Rubbed or abraded

While cleaning a flowmeter for install, employee cut right finger on the sensor requiring stitches.

Fall to the walkway or working surface

While employee was removed tires from a connex for delivery, they noticed overlap of tires with an adjacent stack. As additional force was used to move the overlapped tire, the adjacent stack became unstable causing some tires to fall and strike the employee which resulted in the employee falling and straining their back.

Over-exertion NEC

Employee was tailing a core tube out from a ct-20 to pull the sample. While tailing out the tube, the Kelly hose snagged on the wire line causing slack. As the tube came up changing the angle releasing the slack, dropping the back side of the tube 1 foot causing a strain on the helper right shoulder

Employee experienced eye irritation from an unknown particle and attempted to flush their eye; later in the shift the irritation persisted and the employee was seen for an eye abrasion.

Over-exertion NEC

On 1.21.2026 Employee reported soreness in the right bicep that developed over time while performing normal work duties. Employee noted that on 1.16.2026 EE's shoulder had popped while adding a stick of HWT pipe to a drill string going in. Employee also noted that EE's shoulders pop on occasionally and thought the pain would stop, as pain increased, employee reported the injury.

Fall from scaffolds, walkways, platforms

While climbing a pipe crossover, the individual's hands slipped from the handrail, causing a fall to the ground where they impacted their shoulder and elbow.

Struck against stationary object

During routine maintenance of the secondary slitter, the mechanic was adjusting the cutting wheels. While loosening the set screw, the employee's right hand thumb area made contact with the cutting blade, resulting in a laceration.

Over-exertion NEC

While walking the third floor of the secondary crusher area, the employee reported stepping on an uneven surface and subsequently experienced a sensation in the left leg.

CAT79 experienced a rough load event while being loaded by Shovel 44, positioned on the outer edge of the cut and working in course material.

Haul truck CAT82 was in the process of being loaded by Shovel 44 when, during the initial bucket load, a concealed boulder within the material struck the truck bed, resulting in a rough loading event.

Absorption of radiations, caustics, toxic and noxious substances

At 4 am a haul truck operator began to feel itchy. EE went to the restroom and discovered red bumps (rash) on EE's body. EE notified supervision.

EE was installing belt on pulley. EE's finger became caught between the pulley and the belt, amputating the tip of EE's right ring finger. EE was taken to the hospital for treatment. Taken off work.

Struck by... NEC

EE was working on cleaning out mixer. EE picked up a pry bar to break loose hardened material from mixer flights. While doing so, the ee hit the buildup of material and the force from pry bar hit EE in the face near EE's eyebrow / forehead area. EE suffered a laceration and needed stitches.

EE was using a swing gate to access a ladder. When EE let go of the gate, it swung back closed. The EE's finger was struck and it began to swell. Near the end of shift the EE informed supervision of the incident. EE was taken to the clinic and x-rays were done. EE had fractured EE's middle finger. EE was taken off work.

Over-exertion in lifting objects

EE was assisting a group of co-workers with moving a Knaack box into the Crush/Convey maintenance shop. EE was lifting on side of the box and then they realized they needed additional help due to the box being too heavy. After they were done the EE began to feel lower back pain. EE was seen and released. On 9/29/25 EE was issued Physical Therapy.

Over-exertion NEC

EE was removing a 20"" hugger with an impact when the nut came off of the bottom half of the hugger it dropped. The EE caught it and tweaked EE's back in the process. On 9/19/25 EE was seen and sent back to work. On 9/25/2025 EE was issued physical therapy.

Over-exertion in lifting objects

EE was working on greasing fine ore feeder motors. They picked up a grease gun & felt a pop in their elbow. They continued to work & grabbed the grease gun again & felt another pop with instant pain. They reported it to supervision & was seen at the clinic & released. On 9/10/25 they went back to the clinic & was issued restrictions for an elbow strain.

Rubbed or abraded

Employee was changing a sump pump while working in a muddy area; irritation from right foot friction with muck boots resulted in localized swelling and redness. Reported to supervisor on 08/25/25.

