Miner cut their left pinky finger when they struck it with a hammer installing traffic signs in the plant. Miner was sent to urgent care where they were given stitches and placed on restricted duty.

Miner was using a sledge hammer to remove a stuck rock from the grizzly bars. Upon the rock coming free, a piece of the rock struck miner in the mouth resulting in a fractured #9 false tooth. Miner is still working and having dental corrections made to the tooth.

On the morning of 12/6/2022 miner reported to their supervisor that on 12/05/2022 they had jumped from the Jaw crusher frame rail and injured their right knee upon landing on uneven surface. Employee was sent for medical and placed on restricted duty for a sprain knee.
Miner was using a swinging sledge to tighten the mantel nut on the crusher. When the sledge swung past the wrench the miner attempted to catch the sledge on its backswing and got finger smashed between the sledge and the wrench. Miner received medical attention for the injuries and stitches for the cuts.
Miner was changing guarding on crusher when miner stepped backwards and stepped on another guard lying on the ground. This resulted in miner twisting an ankle. Miner was placed in a removable boot from medical and placed off work until Jan 18th, 2022.

Miner was swinging a 20 lbs sledge tightening the locking nut on the cone liner during a liner change. While swinging the sledge they accidentally struck a near-by chute which redirected the sledge missing the nut and struck right foot. The injury resulted in positive x-rays for a broken right foot.

Miner was kneeling on right knee while shoveling material out from under the cone crusher. When the miner stood back up they noticed that the right knee did not feel right. The right knee sustained a strain injury.

Team loaded excavator onto trailer for transport. Employee climbed to top of equipment to get a height measurement for permitting. EE was not wearing fall arrest gear. While climbing back down, EE slipped on some hydraulic fluid and fell approx. 16' to the ground. EE's left arm hit the trailer beam, EE bounced off the track and landed on the ground.

6/6/2016 Miner pinched right middle finger between a clamp rail and angle iron while changing screens when EE laid the rail onto the screen box to use later. Miner refused medical treatment and it was reported as a first aid only that day. On 6/8/2016 miner went to the hospital to have fingers looked at and it was reported by the hospital as broken.
Working on the toggle plate at the jaw crusher, miner was using a pry-bar to break the toggle plate loose. As the plate came loose it caused the pry-bar to shift compressing the miners hand against the wall of the crusher. Miner believed EE was ok at that time. Six days later the miner went to doctors office and had it looked at. An X-ray was taken and discovered a broken bone.

EE was using an impact gun to remove a 3/4"" nut from a bolt. As the nut loosened, the impact gun kicked back and struck EE above the left eye (between his safety glasses and hard hat) causing a laceration above his left eyebrow. EE was taken for medical treatment, received 4 stitches for the laceration and was released to work with no restrictions.
While operating the water pull the miner was looking back at the sprayers located behind the operator stations view. As the miner turned back forward she felt a pull in her back like it was getting a spasm. Miner described pain as in the lower part of her back shooting down towards her leg.
On February 25th, three workers were unloading an excavator bucket from a trailer deck. Injured employee saw that the load wasn't center. As he grabbed hold of the chain to center the load, co-worker lifted the bucket catching injured employee's middle finger between the bucket and chain.
Dusty environment resulted in foreign object in miners right eye causing a cut to his Cornea.
Driving wedges out of crusher liner and the last wedge was hung up holding the liner up still. Miner used a small drift punch to free up the hung up wedge, upon the wedge coming free, the liner fell resulting in the miners left index and middle finger becoming crushed under the punch and the top shell. Miner was sent to the hospital to treat his injuries.

Miner was using a chipping hammer to clean away slag on a new metal cut. Small piece of slag struck miner in right face check and bounced under his safety glasses and into his right eye. Miner received a corneal abrasion when the slag embedded into his cornea. Miner went to hospital and had it removed. Received a prescription to use for 7 days. No other doctors visits needed.
Miner was lifting a metal object when he felt a pop in his right shoulder. No medical treatment was required. There was no lost time or restricted duty and no doctors visits. 11/12/2015 Miner went to medical doctor and was given a shot of cortisone for this old injury. As of 11/13/2015 there has been no restricted work days and no lost time for this injury.

While miner was coming out from under a service truck he was working on, he reached for the rear of the truck to leverage himself up off the ground. There was an unbolted vice located at the rear of the truck in which he grabbed pulling it down onto his leg. This resulted in a contusion to his lower extremity.

WHILE CHECKING HIS SCREENS ON THE WASH PLANT HE SLIPPED IN THE 57 CHUTE AND HIS FOOT SLID BETWEEN TWO PIECES OF METAL CUTTING HIS LEG.
HE WAS INSTRUCTED BY HIS SUPERVISOR TO WELD A GURARD FOR THE DRY SCREEN, THE WAS NOT PROVIDED WITH ANY TYPE OF PROTECTIVE EQUIPMENT FOR THIS JOBTASK. HE PROCEEDED TO WELD FOR APPROX.30 MINUTES AFTER COMPLETING THE JOB HE WENT BACK TO HIS REGJOB OF OPERATING THE LOADER. AT APPROX. 7PM HE BEGAN TO FEEL BURNING IN BOTH HIS EYES.

AS HE WAS REMOVING THE LEFT HOIST CYLINDER FROM THE FRONT-END LOADER, HE HAD THE BUCKET UP SITTING ON TOP OF A HOPPER. HE STARTED TO REMOVE OIL LINE UNDERNEATH THE MACHING WHEN SUDDENLY THE BU CKET FELL. THE LOADER ROLLED BACK AND AS HE TRIED TO GET OUT FROM UNDERNEATH HE TRIED TO GET OUT FROM UNDERNEATH HE TWISTED HIS ARM AND HURT HIS LEFT HAND.
