Employee was standing too close to another coworker when a bearing was being removed. It broke loose and struck their lip, splitting it. They needed stitches. No lost time at work beyond the end of that shift. No time off work or work restrictions required by medical personal.

On 1/14/22 EE experienced first aid injuries when EE fell through a catwalk. On 6/15 EE had EE's shoulders medically checked. EE was released to full-duty and light physical therapy. On 8/15 EE was still having discomfort, a MRI showed a slight tear in both shoulders. EE had surgery on EE's right shoulder on 10/10/22.

On 5/4/19 employee was taken to the local hospital where EE received surgery for a puncture wound. While greasing the left lift cylinder of the 775E haul truck, and holding the grease lines together, the grease gun came loose of the grease zirk resulting in a puncture to the left thumb.

A sharp object was in a chute EE was examining with another employee and attempted to avoid it while exiting chute. Object snagged EE's safety vest and cut employee while exiting.

On 10/21/15, Employee requested and received medical treatment for an injury that occurred on 10/2/15. He was taken to the Company clinic and was placed on modified duty which was accommodated. The employee reported that kneeling on both knees to cut a steel plate resulted in L-knee pain and swelling. He was diagnosed with Prepatellar bursitis.

The employee was taken to the Company clinic on 4/17/14 and was placed on restricted duty which was accommodated. While changing a screen cloth with a co-worker, the employee's R-hand was caught between a wrench and the metal framework. The employee was diagnosed with a R-hand contusion and released to full duty on 04/28/14.
The employee requested medical treatment on 3/18/14, was taken to the company clinic and placed on restricted duty which is being accommodated part-time. The employee has a pre-existing back condition which possibly aggravated when he slipped on a step while descending stairs. The employee was diagnosed with a Lumbar Strain. Claim is questionable.

The employee reported while descending the ladder on a loader, his right foot slipped off causing him to lose grip with left hand and placed weight on right arm and shoulder.

EE was washing a sweeper truck with a pressure washer. While walking backwards, he tripped over a concrete parking block. He was taken to the industrial clinic where he was diagnosed with minor contusions and an abrasion and was referred to a hand specialist.The hand specialist diagnosed him w/a radius fracture to the left wrist.
Employee was descending down the ladder on a PC1100LC-6 excavator when he slipped hitting his wrist and hand on the tracks.
Tech was working on the engine of a HD605 Haul truck. When another tech lifted the hood with a crane, the hood slipped and fell on the tech working on the engine hitting his head. The tech had a hard hat on, but substained a strained to his neck.
Employee stated that he fell while stepping over trailer to retrieve parts from the back of truck, resulting in sprain of right third finger. Became reportable following medical treatment on 04/04/2008.

While cleaning up after a job, employee was carrying a bucket of tools that weighted approx. 15 lbs in his left hand and a bucket of flexcos in his right hand. He stepped off the concrete slab onto dirt surface, walked approx. three feet, stepped on a rock and lost footing. Right leg turned under and twisted. Employee fell to the ground and called for help.
Employee was repairing hydraulic leak on accumulator on HP500 cone crusher when pressurized hydraulic fluid was released from the accumulator causing employee to strike himself in the chest with a pipe wrench.

EE WENT TO CLINIC REGARDING THIS INJURY ON 2-16-00 AT 8:40 AM AND IS ON LIGHT DUTY AS OF 2-16-00. WHILE TURNING WHEELS ON CONVEYOR STACKER, PULLED MUSCLE BEHIND LEFT SHOULDER.