Accident type, without injuries

On 8/7/2025, Mill control stated that an employee was stuck in the east freight elevator at the mill. MSHA was notified and at 1:06 pm the EE was freed from the elevator. Elevator company was dispatched to scene and freed the EE. Investigation is on-going.

Struck by falling object

During the harvest process at Tankhouse, ee had to remove a cathode, weighing approx. 152 lbs on the strongback prior to unloading. The ee lowered the strongback to approx. shoulder height to detach the cathode by removing the hanger bar. The loop failed and broke off, landing on the ee's foot. EE sustained a fracture and laceration. Received stitches and taken off work.

EE was ball charging in the Mill. The bin became low. They used a ball to throw up into the bin to attempt to get more balls to come down. EE was wearing impact gloves. The balls started to come down and pinched their left pinky finger, causing a fracture to the distal tuft (tip of finger). Was released back to work wrapped with coban.

Caught in, under or between a moving and a stationary object

RAM Belt Team was performing work on the 8D feeder belt. While maneuvering the I-beam on the deck, two RAM employees - one pulling and one pushing - attempted to slide the I-beam into position. During this process, one team member's right pinky finger was pinched between the I-beam and the structure.

Fall from machine

EE was dismounting a haul truck from night shift. EE was on the lower platform of the truck when EE's backpack became hung up on the handrail. As EE was trying to free the backpack, EE lost EE's footing and fell to the ground. EE sustained a laceration to EE's face and soreness in EE's neck. EE received 13 stitches and was taken off work.

EE was removing placement bars on the reject chute cover. The last bar was under tension and when it released it struck EE in the face. EE sustained a broken tooth and a laceration to EE's mouth. EE received 8 stitches.

Accident type, without injuries

1 EE was inside the Primary Crusher elevator when it stopped. The elevator would not move.

Over-exertion NEC

EE was providing a site tour to auditors. EE was walking out of the man van and twisted an ankle without feeling pain. EE notified supervisor was treated and released. On 8/13/2025 they were diagnosed with a nondisplaced microtrbecular fracture. EE was also issued PT.

Contact with heat

EE was showing other employees how the computers are utilized in the office building. EE began to feel ill. ERT was contacted and EE was taken to the hospital for further evaluation. Diagnosed with dehydration.

Employee was working on a buggy whip when the screwdriver being utilized slipped and penetrated the right index finger. Employee received stitches and was released to work.

Caught in, under or between a moving and a stationary object

Employee was removing liner plates. While handing an 8"" x 4' piece from the regrind cell to the employee standing on the deck. Their finger was caught between the piece and the handrail.

Accident type, without injuries

Motor grader fire. Unknown cause. Operator exited equipment and issued a Mayday. Local fire department and FMI water trucks responded.

Fall from machine

EE was getting off a dump truck. As they stepped back, they slipped on mud from the top step. As they landed, they rolled their left ankle. EE sustained a sprain to an ankle. EE was issued restrictions.

While removing the axle shaft from the right final for the final PCR, the shaft was stock. The technician started to move the shaft to release it. The shaft shot backwards, pinning the technician's right hand against the forklift and mast. The technician sustained a fracture to the hand. Employee was issued restrictions.

Struck by flying object

EE was conducting work in the mill underflow tubs by chipping away steel ball chips from the grating when a piece of metal went into their eye and began causing discomfort. EE was taken to get medical treatment and was prescribed an antibiotic.

EE was removing a valve stem cover plate. After removing the first 4 bolts, they proceeded to remove 2 more. As EE was removing the second, the knife valve split apart, causing slurry to shoot out the side. Slurry came into contact with the EE. EE sustained abrasions and multiple contusions. EE was issued restrictions.

while lining up the new filter to insert all thread a pebble fell off the shelf bounced past my face shield and passed through the top gap in my glasses and into the inside corner of my right eye. It instantly began burning

Fall down stairs

EE was inspecting #2 drying air compressor. After they inspected the equipment, they stepped back off the concrete pad & their foot missed the lower step. Their foot caught a wood plank which gave way. They stumbled and fell to the ground. They reached back to brace their fall & injured left shoulder. They felt & heard a pop. Employee was issued restrictions.

