Struck by falling object

Employee asked another employee to release bucket on skid steer. employee raised bucket tilting it down. employee stood in front of bucket (in the line of fire) released the pins and the bucket slid off striking employee in their left foot. EE sustained fractures to 4 bones in their foot.

Fall from machine

EE was in the process of ascending on to a closed cab skid steer. As they attempted to open the door of the cab of the skid steer, the EE lost footing, pivoted to the side as they held on and then fell on to the ground resulting in a laceration to the upper part of the EE's head.

Struck by flying object

Employee was working on a cone liner change at our secondary plant. They were hitting the bowl liner with a sledgehammer to dislodge the liner when a sliver of metal broke off the liner and struck them in the right calf area. Wound bled and scabbed over. No movement difficulties or pain reported by employee.

Struck by falling object

Our laborer was shoveling the walkway next to the conveyor belt in our secondary concrete tunnel. While EE was shoveling a fan attached to the tunnel ceiling which was over the conveyor belt came loose and swung down and contacted the laborer on the right side of the face. EE had a bruised cheek and a scratch. The fan was guarded 100% from moving parts.

Employee was parking a manlift and ran over a rock abruptly moving the manlift basket upwards. This caused the employee's right foot to come out and over the safety pedal cage. When the right foot landed it was on top of the cage and the right ankle was injured. A harness was being utilized.

Mechanic was cutting ripped conveyor belt from a stacker conveyor belt. As EE did this the knife contacted EE's skin above EE's left bicep and EE got 12 stitches.

Fall to the walkway or working surface

Employee tripped on the edge of an energy isolation mat in front of an electrical panel causing employee to fall to the ground. Employee caught self with left hand and felt pain in the hand. Employee was examined at a clinic and diagnosed with a possible fracture in hand, given a brace and returned to work with restricted use of the hand.

Employee was checking for leaks under a loader using a creeper. EE rolled out from under it, stood up, and bent over to pick up the creeper when EE felt a pull in back.

Struck against stationary object

Laborer was walking on a concrete walkway and stepped on a rock causing employee to twist employee's knee. The result was a strained knee which causes minor discomfort when employee walks on it.

Over-exertion in pulling or pushing objects

Employee pulled on a spreader bar in the back of employee's truck to roll it off of a bucket and felt a sharp pain in employee's left middle finger. Following that, a soreness persisted followed by numbness in the fingers/palm.

Employee was troubleshooting a conveyor when a rock bounced off and struck employee in the top of the hard hat. As a result the employee is experiencing neck soreness and headaches.

During annual hearing tests for our hearing conservation program, it was discovered that the employee has a threshold shift in both ears.

Struck against a moving object

Employee was operating a Caterpillar 988H loader digging in a face, the loader encountered some hard material. The loader jolted as he tried to curl his bucket back. He had his hand on the joystick steering control when the jolt occurred. As a result, he felt tingling in his fingers and palm.

Over-exertion in lifting objects

988H loader operator was in the process of removing the wheel chock from the holder mounted on the loader when he felt a pain in his left shoulder. He felt the soreness would go away so he refused medical attention. On 12/9/2013, he indicated that his condition wasn't improving and he wanted to see the doctor. The doctor determined that he has a muscle sprain in his shoulder.

Employee was climbing into the feed basket of a cone crusher to inspect; he lost his footing and bumped his right knee. Employee reported the incident but felt he didn't require medical attention. Today, the employee informed me that his knee was bothering him and he wanted to be seen by a doctor, so I took him to the clinic.

Employee was assisting a co-worker when a 3 foot by 6 foot piece of metal started to slip and employee reached with his left hand to grab it and was lacerated on the thumb.

EE was putting a piece of metal in the back of his work truck when it slipped out of his hands and fell down onto his left foot breaking a bone in foot.

EE was walking up steps to MA control room when he had a cramp in his leg causing him to fall to ground. When he fell to ground he hit his right shoulder on steps. Employee initially didn't wa nt to go to doctor. After a couple of months it was still bothering him so he went to doctor. EE had surgery to repair torn rotator cuff on 4/24/12.

Non CEMEX employee was found dead floating in the pit. Police are investigating as potential homicide. Called the MSHA emergency number. They came out and investigated. Per investigator it will be recorded as a non - chargeable incident.

Employee was walking down an incline to the primary crusher when he stepped on a rock and over-extended his left knee.
