There was an opening in the bottom of the attached J-box to the load side of the adjacent breaker. This J-box was used as a raceway for cables going to the load side of the de-energized breaker and another set of cables that were abandoned. While disconnecting the drain wires from the cables one of the drain wires landed on the load side of the adjacent breaker and shorted out.
Employee lifted up chocks from side of wheel of forklift, went to toss the chocks in the bed of the forklift, chocks caught on frame causing chock to be pulled out of the employee's hand falling on top of right foot.
5/31/13 at approx. 4:30am, while hammering on the mill liner, a shard of metal came off caused by the concussion of the hammer and impacted the leg of one of the contractors on site. The injury was reportedly minimal and did not require the immediate need of a medical professional.
Employee missed grab hold on forklift, causing pain in upper right arm.
While running up the stairs with both hands full employee mis-stepped causing him to stumble and banging his right wrist on the hand rail and twisting his back. (Lay off planned for 12/12/08)
EMPLOYEE STATED ON 12/3/08 THAT HE SUFFERED A WORK-RELATED INJURY ON 5/5/08 WHILING LIFTING BAGS IN THE PACKHOUSE. HE HAD BEEN TREATING THIS UNDER HIS PERSONAL INSURANCE UNTIL HE FOUND OUT THAT HE WAS SUFFERING FROM SLIPPED DISC. (NO MANAGEMENT KNEW OF THE ALLEGED INJURY UNTIL 12/3/08)
Hatch lid on tanker truck struck employee while opening. Employee was placed back to full duty at time of accident and then on 2/22/08 after doctor visit was placed on Modified Duty.
Placement of body inbetween stationary and moving equipment.
The EE was working on the DC on the bin building. While walking down the stairway he fell. He sustained a bruise to his left heel and twisted left ankle.
Employee claimed he sustained swelling in his hand due to repetitive trauma. He continued to work his normal duty until being taken off work on 11/6/07.

While operating a loader the employee was being bounced around in his seat and felt a pain in his lower back.
Lighting went out in area employee was traveling while accessing stairs. Employee missed a step causing him to trip resulting in abrasion to his knees and arms.
Cleaning area around #3 Grey Finish Mill, twisted and heard knee pop.
Employee was using a wire whell on a 41/2"" grinder when the wire wheel got caught on the tie wire causing sprain of left hand pinky finger. EE was dispensed medication. EE was released to full duty on March 16, 2007. The project ended on March 16, 2007.
Improper dismount from loader- strained left knee
Using hammer to unchoke plugged chute. Hammer was being swung across the employees body and overhead in attempt to loosen material stuck in chute. Strained right hand.
While lifting a full bag of cement from the pallet, he felt a strain in his lower back. Was given off work order from Doctor - 9/13/05. Rx - Naproxen, Soma, 1/H Pack, Banalg, Lumbar Support, Acetaminophen, Vicodin, cane.
While putting cement bags on packer he twisted lower back. Rx - Naproxen, Tylenol, Acetaminophen, Banalog, Soma, Vicoden, - Receiving therapy.
While working on electrical cart in shop rewiring tail lights, a wire started to smoke. He reached to grab the wire to remove it from the power source and it burnt through his glove and into his finger.
She was collecting paperwork at the packhouse when she missed a step, twisting her ankle and shoulder.
While adjusting bags on districutor belt because palletizer mis-counter, he strained lower right side of back.
EE was trying to open the knock over valve for #7 silo, when he pinched his right hand-ring finger between the bar he was using to pry the valve open and the recirculative fkline, the valve was sticking and would not open all the way.
While welding the employee did not wear ear muffs, only ear plugs. A spark entered right ear burning it.
THIS INJURY OCCURRED ON THE STACKER ON BC7. (ON THE 2ND FLOOR BY THE HEAD PULLEY FOR THE WEST BOOM.) HE GOT DUST IN HIS RIGHT EYE WHLE TRYING TO UNCHOCK THE WEST BRIDGE BELT.

ON PREVIOUS DAY, EE WAS PERFORMING MAINTENANCE WORK ON PALLETIZING EQUIPMENT IN THE PACKHOUSE. THE NEXT MORNING, WHEN HE ARRIVED AT WORK, HIS LEFT EYE WAS VERY SWOLLEN AND IRRITATED.
TRUCK DRIVER CUSTOMER WAS TYPING HIS LOAD. HE WAS STANDING ON HIS TRAILER, PULLING ON ROPE TO TIGHTEN, WHEN THE ROPE BROKE, CAUSING DRIVER TO FALL.
EMPLOYEE WAS HAMMERING A KEY INTO PLACE IN A TIGHT SPOT AND TOOK HIS GLOVES OFF TO GET INTO THE SPACE EASIER.