EE was dumping in the drop box. As they were placing the cover back on it broke off & hit their little toe on right foot behind the steel toe cover. The contact caused a fracture to the little toe. EE was issued restrictions.

EE stopped for an operator break. EE went to the passenger door to get hard hat. When EE closed the door, EE's sleeve became caught on the door and EE hurt wrist. EE was taken to the emergency room and given a splint. On 1/27 EE was issued restrictions.

Over-exertion in pulling or pushing objects

EE was moving skimmers onto a flatbed truck. EE was pulling/pushing the skimmer when EE heard a ""crunching"" noise in shoulder. EE reported the incident & was taken off site. On 1/18 EE was issued restrictions.

Struck by... NEC

EE was using an impact wrench to remove bolts from the wear segments on a loader. EE placed a hand on the bottom edge to hold the carriage bolt up while using an impact gun. It spun the nut off. The bolt head spun causing a laceration to EE's left index finger and middle finger. EE was wearing protective gloves at the time. EE received stitches and restrictions.

EE was in the process of changing a pin on the incher. When EE tried to remove it by hitting it with a hammer, a piece of the hammer metal came off of the pin and went into EE's leg. Embedded piece was not removed.

EE was attempting to connect a hose to a vac truck when a piece of metal that was fraying from the hose penetrated the employee's right middle finger. It traveled inside & exited the finger. EE received stitches & restrictions.

EE had completed a change out of an idler. EE was walking down the grated walkway when EE stepped on a rock in the grating and twisted right ankle. EE sustained a right ankle sprain. EE was issued restrictions.

Fall onto or against objects

While the employee was preparing emulsion in the rear of a slurry truck, a coworker called for their attention. As the employee descended the stairs and began walking towards the coworker, they did not notice a 2-inch load hose lying on the ground. This resulted in them tripping over the hose and sustaining a fracture to their right ankle.

Bodily reaction, NEC

On 10/16/24 EE was supporting verimill project. EE was walking up & down stairs for 1 to 2 hours. Began to feel some discomfort. On 10/22 EE was issued restrictions, had first appointment on 10/31/24.

Over-exertion in lifting objects

On 10/3 an EE was prepping up the bleeders on Tailings. EE lifted up the pipe and felt a pop on their left side. EE went home. On 10/8/24, EE notified the site of the pop and was seen at the clinic. EE was then issued restrictions.

Over-exertion NEC

At the end of shift, an operator was getting into the man van in the passenger side when EE's knee popped. EE was taken to Room 100 then to an offsite medical facility where EE was treated for a knee sprain. Was issued a knee brace and given restrictions.

Contact with hot objects or substances

EE was doing a fix on a drill. They were using the torch for steel bars. Their left hand was holding the bar & they turned to the right. The torch began to heat their glove. When they felt the heat they turned off the torch, removed their glove & saw their hand was burned. They were taken to the clinic & injury was debrided. Employee was issued restrictions.

An ee was cleaning the rust off a dog ear by placing it flat on the scaffolding floor. A coworker was climbing on the scaffolding that made it move. This caused the grinder to shift & make contact with the EE's finger, causing a laceration. EE had stitches & restrictions issued.

An employee was assisting another employee w/ removing a zip tie off a fuel can nozzle. EE was holding the can while a second employee was cutting the tie w/ a knife. Knife cut thru the tie & the momentum continued forward causing a laceration to the top side of the EE's thumb & index finger on the webbing. EE had stitches & issued restrictions.

Over-exertion NEC

EE was carrying hand tools (broom and shovel) in their hands and stepped off a small retaining wall and twisted their leg/ankle. They sustained a fracture to their leg and dislocation to their ankle. EE was issued restrictions and referred to a specialist.

