did not notice any discomfort at time of removal. After completing the task and taking next scheduled break, employee realized that ee had discomfort to lower back.

Operations of heavy equipment over a ""long period of time"". Employee states that EE has lower back pain, not directly related to one single incident. Employee on restricted duty, not lifting over 10 lbs., no mobile equipment operations, and ""sitting work only"".

Accident type, without injuries

CAT 777D operator was moving waste to the waste dump at mack's peak. The operator dumped load and started to drive away with the bed in the up position. The operator traveled 340 feet before making contact with a 12kv overhead power line. The power line made contact with the raised bed causing the line to arc and pole to break. No injuries occurred during this incident.

EE was walking and stepped on a rock and heard a pop in right knee. Pain was minimal EE did not seek medical attention until 7/19/17. EE treated as FA and released back to work with no restrictions or pain meds. EE went back to Dr on 8/24/17 and placed on modified duty. EE stated knee still pops and pain is still lingering.

EE reported injury on 5/11/17 but did not want medical evaluation until 5/12/17. EE stated while walking during shift felt a sharp pain in lower back. EE cannot recall anything they did that caused the pain. EE taken for medical evaluation and placed on modified duty for 4 working days.

EE was taking the passenger elevator on the preheater tower when a gust of wind moved the power cable and got attached to 8"" piece of conduit. The power cable wrapped itself around the conduit and pulled the power cable apart, electrically disabling the elevator car. Fire rescue and Executive Elevator had to be called to rescue the EE from the elevator car.

The EE was hammering in pins on feed chute, while hammering the pins the EE struck the pin and EE's hand at the same time. The pin caused a laceration that required stitching to close. The EE was taken off work for two days and placed on modified duty after that.

EE was clearing dust collector pipes, when EE was clearing the pipe the tipping valve started moving and hit EE above the left brow causing a laceration that required medical attention. The laceration was closed and EE placed on modified duty.

EE was performing pre-shift examination of EE's 992C front end loader and while walking around the backside of the loader EE stepped on a rock that caused EE to loose EE's balance. EE twisted EE's knee and heard EE's knee pop. EE was taken for medical evaluation and placed on modified duty.

EE was welding flange on top of station 4 silos, when EE finished welding on one side of the flange the EE lifted welding hood to inspect work and move to the other side of the flange. While the EE was stepping to the other side the welding hood dropped in front of face during mid-step causing EE to fall onto shoulder. EE placed on modified duty.

EE was attempting to cut a plastic vacuum hose for the vacuum truck and while doing so the EE cut towards left hand that was being used to hold the hose with male fitting on the ground. The knife skipped across the hose and cut the EE's left hand. The cut required 5 stitches to close and EE was placed on modified duty for 4 days.

EE was installing hydraulic ram at top of finish mill elevator. EE attempted to kneel down to work from his knees and hit his right knee on the bearing of the mill elevator. EE reported the injury on 11/14/15 and seen at Dr. office and placed off work until 11/17/15. Returned on modified duty and released to full duty on 11/20/15

Employee was at end of shift storing his tools in his cabinet, which is the normal location when he heard and pop and felt pain in his right shoulder.

Employee was using a truck rack, as he was pulling it back into stored position he felt a pain in his right knee. Employee refused medical treatment at time incident was reported. Requested medical treatment that following Monday. Employee had placed himself in an awkward position which contributed to this incident.

Employee was using an air lance, wearing all appropriate heat resistance clothing when material blew under heat resistant hood. Hot material was blown on employee's face (nose, cheeks, brow and upper lip)

Struck by flying object

Employee was using a sledgehammer to drive a wedge behind liner. A small piece of metal broke off and hit him in the stomach.

Employee was stepping out of storage shop, stepped on pebble twisting/spraining right ankle.

Employee was wearing appropriate heat resistant protective clothing working on shutting off the air to an air cannon, as he shut off the air flow, the air cannon ""popped"" causing hot material to become airborne. Material fell down the hot work gloves causing burns to both wrists.

