Employee was operating a CAT 988H loader, when EE struck a pile of recycled asphalt pavement on a roadway that EE was traveling on. Resulting in scratches to both forearms, neck, back, and knee pain. On 8/27/2024 Employee visited the clinic and was given work restrictions.

EE had gone to the restroom, walked out and put hard hat on and went to put safety glasses on and the arm of the glasses went in eye.
Employee was replacing a screen panel. Employee stated the they weren't feeling well and was taken to an air conditioned MCC room to cool off. Employee was not recovering as expected and EMS was called. Employee was taken to the ER and treated for heat exhaustion.

Employee was using ""Dog and Wedge"" to better position a clad cheek plate, striking pieces with hammer. Fragment from steel hammer broke off hammer enter eye from below safety glasses. Fragmentation caused from steel to steel strike.

Employee was servicing the loader 1078076 when EE felt a sting in EE's left shoulder blade area, brushed a spider off of EE's back.

While installing the mounting flange to the wash plant waste pump, the flange fell off the housing seat approximately 2"" smashing his left thumb between the flange and shaft of the pump. His thumb required some stitches but no broken bones.

While closing a sliding window in the crusher shack, the window came off track. EE pushed on the window to put it back on track and the window glass broke cutting his right hand.

While removing cutting edge bolts from a cat 988H loader, EE used his hand to push up a volt to lock it into place. At the same time he hit the trigger of the impact wrench with his other hand causing the bolt to spin in his hand. The worn bolt head cut his thumb through his leather glove. He was wearing all requited PPE and the loader was locked out and tagged.