EEs were setting a pinion into a rack with a bridal chain. The bridal chain became caught between the old pinion and rack. The chain was being moved and the pinion rolled, catching the EE's left thumb and causing a laceration to it. EE was taken to the clinic where they received stitches and restrictions.

EE was working on a dust collector. They stood up from a crouching position & became dizzy & lost consciousness. A coworker helped them to the ground. ERT was called, they were transported to a medical facility & treated for dehydration.

A mine ops technician was monitoring the 53 tie down when they noticed a haul truck operator was walking in a weakened state as they were leaving the restroom to get on their haul truck. ERT was called to check the operator. Operator was taken to a hospital & was treated for dehydration. Operator was given 3 IV bags.

Over-exertion NEC

EE was headed to the basement area of the tankhouse to check up on the cell repair. On the bottom of the stairwell, EE mis-judged the landing and stepped down on EE's right foot. EE lost EE's balance and landed with EE's full weight on EE's right leg. EE was issued restrictions.

A shovel operator was swinging a full bucket to load into a haul truck. The bucket made contact to the tail end of the bed. The contact caused a rough loading event. On 5/31/24 the haul truck operator issued further medical treatment for chiropractic care. On 6/11/24 the operator received first chiropractic treatment.

An employee was eating almonds while on a break and they cracked a tooth. They went to the dentist to have the tooth repaired.

Struck by flying object

EE was charging #15 ball mill. An employee was near the ball chute on the Fine Ore feeder when a ball popped out of the chute hitting the employee on the right shoulder. EE was evaluated. On 4/30/24, EE was issued restrictions.

Over-exertion in lifting objects

On 4/23/24, 2 employees were loading a gasoline generator into the bed of a pick up truck. They then traveled to their jobsite. Once at the job site, an EE stated that EE thought EE pulled a muscle in EE's back while lifting the generator. EE was taken to the clinic & released. On 5/7/24 EE returned to the clinic with back pain from the generator lift. EE was issued restrictions.

Over-exertion NEC

Employee was coming off scaffolding and missed bottom 2nd step and twisted wrist. Employee checked wrist, moved it around and it felt okay. Employee mentioned root cause was missed step.

Struck by... NEC

IE was assisting cutting all thread, the all thread was in a cardboard case, the cardboard case broke allowing the all thread to cantilever up striking the employee in the right wrist requiring 3 sutures.

On 4/16, a loader operator was working all day on the loader. EE indicated that EE did not want to be on the loader the next day due to the seat hurting EE's back. On 5/3 EE returned to the clinic and was issued restrictions.

EE was fueling a generator in the parking lot at the Tailings shop. EE was removing a hose from the generator. The generator door swung shut and struck the EE in the left ring finger. EE sustained a fracture to the left 4th finger. Issued restrictions.

Employees were changing wall liners on 9-ter feeder belt. An employee was using a prybar to remove old liners. As EE was unbolting the first bolt, the two liners on top fell and hit EE's hand. The employee sustained a laceration to the right finger. EE received stitches and was issued restrictions.

EE was walking up the ramp from the tunnels. The EE had a tool bag that kept slipping off EE's shoulder. EE went to make an adjustment to the tool bag and felt a scratch on the left palm of their hand. EE noticed was bleeding. EE was taken to the clinic and stitches applied to the laceration. EE was also given restrictions.

EE was cutting trip cable with a grinder / cut off wheel. When it cut thru the cable whipped up and went under their face shield, hitting them in the lip. EE sustained a laceration, EE was taken to a medical facility and received stitches inside and outside of their mouth. EE was released back to work.

Operator was loaded with blocky material and felt a pain in their neck as they were getting loaded. They proceeded to dump the load in the shovel pit. They dumped out the load and then called for supervision. Operator was taken to the clinic and issued restrictions.

Operator stated that they had hurt their neck when they looked up and down while traveling up Pushback 82 ramp on the previous shift. EE was issued restrictions.

Over-exertion NEC

EE was walking away from the truck when EE slipped and caught self on the truck. EE slipped near the 57 screw conveyor. EE began to feel discomfort to their lower back. EE was issued restrictions.

