Emp exited water truck end of shift stepping onto uneven pavement rolling ankle. Injury treated with RICE. Emp wanted wait see if would be ok but swelling worsened, Supervisor took emp to clinic 03/08/19. Emp prescribed Tylenol, Diclofenac sodium, air boot for up to 6 hrs per day and physical therapy. PT noted boot was not effective after day 3 recommending boot is not required.
This injury is stemming from a first aid injury that occurred 9/10/14 and was reported at that time but no further medical treatment was sought. This was again reported 02/10/17 with the employee stating it was due the the injury in 2014. Original first aid was due to back strain as a roving mechanic at that time. *EE was shoveling on 2/10/17 per call.
On 05/20/2011, the employee was taken to the hospital and was admitted for more than 24 hours. The employee stated that while descending the ladder of a haul truck, he fell and bumped his head.
Installing expanded metal 4 x 8 sheet. Employee alleges back strain.
While moving scrap parts in the boneyard, a t-roller fell on his hand, cutting finger on left hand
Employee was trimming rubber liners with a razor knife when it slipped and cut his left index finger
Employee was picking up five gallon buckets of material when she bent over to pick up bucket and stood up - felt back ""pop"".
While replacing guard on conveyor, employee lifted and twisted at same time causing strain to lower back.
Employee was using hammer on clamp rail of second screen deck. Hammer struck upper deck frame glancing off course and striking employee on left thumb.
Employee went to see company doctor on 3/11/04, received stitches and was placed on restricted duty. Adjusting forks on forklift to pick up palet. Fork fell against back brake causing an avulsion and laceration to two left fingers.
AFTER FUELING COMPRESSOR ON BOOM TRUCK, STEPPED OFF OF EDGE OF TRUCK AND FELL. STRIKING HIS CHIN ON CONCRETE CURB.
STRAINED BACK MUSCLES WHILE OPERATING FRONT END LOADER.
WHILE GETTING OFF FRONT END LOADER, SLIPPED ON MUD AND FELL TO GROUND ON BACK.
EMPLOYEE WAS USING A LONG STEEL BAR TO DRIVE OUTA LOADER BUCKET PIN.
SLIPPED OFF OF CONCRETE SLAB, CUT ON RIGHT SHIN BONE.
SPIDER BITES (ACCIDENT HAPPENED ON THE 19TH, EMPLOYEE WENT TO DOCTOR'S OFFICE THAT SAME DAY AND WAS PUT ON LIGHT DUTY. IT WAS NOT REPORTABLE AS WORK RELATED UNTIL SECOND VISIT TO DOCTOR, ON TH E 24TH. DID NOT RECEIVE INJURY REPORT UNTIL 5-1-00.
EMPLOYEE WAS WALKING, STEPPED ON ROCK THAT ROLLED UNDER HIS FOOT.
