Employee initially fell on 02/15/2019 Incident was treated as a first aid. Employee continued to work regular duty/regular hours until EE complained of soreness. Employee started to attend physical therapy to improve motion without improvement. MRI indicated rotator cuff tear requiring surgery. Surgery performed on 6/12/2019 raising severity of incident to MSHA reportable.

While performing weed abatement at east end of scale, employee pulled on a weed that broke causing EE to fall same level striking EE's knee on the concrete slab. Fall resulted in a hairline fracture without dislocation of the right knee cap. Modified duty is provided

Employee said that while exiting the cab of the 769 haul truck he bumped his right elbow on the arm rest. The next day 4/16/09 he noticed minor swelling so he was sent to the doctor for an evaluation.
Injured employee was in the process of fueling a Caterpillar D-9R Dozer when he slipped and lost his footing. The employee grabbed onto the dozer with his right hand supporting all of his weight with his right arm. The result was a right bicep tendon rupture, return to work with modified duty. No lifting over 5 lbs with the right arm may require surgery pending medical evaluations.

Employee struck his right knee on the lower corner edge of the driver side door of the truck.
Injured employee was replacing troughing rollers on the utility sand conveyor and pulled a muscle in his right shoulder while tightening the bolts with a wrench. 1/6/09 the employee reported that his personal doctor suggested surgery and that the injury apears to be work related. A claim was submitted and shoulder surgery is scheduled for 5/4/2009.

Employee failed to wear a seatbelt, and buried bucket into the ground of his front end loader, causing employee to hit windshield and break thru windshield
Employee is a loader operator who was going to flip the teeth on the loader bucket. Prior to begining work, the employee was wearing safety glasses, but removed them to look for a hammer in the toolbox. The employee did not put his safety glasses back on prior to commencing work. The employee hit one of the teeth and a metal sliver flew into his right eye.
While working on the Wash plant changing out the deck screen frame, the employee cut the welds holding up screen deck causing screen deck to fall onto right arm/elbow.
