Flash burns (electric)

There was an opening in the bottom of the attached J-box to the load side of the adjacent breaker. This J-box was used as a raceway for cables going to the load side of the de-energized breaker and another set of cables that were abandoned. While disconnecting the drain wires from the cables one of the drain wires landed on the load side of the adjacent breaker and shorted out.

Struck by falling object

Employee lifted up chocks from side of wheel of forklift, went to toss the chocks in the bed of the forklift, chocks caught on frame causing chock to be pulled out of the employee's hand falling on top of right foot.

5/31/13 at approx. 4:30am, while hammering on the mill liner, a shard of metal came off caused by the concussion of the hammer and impacted the leg of one of the contractors on site. The injury was reportedly minimal and did not require the immediate need of a medical professional.

EMPLOYEE STATED ON 12/3/08 THAT HE SUFFERED A WORK-RELATED INJURY ON 5/5/08 WHILING LIFTING BAGS IN THE PACKHOUSE. HE HAD BEEN TREATING THIS UNDER HIS PERSONAL INSURANCE UNTIL HE FOUND OUT THAT HE WAS SUFFERING FROM SLIPPED DISC. (NO MANAGEMENT KNEW OF THE ALLEGED INJURY UNTIL 12/3/08)

The EE was working on the DC on the bin building. While walking down the stairway he fell. He sustained a bruise to his left heel and twisted left ankle.

While working on electrical cart in shop rewiring tail lights, a wire started to smoke. He reached to grab the wire to remove it from the power source and it burnt through his glove and into his finger.

EE was trying to open the knock over valve for #7 silo, when he pinched his right hand-ring finger between the bar he was using to pry the valve open and the recirculative fkline, the valve was sticking and would not open all the way.

While welding the employee did not wear ear muffs, only ear plugs. A spark entered right ear burning it.

THIS INJURY OCCURRED ON THE STACKER ON BC7. (ON THE 2ND FLOOR BY THE HEAD PULLEY FOR THE WEST BOOM.) HE GOT DUST IN HIS RIGHT EYE WHLE TRYING TO UNCHOCK THE WEST BRIDGE BELT.
