Struck by... NEC

Employee was crawling out of CR02 chute after using a jack hammer to break rock plugging chute. Employee was pushing the jackhammer in front of EE. it caught a rock causing the bit end to pop up towards the employee, striking EE in the teeth. resulting in a broken tooth.

Over-exertion NEC

Operator was changing out a bit; they stepped off of the deck and when their left foot made contact with the lower deck of the drill, they felt their left ankle roll to the outside and felt sharp pain in the left knee.

Operator was stepping off of pickup truck and right food stepped in a hole and rolled right ankle, resulting in a sprain. restricted duty began on 12/11/2025

The driver was completing a pre-trip inspection on the trailer. Driver was crouched down and stood up, thinking they had enough clearance, and hit their head on the trailer. Driver was wearing a hard hat, which compressed their neck.

Employee was feeding a cathode through the roller when the end of the cathode grabbed the tip of their left middle finger and pulled their hand into the guard. Resulted in avulsion of left middle finger tip.

Dump bed fell from cribbing causing multiple fractures to hand

Employee was installing a new bearing using a 10lb sledgehammer when they hit their foot just past the steel toe. resulting in the fracture of the minimus pedis (Little toe)

While dismounting a trailer, Temp employee encountered a loose step causing EE to fall 3 feet onto ground scratch on left elbow and left foot pain resulted in left foot fracture.

Employee was drilling a hole in a new disconnect box. The drill bit got caught and pulled the box into employee's hand. Caused a cut and tuft fracture to right index finger.

Assigned to change out a steering pump and in process used a come-along to hoist up pump into place, felt a pop in left bicep

Employee was cleaning the lime storage tank due to faulty work boot, employee used someone else's PPE and failed to inspect it prior to use, resulting in blisters, treated as first aid and on 10/24/2024 work restricted duty.

Employee struck a pin with a hammer and a small fragment of metal hit EE in the face causing a piece of metal to embed into the skin.

Smashed their left thumb between the valve wrench and a flange resulting in a fracture.

Employee was diagnosed with a standard threshold shift by the Doctor on 08/28/2024. This hearing loss is consistent with noise induced loss to the right and left ear.

Upon pulling the cord, the cord jerked EE forward causing a popping sensation in EE's right arm resulting in a right wrist strain.

The employee was walking when they tripped over rocks on the ground and caught themselves by holding onto a tanker vehicle, injuring their right wrist. The driver was evaluated by a medical professional and released to full duty. On 11/11/24, a surgery was performed to address injury at which point the driver was taken off work for surgery and recovery time.

Employee was working on changing a screen deck panel that had a hole as they were hammering in the new panel, they smashed their left thumb between the hammer and angle iron causing a laceration employee was taken to First aid and seen By EMT and the nurse was called out employee, was sent to have imaging done on the thumb.

Employee traveled to the mine's main change room to use the facilities. EE proceeded into a bathroom stall where evidence (paraphernalia) suggests drugs were taken and employee was found unresponsive. Attempts to revive the employee with CPR and AED were unsuccessful, as well as the administration of epinephrin and Narcan. Employee was pronounced deceased at 10:37 pm.

Pulling on a 4"" nylon sling, bent down to pull up and felt a pop on left hip, on 01/23/24 EE was placed on restricted duty and returned to regular duty on 01/25/2024.

Employee was cleaning out the lime slicker, went to turn around and stepped on a 2x4 plywood and rolled right ankle.

Employee was checking the chute at the ST100 when twisted ankle at east side.***Employee did not become reportable until 01/18/2024, when they were placed on restrictions***

Employee was servicing dozer 524 once the service was completed, employee was climbing down from the tracks with the service hose in one hand and the other hand on the grab iron for support when foot got caught in the tracks falling onto bilateral knees. Restricted Duty began on 12/7/2023.

Employee was doing a cathode press while adjusting the cathode, the copper bar released from the strong back arm striking right forearm sustaining a laceration that required 4 sutures.

