Employee cut his right arm when he jerked a pneumatic grease pump (app. 30 lbs.) out of an empty grease barrel. The pump apparently hung on something in the barrel. When the pump came loose, it hit the employee in the right arm, causing a laceration that required 4 stitches.

Employee was shoveling under tail wheel located beneath primary dump hopper when a rock weighing app. 10-15 pounds fell from the side of the dump hopper and hit him in the head. No trucks were dumping at the time of the incident. The rock penetrated through his hard hat and caused a laceration which required 4 stitches.

Employee was walking to tail pulley; stepped on a loose rock and twisted/sprain left ankle. Initial date of injury was 6/1/04 and was seen by company physician on that date. On 6/8/04 physician decided to place employee in an ankle splint.