EE was in the process of loading haul truck when the dipper door opened and dumped the material on haul truck's dove tail, causing rough loading event. EE stated felt fine. EE was issued physical therapy and on January 31st EE attended PT. Health & Safety not notified.

Caught in, under or between a moving and a stationary object

EE was cupping an impact gun when the bolt came loose and the weight of the impact gun fell and pinched a thumb against the rim of the haul truck wheel. EE sustained a laceration to their thumb that required stitches.

Over-exertion in lifting objects

EE had tipped a bleeder and felt a pinch in their back. EE did not report it right away. On December 1st they informed supervision that they had gone to Urgent Care to seek medical attention. EE was seen and released. EE was sent to the clinic to be evaluated and issued restrictions. EE did not come to work on the 2nd or 3rd.

Haul truck operator was queued up at the crusher. A loader operator had begun clean up on the floor in back of the HT. The loader had backed into the parked HT bed. The HT operator felt some discomfort in lower back as the night went on. The haul truck operator was taken to seek medical attention and was issued restrictions.

Driller had exited the drill to put up drill stakes to mark the holes EE had drilled. EE was walking to the left of the cab and fell into an unmarked drill hole that had no cutting around it. EE's left leg went into the hole and made EE fall to the ground. EE sustained a fracture to EE's ankle. EE was taken off work.

EE noticed redness in their right eye. EE flushed out eye with no change, was still experiencing some discomfort. EE was seen by tele-med and was advised to go to the emergency room. Emergency room did not find any foreign object in eye, but did prescribe prescription eye drops. EE was released back to work with no restrictions.

Struck against stationary object

EE had sustained a laceration to EE's finger with a piece of hanging metal. EE was seen by tele-med and it was suggested that EE received stitches. The EE was sent to an emergency room and glue was applied along with butterfly sutures to close the wound. EE was then released back to work with no restrictions.

Caught in, under or between a moving and a stationary object

EE had been assigned to the 59 haul truck. During the preshift inspection EE noticed they were missing a fire suppression tag and called a mechanic. While looking for a BO flag to put out in front of the truck, the EE slammed the passenger door on their hand. They sustained a laceration on their middle finger. 4 stitches were placed on the laceration. EE was returned to work.

Employees were installing an expansion boot on the bypass box. As they were lifting, the boot ripped and the rigging became loose and struck the employee in the mouth (upper lip). EE was taken to the clinic for medical treatment. EE sustained a laceration and received 5 stitches in the mouth. EE was released back to work with no restrictions.

The employee was repositioning themselves with the jackhammer, when they lost their balance. The air hammer tipped forward catching their left thumb between the skid and the air hammer.

Heat related. EE began felling nauseous.

EE was using a J hook to correct proper anode location on the cell line. The J hook slipped from the bottom of the anode and this caused the employee to lose their balance and trip and fall. They injured their face and lower leg. EE issued restrictions.

EE was trying to remove a liner from the underflow tub by using a 5 ft pry bar. The EE's left index finger became smashed in between the pry bar and the handrail. EE sustained crushing injury to left index finger. Given restrictions.

Struck by flying object

EE was tightening bolts on a dozer track with an impact wrench. The impact exhaust kicked up dust and debris into EE's face and some went into EE's eye. EE had metallic foreign body imbedded into the eye. Taken to clinic to have metallic piece removed and issued medication and referred to ophthalmologist.

EE and crew were stripping back 2"" stringers on the W2 leach field. The EE slipped and tried to catch their balance and slipped a second time and fell, hurting their ankle and upper foot area. EE was taken to the clinic and was issued restrictions. Was taken off work.

EE was dismounting a forklift and using three points of contact. EE stepped wrong as EE touched the ground and rolled ankle. EE was taken to clinic and issued restrictions.

EE was pulling replacement liners off in the B1 feeder belt. EE was the inside person doing this. The employee on the outside the feeder was tightening bolts and EE was holding it. The EE was not ready when the liner was sucked to the wall, causing the EE's eight fingers to become pinched. Several sustained fractures. EE was issued restrictions.