Employee was sitting behind driver in van 958 when the driver of the van hit a rock and employee who was seat belted into the seat struck their hardhat on a roof of van. Reported back and neck pain. On 12/12/2023 employer notified that pain is related to the work injury and not preexisting condition. Diagnosis cervical strain.

Employee was riding in the back of ASARCO bus when the bus hit a bump and employee was uplifted from seat came back down onto the seat in front striking abdomen on bar/frame.

While removing an access panel from a wheel motor the panel came off and fell before able to secure. The Panel struck left hand resulting in a fracture of the 5th digit.

Employee was rebuilding a mantel while setting the bottom liner with cable when the crane was lowered and pinched employee's right hand (4th digit) between C hooks and punctured with the latch. X-ray was performed on 8/22/23 showing a nondisplaced distal phalanx fracture of 4th digit.

Employees were using an air impact wrench and as they bled off air pressure a rod that held a swivel in place came off the fitting striking an employee in the chest. The employee was transported to an emergency room to have the rod removed. Employee was given medication during a subsequent follow up.

Left Ankle sprain unknown cause, employee reports no mechanism of injury. Did not report until 7/28/2023.

Employee was at booster station working on a pump installing bolts and nuts when right pinky was smashed between the casing and nut by another employee using a 1.5"" impact. X-ray was performed on 7/25/23 confirmed fracture of right pinky and referred to medical specialist.

Employee was walking down stairs, stepped wrong twisting right ankle. Restricted duty began 5/30/2023.

Employee was off loading pipe from flat bed trailer when pipes band broke, causing it to break free of its restraining devices and roll off trailer, striking employee's right shoulder.

At approximately 3:45pm the electrical superintendent was accessing the transfer tower when a gust of wind blew a metal disk shim from an upper level. They reached out to catch it and sustained a laceration on their right hand.

Employee was cleaning up dust and debris on the transfer tower when a wind gust caught the material. They felt material get into eyes and attempted to clean them out. They continued to feel irritation in right eye and reported it to supervision. Employee was treated by an ophthalmologist who cleaned material out of the eye and prescribed prophylactic antibiotic eye drops.

Employee was getting off the bus at the north dike tiedown walking towards haul truck 423 when employee stepped on a rock twisting left ankle.

Employee was using an air lance to stir up the solids in the Lime Slacker Dilution tank and overextended EE's right arm, felt a sharp pain from EE's elbow to EE's wrist. On 10/27/2022 x-ray was performed and referred to specialist

Employee was removing the pivot pin with fellow employee using an air hammer when the bit chipped off and struck right forearm causing small cut. Xray was performed on 10/4/2022 4mm foreign body was discovered in soft tissue of Right forearm and referred to medical specialist.

Employee was working in the regrind area around the sump. They tripped on the 2"" hose connected to the sump causing employee to slip and fall on to right hand. On 9/12/2022 employee supplied a report showing a fracture of the fourth metacarpal.

The injured employee was removing a belt from a pulley. As another employee rotated the equipment, the injured employee got their glove and finger caught in the belt and pulley. The injured employee sustained an avulsion-type wound on the left index finger. EE was treated at an emergency room and received sutures to close the wound.

The injured employee was using a battery operated impact to replace liners. EE needed a replacement battery and another employee attempted to slide it down along the liners. The battery caught the edge of a liner and bounced out striking the injured employee on the nose and cheek causing a fracture in the nose.

Employee Was Working on haul truck tire. While using the RAD gun to torque the wheel the employee caught a finger between the RAD gun and the wheel causing a laceration to the left index finger. Required 12 sutures.

Employee was placing a pan on the apron feeder. When positioning the pan it caught left glove and pulled the hand toward the pans pinching left hand just below the 5th digit causing a laceration. Employee received 5 sutures.

Employee was attempting to move pothead into position to pull off more cable to reach inside the building. While turning EE stumbled and the pothead slipped in EE's grip, slicing the left palm. EE then fell forward dropping the pothead cutting the skin on inside of left knee.

