Operator was doing his pre-plant inspection and dropped his pen. When he bent over to get it he had a muscle spasm that put pressure on his Sciatic nerve resulting in pain in his leg.

EE was exiting loader, felt sharp pain in lower back

EE was stepping over conveyor frame, foot slipped and cut shin above the boot

While puling on a bar to line up an I-Beam, employee felt a strain in his right shoulder.

Employee getting out of the man lift and slipped on some sand - went to grab himself and according to employee ""popped his right shoulder out of socket."" Employee stated that this had happened before and he was able to pop back in place, did not want to go to docotr, however called next day and wanted to see a doctor.

Shoveling under crusher area belt and strained right forearm.

EMPLOYEE WAS ASSISTING IN THE CHANGING OF BEARINGS ON A TAIL PULLEY, HE LEANED FORWARD AND HIS HAND SLIPPED CATCHING IT BETWEEN THE BEARING AND THE FRAME.

EE WAS REPOSITIONING 10"" PVC PIPE WHEN PIPE SLIPPED OFF BRACE AND CAUGHT EE'S LEFT HAND BETWEEN BRACE AND PIPE SEVERING MIDDLE FINGER OF LEFT HAND JUST BELOW FINGERNAIL.