EMPLOYEE WAS STANDING BESIDE MILL, PREPARING TO ASCEND LADDER WHEN CEMENT DUST FLEW INTO EYES.
WHILE ASSISTING SUPERSUCKER OPERATOR PULLING A LENGTH OF SUPERSUCKER HOSE TO THE TOP OF MILL, 6' PIECE BROKE OFF, HITTING INJURED EMPLOYEE'S HAND, CUTTING IT ACROSS TOP.
WHILE CUTTING A PIECE OF STEEL WITH A TORCH, A PIECE OF SLAG FLEW BACK INTO EE'S EYE.
EE WAS MOVING LOCKERS IN THE MAINTENANCE DEPARTMENT. THE NEXT MORNING EXPERIENCED BACK SPASMS. APPARENTLY, LOCKERS WERE TOO HEAVY TO BE MOVED BY ONE PERSON.
DUE TO HEAVY DUSTING, EE PLACED A PLATE OVER OPENING TO DISCHARGE OF AIRSLIDE. WHEN REMOVING PLATE, IT HAD A LOT OF CEMENT ON IT AND WAS TOO HEAVY FOR EE TO HOLD, CAUSING EE TO FALL BACK BENDI NG THUMB BACKWARDS.
EMPLOYEE WAS SPREADING GATE APART ON TRASH BIN WITH A PORT-A-POWER JACK AND EXTENSION. HE MOVED AND STRUCK HOSE ATTACHED TO JACK, CAUSING THE JACK AND EXTENSION TO JACKKNIFE AND HIT HIS FACE.
EE WAS OPERATING FORKLIFT ALONGSIDE RR TRACKS. THE AREA WHERE THE LIFT WAS BEING USED WAS UNEVEN AND THE FORKS WERE NOT AT THE PROPER HEIGHT, HITTING THE RR TIE, CAUSING EE TO BOUNCE FORWARD INTO STEERING WHEEL
EE WAS WALKING UNDER BC-31 BELT WHEN HE SLIPPED ON A CLINKER BALL & FELL ON RIGHT SIDE OF BODY BRUISING UPPER ARM & ABRADING LEG
AFTER MAKING SEVERAL TRIPS UP AND DOWN STARIWAYSTO DUST COLLECTORS, EE BEGAN TO FEEL PAIN UNDER HIS LEFT KNEE CAP AREA.
EE WAS STEPPING ONTO PALLET ROLLERS WHEN HE BUMPED HIS HEAD WHICH CAUSED HIM TO SLIP, BRUISING AND ABRADING HIS RIGHT SHIN.
PARTICLE FLEW INTO EMPLOYEE'S EYE WHEN HE WAS UNWRAPPING PALLET OF BAGS.
EE WAS EXITING BOBCAT WHEN HE FELT SLIGHT PAIN IN RIGHT THIGH
AS EMPLOYEE WAS EXITING BUILDING, MISSTEPPED ON STAIRWAY AND TWISTED RIGHT ANKLE.
EE EXPERIENCED BACK PAIN AFTER SPENDING SEVERAL HOURS REBUILDING PALLETS.
EE WAS BEATING ON SPOOL ON DUST COLLECTOR, UNCHOCKING HOPPER WITH VIBRATOR & HOPPER DOOR CAME LOOSE & FELL ON EE'S SHOULDER
EE WAS DRILLING THRU STARTER BUCKET DRILL CAME IN CONTACT WITH BUSS BAR, CAUSING A FLASH BURN TO EE'S HAND
WHILE LIFTING COMPRESSOR ONTO BALL HITCH, FINGER WAS PINCHED BETWEEN BUMPER AND TONG.
CONTINUOUS TIGHTENING & LOOSENING OF HANDLE ON XPRESS MACHINE CAUSED EE TO FEEL PAIN IN LEFT HAND
WHEN EE DROPPED STOP PIN OF TRANSFER CAR, HIS FINGER CAUGHT BETWEEN DROP PIN AND HANDLE.
EMPLOYEE OPENED HAMMER VALVE ON PRESSURIZED LINE AND GOT HOT CEMENT DUST IN HIS EYES WHILE ATTEMPTING TO RESEAL.
WHILE WORKING IN THE FINISH MILL, EMPLOYEE GRADUALLY NOTICED DISCOMFORT IN LEFT EYE.
WHILE CUTTING MATERIAL, IT POPPED, CAUSING METAL TO GET UNDER SAFETY GOGGLES.
WHILE MANEUVERING DOOR OUT OF COMPARTMENT, WASHER ON LINE BROKE, CAUSING DOOR TO FALL AGAINST EMPLOYEE, KNOCKING HIM DOWN.