Employee was assisting crane man while lifting cathodes from the floor when employee felt a sharp pain in left elbow. 12/9/21 is date of injury restrictions were applied on 12/14/21 with no heavy lifting of left arm only.

Employee was descending 10 foot ladder, upon reaching second to last rung pant leg got caught in table causing to lose balance and twist right knee.

Employee was assisting another technician with crack repair on the roof of a P&H 4100 Rope Shovel. EE was communicating with other tech and stepped backwards falling through the danger tape and into an open hole.

Employee was performing the monthly inspection on a fire extinguisher, while putting the extinguisher back into the bracket employee's hand caught part of the bracket causing a cut.

Employee Was Removing Driver Side Hoist Cylinder from haul truck. The hoist came down on the drift and employee caught right middle finger in pinch point causing a laceration to right middle finger 7 sutures.

Employee was descending haul truck 418 stepping off the ladder to the ground. Stepped with right foot on small rock and rolled right ankle resulting in right ankle sprain.

Employee was removing lip shroud from bucket with a sledge hammer. The GET shattered and a piece of metal passed through the employee's right calf muscle.

Employee reports while driving haul truck backing into #2 shovel truck struck pothole at same time the bucket was loading the dump bed. Driver felt pain in lower back. On 3/12/2021 the company received notification that a lumbar strain is the diagnosis of injury that occurred on 12/26/2020 and is attributed to a work injury. Lumbar strain is fully resolved.

Employee was swinging a 15lb sledge hammer to drive in bars and strained a muscle in EE's upper middle back.

Employee was attempting to remove bolts from the lid of a Mine owned water truck, using a battery operated impact. While attempting to loosen a bolt, the employee's left index finger was lacerated between the lid and the nut.

Over-exertion in pulling or pushing objects

Employee was closing the 1 pump valve at the 7 booster to balance flows at the plant. EE went to the 7 Booster to close the valve with a pipe wrench. After the valve was closed, EE walked to check the pressure gauge and felt pain in EE's right shoulder and arm.

Employee was backing off the rock breaker when they felt a tug on pant leg. Employee examined the leg and noticed a metal splinter. Splinter was removed and required one suture.

While trying to remove bottom drill steel, employee was attempting to place shim between jaws of deck wrench and drill steel. Operator thought employee motioned to close jaws pinning employee's right thumb between shim and drill steel causing an avulsion of right thumb.

Employee was lining up cathodes and was walking off cells stepping up to catwalk and tripped. EE fell forward and put EE's hands out to brace self injuring left wrist. Causing a fracture of left wrist.

On 9/26/2019 Employee was diagnosed with a standard threshold shift by Dr. in the left ear. This has not been deemed work related at this time.

While climbing up the ladder into cable realer 686, employee dislocated right shoulder.

Employee was assisting in pipe removal when debris fell from above the deck into left eye. Did not report until 8/19/19 at that time they were sent for foreign body removal of debris in left eye and became reportable.

EE was using a 4 foot steel drift to drive out the anchor pin on TD524 l/h ripper tilt cylinder. IE was stabilizing the end closest to the bore with left arm. When the drift came out of the bore as EE was driving it IE was not ready for it and the weight of the drift pushing down on IE's arm caused pain in the left shoulder. Left Shoulder Strain.

Over-exertion in wielding or throwing objects

Employee was throwing away gate valve in dumpster from back of service truck 926. When EE turned throwing the valve into dumpster EE's leg slipped and EE felt a pop in EE's groin. Resulting in a hernia.

EE was working on the A/C unit on Bus 2 EE removed and re install Blower Motors when EE turn it back on the bus got full of dirt from the unit and it got into EE's eyes prescription eye drops given.

EE was removing GET from shovel bucket with all appropriate PPE. EE struck lip shroud with a hammer, a piece of metal struck EE's right leg below the knee.

While washing down the CV13 drain, a pry bar fell over onto right foot in the metatarsal area causing a bruise.

