Struck by... (Not Elsewhere Classified)

Employee was cutting a belt that was positioned on the ground. The safety knife slipped and contacted EE's right leg causing a laceration to EE's right calf muscle, resulting in stitches. Full duty, same day.

Over-exertion in lifting objects

Employee bent over to pick up a tie-off rope to secure a barge. When EE went to put it on EE's shoulder, EE felt intense pain in lower back and groin. EE is being referred for an ultrasound to rule out a possible hernia; placed on restricted duty until confirmation from ultrasound.

Over-exertion in lifting objects

Employee palletizing bags; felt discomfort in abdomen on 2/7/24. EE did not notify supervisor until 2/13/24 when discomfort increased while moving bags of sand. Employee was seen by an occupational physician on 2/13/24. EE was placed on precautionary lifting restrictions of 5 lbs. pending the results of the ultrasound. No physical signs of hernia present during initial exam.

Struck by flying object

Employee was cleaning rust/debris from floor in the engine room. The generator kicked on and exhaust air stirred up dust, causing irritation to left eye. Employee sought medical attention on 5-27-23 after irritation persisted overnight; no foreign objects were found to be in the eye: however, the DR prescribed an Rx antibiotic eye drop as a precautionary measure.

Struck by rolling or sliding object

Employees were removing an inspection cover plate for the dredge pump. The plate slid and struck the employee in the right hand / fifth finger, causing a fracture.

While replacing ladder cable, injured employee was instructing another employee on how to pull cable off of the drum of the winch. They were at the end of the cable where it attaches to the drum and the injured employee was standing by to secure the component that attaches the cable to the winch. The cable became free and struck the employee on the chin, causing a small laceration.

The employee was working on the feed plate on the gravel plant screen. He was using a drive ratchet to tighten the bolts on the plate. The bolt slipped causing the ratchet to strike the employee under the chin. The strike resulted in a laceration approximately one inch long and 1/4 inch deep.

Employee was tightening bolts on a screen bearing and cut his right index finger on a wire holding the socket on a 1"" impact wrench.

EMPLOYEE IS NOW (5/25/04) REPORTING AN INJURY THAT HAPPENED ON 11/19/03. HE ORIGINALLY SAID A COW STEPPED ON HIS HAND AND CLAIMED IT ON HIS INSURANCE. HE QUIT 5/13/04 AND HAS AN ATTORNEY. HE NOW CLAIMS HE GOT HIS HAND CAUGHT BETWEEN A ROLLER AND CONVEYOR SKIRT.
