Operator reached in the cab of the loader to grab EE's hard hat and while stepping back, missed the step of the loader falling to the ground.

Employee was using a utility knife cutting towards self cutting thigh.

The employee sought medical treatment and was placed on restricted duty , which is being accommodated. While pulling on a ratchet to turn a take-up bolt on a tail pulley located beneath the cone belt, the employee sprained his right shoulder.

The employee received dental treatment on 8/29/12, beyond first aid. The employee whipped a chain and hit his face resulting in a broken tooth.

Employee rolled his truck.

Employee allegedly slipped and fell injuring his right shoulder. Employee was walking on wet concrete surface towards a sump pump to perform a repair. This became reportable when the employee was placed on restrictions on the initial visit on 10/24/2009.

Employee was working on a sand screw out of a course material washer which was supported by two jack stands. The log fell off crushing the right foot and causing a small laceration.