Employee was sitting in wheel axel housing hooking up electrical wires and hydraulic lines. When the employee stood up employee felt left knee lock up. Received notice 4/18/2019 that the knee is an industrial injury.

Employee was installing pin in the push arm of dozer. The pin slipped smashing EE's 5th finger causing an avulsion. This required three stitches.

Employee was lifting an 8x8 tie at CR4 when they lost their footing. Employee was able to catch themselves and in doing so strained their lower back.

Operator was trying to remove large rock from tracks of TD516 when the rock shifted pinching the operators fingers. This caused laceration to right middle finger. Two sutures required.

Employee had been welding earlier in the day and while in the process of wiping the sweat from EE's head EE used EE's sleeve and got foreign body in EE's eye. After it was reported EE was taken to an eye specialist who removed the material from EE's eyes.

Operator was going east down the stairs and missed the first step falling to the ground and landing on right shoulder. Employee was seen by site ERT and sent to a local clinic where it was determined employee had a fractured right clavicle.

While cutting pipe with a chainsaw, some residual water came out causing the mechanic to recoil striking another nearby mechanic's hand causing laceration requiring 6 stitches.

Employee was prepping to remove left front wheel on HT425. They attached the Ram-Holder to the A-Frame, and bent over to pick up extension. The Ram-Holder came off the A-frame and struck employee in the back of the head. Causing a 3/4"" soft tissue laceration. Employee was treated at the ED and received 3 staples.

Employee was walking down ramp when employee slipped and fell injuring left shoulder and wrist. On 12/7/2017 PT was ordered for Physical therapy due to shoulder strain.

Employee bent over to attach hose to acid tankers for unloading. As soon as they bent forward using normal body mechanics they felt a pull in lower back muscles.

Employee strained left knee when dismounting ladder. When ee stepped down ee skipped the last rung and stepped down onto left knee and felt pain.

The driller had plugged up the pipe, then pulled back a joint to the table and broke it loose and raised the top head. The driller noticed the inner barrel was plugged and leaned forwards towards the table a little when trapped air pushed drill cuttings out of the hole and sprayed into face and eyes.

While installing a sweep on PP02, employee was holding the pipe and EE's coworker was trying to hit the pipe into place with a hammer and struck employee's right middle finger. 8/18/2016 Dr. treated employee with a splint to EE's finger.

Employee was using an impact wrench when the bolt caught glove. This caused glove and middle finger to wrap around the socket causing a fracture to right middle finger.

Employee was called to the #4 drill to verify a low coolant light. While checking the coolant in the radiator, coolant sprayed onto the mechanic scalding hands, arms, nose and torso.

EE was changing drill steel on Drill 4. EE was guiding the steel over the hole in deck when steel swung over and smashed left hand between the steel and the carousel. Damaging left middle finger.

Employee was manually tightening bolts on a casing clamp using a 3/4"" drive socket and 18"" breaker bar, when the clamp was jerked the casing to rotate and the socket slip off of the bolt. Employee lost his footing and fell backwards striking the bumper of the pipe truck with his right buttocks, he then stopped his fall with his extended arms and hands to the ground.

While installing a new air cylinder, the air cylinder fell striking employee's right index finger. Dr. dx minimal displaced fracture.

Employee was syphoning RIPPER 1 soap conc. from a 285 gal. tote. Employee swallowed approx. 8oz. of product. For no reason should a syphoning technique have been used. Irritation of the esophagus.

Our driver was offloading lube oil at a remote fuel station for the haul trucks. He stepped into edge of trench used by haul trucks as a chock and twisted his ankle. Transported and treated off site with an air cast and pain meds. Further treatment by follow up doctor resulted in a boot and crutches. Released for light duty. Full return expected in a few weeks.

Employee was driving a pin out of ripper shank assembly with sledgehammer; when he struck pin and a sliver of metal struck him in his left leg just below the knee. Employee received stitches.

Southwest Energy employees were reloading PT496 and the prill was hanging up badly and had to use hammers to free product. The following day employee came to work with pain in right shoulder. He was taken to the Cobra Valley Clinic in Kearny to be checked out. The Occupational Dr. asked him to get an MRI and after getting results he was to schedule an appointment with Orthopedic.