Employee was pushing a broom, cleaning up coal from around the coal unloading area. She stated she stepped on a piece of coal which caused her left knee to twist. She denied medical treatment and working through the rest of her shift without any issues (incident happened at 9:00 a.m. employee's shift ended at 3:30 p.m.)

Employee stated that he strained his lower back while operating a loader maybe 2-3 months ago but is unsure but know he did it at work. Employee stated that the bouncyness in the loader aggregated his lower back.

Employee was operating a loader, as he pushed the loader bucket into the stockpile, he stopped. During that time a rock rolled from under the bucket to under the left front wheel. As the operator backed away from the stockpile the left front tire drove over the rock, causing the tire to slam down on the ground.

Employee reported incident on December 15, 2011, which he stated occurred on December 13, 2011. Employee stated that while performing housekeeping duties in the storeroom he sustained a pulled muscle in his left shoulder.

Employee stated injury occurred on Saturday, 6/5/10 but reported it on Tuesday, 6/8/2010 as Report only, no medical treatment requested. Company sent employee to clinic where employee stated he was in a lot of pain. Employee was taken off work for a couple days. Injury sustained was lower back pain while climbing vertical ladders.

DEMO ACTIVITY: NON-MINING OPERATION: Employee was cutting off end cap on beam, which was on the ground. After the cut the employee moved forward to move his welding lead and the beam dropped the 7"" onto the top of his steel-toed boots. Employee sustained three broken toes on left foot. Employee is off work until toes heal.

EE was welding at the top of polycom roll. Area was being heated with propane and diesel heaters to maintain the proper temp. of roll being welded per specifications. Goggles and welding hood #12 lenses were being worn. EE flushed eyes when irritation began and during shift, however burning sensation increased by end of shift.

Unattentiveness, not watching where walking, no keeping three-points of contact while descending stairs. While descending stairs employee was carrying items, looked away, missed the bottom step and fell forward onto ground. Abrasion to right knee and contusion to left hand. Employee was using a hammer drill. As it rotated, the drill handle caught 2 of his fingers and the drill rotated out of his hand. The following day he went for x-ray which revealed hairline fracture to small finger and ring finger on right hand.

Employee was in manlift when the bottom portion of the cage came in contact with the bolts on the outside of the Finish Mill. This caused the manbasket to jerk, causing the employee to hit his left side on the hand rails of the manbasket. Employee was wearing harness with secured lanyard while in manbasket.

LATE REPORTING BY EMPLOYEE. NOTIFIED COMPANY ON 06/11/2009. Employee was picking up trash stood up and hit his head on cross beam.

No Injuries occured when the hydraulic line broke below Clinker Cooler Bedgrate causing oil to be picked up by the cooling fan air flow and taken into the chamber with hot clinker. The oil mixed with the high temperatures caused an ignition.

Injury was reported on 8/21/07 with no work restrictions. Aggravation of the injury, according to the doctor, caused the need for left hip surgery. Employee underwent left hip surgery on 4/27/09. (No accident or injury was reported after 8/21/07).

Employee late reported accident on 3/16/09. Employee stated that while she was pulling up dust collector bags she felt a pain in her left shoulder. She felt that it wasn't much so didn't report it. During the lay-off she stated the pain was bothering her so when she returned on 3/9/09 she felt she needed to report it on 3/16/09. (Improper Lifting Procedures)

Injury was a contusion below left knee, which occurred while employee closing sectional elevator door. Employee was returned to full duty date of injury. However area started to swell and discolor, so employee when back to doctor on 2/3/09. Doctor lanced the bump and inserted a drainage tube. Employee then placed off work as of 2/3/09.

Employee was assisting in removal of a faulty diverter valve using bull rigging when the valve got in a bind. Other employees helped to free it from the bind. When the valve freed from the bind it swung around and caught the employees arm in between the valve and a pipe support stand.

The tips of the forklift forks had placed the beam on its side so that it stands instead of lying. The beam became unbalanced and began to fall.