Employee was getting off the HT490 while stepping on the middle step of the swing ladder he slipped and fell and injured left knee. On 10/13/2014 Dr. placed him on restricted duty for his left knee.

Employee felt pain in lower abdomen area while trying to lift a 70 lb. box. On 7/10/2014 received notice employee was placed on Restricted duty.

Employee was picking up a bit basket, twisting her wrist. On 9/29/2014 employee was seen by Dr. for employee's complaint of pain in shoulder and was dx with a shoulder sprain and was put on restricted duty.

Employee was exiting down the ladder of the water truck, lost her footing and her right hand grip and felt pain in her left shoulder and lower back. Notified on 7/28/2014 of doctor's orders for physical therapy.

Employee's right index finger was smashed when he set idler into the loader bucket. Sutures required for laceration just below fingernail on right index finger.

Employee pulling on ratchet, the socket slipped off bolt, he fell on his back side. When he landed, he braced himself with right hand. He felt no pain or strain at that time but felt it later that evening. On 6/3/2014 employer was notified EE had MRI and the results deemed by physician on 6/2/2014 to be work related injury as a result from first aid occurring on 3/17/2014.

Employee was working in basket of boom truck with LN-25 welder. The welder was knocked over and welding lead hit employee in arm causing injury.

Employee was starting up the ladder when his foot slipped off the rung and inside the ladder rolling his foot and fracturing his ankle.

Employee had an exacerbation of his personal medical condition, refused ambulance transportation and left property. On 3/21/2014 we received notification from our workman's comp carrier that a claim has been accepted for a fracture to the right knee.

Employee went to lean on the concrete barrier and miss judged the distance. Employee fell to the ground and sustained a lumbar strain.

Employee strained back moving a 20lb hammer. Employee went to put it in the tool rack while getting ready to repair bed in the welding bay. Employee became a reportable injury on 12/20/2013 when company became aware that he was placed on restricted duty.

EE and another ee were loading a 4'X4'X1/4"" steel plate onto the bed of truck #185. EE was pushing the plate over the top of the tool box while another ee was on the other end guiding it to the bed of the truck when the plate came down pinching his middle index finger near the palm of his left hand. EE was wearing his gloves at the time of the incident.

Employee was lubricating the bead of a Michelin tire for mounting and received a laceration to the middle finger of his right hand from a loose piece of metal sitting somewhere in the bead area of the tire.

Employee was walking out to replace the battery in unit 197 when he stepped in a small hole jarring his back. With the battery in hand and tweeting to the right, he stepped down off the step he was using, his back gave out. He went to the ground in pain and had trouble getting up. Employee sustained a lumbar strain.

Employee was setting up on a job and there was a gust of wind causing some dust to blow up and under his safety glasses. Employee had some dust in his right eye. Employee reported the incident immediately after he was done flushing his eye. Employee was sent to the nurses station to be check out. On 11/5/2013, employee was sent out for foreign body removal from right eye.

Operator was pulling power cable for the shovel and hurt his right shoulder. On 11/4/2013 Employee saw company doctor and was placed on restricted duty. Right shoulder strain.

Employee was confirmed to have a STS on October 24, 2013 by the company doctor. This STS has not been deemed an industrial at this time.

While assisting in the Auto Valve change on the #2 filter at the Tank House, employee's thumb got caught between the valve disc and body causing a laceration of the left thumb.

Employee had crawled between PP16 & PP17 at the regrind cleaner sump to get to the spray bar and repair it. While crawling towards the spray bar, employee caught his right index finger on the grating next to pumps and cut it.

Employee was casting at the CR4 stockpile. When he curled the bucket back, it hit hard and caused the loader to shake, jarring his back.

While employee was trying to break nose bars using another nose bar he smashed his finger causing a fracture, employee committee an unsafe act by not using the proper tool for the task.

Employee was moving cable when he lost his footing, and fell backwards on ground. Employee landed on right elbow felt pain in both elbows. Employee was treated as a first aid. On 1/9/2014 he sought medical treatment due to ongoing pain and was placed on restricted duty.

Employee was coming down the stairs from the maintenance shop lunch room when his left foot slipped from the step next to the bottom step causing him to fall to floor level injuring his left leg.

EE was unscrewing a bale, the pipe turned in the clamp, the bale fell on his hand, crushing the hand between the clamp and bale.

Operator was disconnecting discharge hose at #4 acid station and hose blew off and sprayed him on side of face and back of neck causing employee to fall and sprain his wrist.

The truck shop boiler makers were assigned to clean up behind the bed repair area. Employee was directing using hand signals for EE on the forklift to separate two sheets of 600 plate. Employee signaled the operator to lift the plate to install a 4x4x4 piece of wood to place between the plates. While twisting and pushing Employee felt a pull in the lower center of his back.

While removing a tramp release cylinder from the CR02 MP1000 crusher utilizing a come along, the cylinder tipped forward coming in contact with contract employees right foot causing a contusion to the metatarsal portion of the foot.

Employee was audio tested on 8/19/2013. On 09/16/2013, Dr. notified the company that this employee has a standard threshold shift on hearing. The hearing loss has not been ruled as industrial at this time.

Employee along with 2 other MRT members were setting up a harness rescue with the training manikin. After they attached the lanyard to the hand rail they pushed the manikin over the edge. When the manikin fell employee right thumb got caught up in the lanyard. Employee was wearing gloves.

Employee was riding as a passenger in VN975. The van was coming up the hercules ramp when it hit a pothole in the haul road and jarred the occupants of the van.

On 6/19/2013 employee had a first aid injury. Operator was coming down the ladder when his right foot slipped causing him to twist the right knee. On 1/9/2014 received notice that employee is having surgery on right knee with a causal relation to the first aid that occurred on 6/19/2013. Employee has been full duty up to surgery date of 1/16/2014.

At the end of shift the operator was exiting the Porta-john at the Guzman Project work area when she stepped on a rock, causing her ankle to twist; she then fell to the ground. Ankle strain. Was treated as first aid. Became reportable on 6/6/2013.

Employees were using an air nail gun to frame the walls in the new ass in the truck shop. Employee attempted to nail his side when the nail gun went off and struck his lower forearm. Employee was checked by the nurse transported to clinic treated and released back to work.

Employee was assigned to rebuild the bed from HT406. During assessment he was sitting on a rolling stool. After inspecting the frames he rolled out and thought he was clear of the bed and stood up. This knocked his hard hat off he was diagnosed with a neck strain.

While employee was hooking up the coupler he felt a pain in his back. Patient did not become reportable until 5/13/2013.

Received ICA claim on 5/8/2013. Employee reports on April 25, 2013 he injured his left hand. He saw a specialist and has been placed on restricted duty.

Employee's left hand spun around the extension injuring the tip of the middle finger while working on HT426 installing the left rear tire. Employee was using a one inch impact. Employee became reportable on 4/26/2013 when he was diagnosed with a fracture of the middle left finger.

Officer was leaning over to pick up something while sitting on a chair and the chair rolled out from under her causing her to fall on the floor and hurt her wrist.

Employee was getting on the bobcat at the load out tunnel, he grabbed the right side safety handle and stepped on to the bucket. His left foot fell between the bucket and loader. Employee began to fall. He pulled hard on the handle pulling a muscle in his right shoulder. Dr ordered physical Therapy 3/28/2013

On 12/31/12 Employee was stepping down off of ladder he missed the last step placing his foot onto the ground at which time he felt a pop in his right Knee. Employee was treated as first aid. On 4/30/2013 Employer received MRI results that show Employee has a tear of the meniscus in the right knee.

Employee was walking across the deck of HT425. She tripped on some bolts that hold an inspection cover to the deck. She fell to the deck injuring her right knee.