Over-exertion in pulling or pushing objects

EE was delivering drill bit to 90 drill. Oiler was standing next to 90 drill. EE was unloading drill bit. As they grabbed the handrail to the drill they tried to pull themselves up and felt a pop in shoulder and a snap. EE was taken to a hospital for treatment. Issued restrictions for a dislocated shoulder.

EE was walking on the truck deck when EE slipped on the catwalk and sprained ankle. EE was issued restrictions.

Struck against stationary object

Employee sprained ankle when EE stepped on a rock while getting out of truck. EE completed EE's tasks & returned home. EE noticed swelling and bruising of EE's ankle that evening. EE went to a clinic the next day & was told to take OTC meds & RICE therapy. No work restrictions were assigned at this exam. Follow up exam on 6/23 resulted in work restrictions & an order for PT.

EE was raking under the idler 7B conveyor when the rake became caught in the belt and broke. The employee's hand struck the support beam and the contact caused the employee to sustain several lacerations to the back of their hand. EE was issued restrictions.

An operator was inside of a drill & noticed the door's rubber seal was coming loose. Operator opened the door to adjust the seal with right hand & then closed the door with left hand. Operator's two fingers on right hand became smashed in the door frame. Operator was taken to a medical facility & sustained a fracture to a finger.

Struck by powered moving object

EE was working in removing petron barrels. EE was helping a skidsteer operator by holding & guiding the barrel into the bucket of the skidsteer. The barrel started to tilt over & to prevent it from falling the skidsteer operator put the bucket down, pinching the EE's foot. EE was take to clinic & issued restrictions.

Over-exertion NEC

EE was coming out of the man-elevator and stepped off of the step. They rolled their right ankle. Employee fell to the ground. Ankle began swelling. Employee was taken to the clinic and placed on restrictions.

Absorption of radiations, caustics, toxic and noxious substances

EE was swapping out ball valve #4 to #2. The lime line broke at the valve and shot up from under the grading. Lime went under the EE's safety glasses into both eyes. EE flushed eyes out with water. They later went to ER to get medical attention. Employee was prescribed multi use antibiotics.

Struck against stationary object

An employee was exiting a dozer at the B4 stacker area. As EE was getting off of the track EE's left foot landed on a rock that rolled, causing EE's right leg to twist. EE heard a pop. The pain began to increase later that day. The next day EE went directly to the clinic and was evaluated. EE was released with restrictions.

EE was working with a co-worker lifting a roller a couple of inches off the ground. The roller slipped out of their hand & landed on left index finger. EE sustained crushing injury to finger & lacerations. Stitches applied & restrictions issued.

Absorption of radiations, caustics, toxic and noxious substances

EE was walking to the shop when EE could feel the acid mister liquid hitting EE on their face. Some had gotten into EE's eye. EE tried flushing it out. Later that afternoon EE reported the incident, EE was taken off site to a medical facility. EE was given multi use eye drops for the eye.

Rubbed or abraded

EE went to the 04 warehouse to pick up some tools. When they arrived at the warehouse the attendant buzzed the door to unlock it but it didn't unlock properly. The EE went to turn the handle and pulled on the door hard. It caused the handle to break and the EE sustained a cut to the palm of their hand. EE received stitches to their hand.

Inhalation of radiations, caustics, toxic and noxious substances

Control room operator tasked with switching scrubbers. Field operator was asked to close the crossover gate/valve on the wrong roaster by control room operator. During process it caused SO2 gas to vent thru #1 roaster building. EE attempted to remove themselves from area. Was overwhelmed with gas in building. Transferred to Hospital, kept overnight.

NEC

Employee had just finished lunch, grabbed a water bottle on EE's way back to EE's position on the job. EE collapsed but was awake and confused.

Chain hook slipped, hitting the worker in the cheek area under eye.

IE was assisting with replacing a concave into the primary crusher. While guiding the concave the IE attempting to push the concave into place and felt a slight pinch in EE's lower back. IE diagnosed with slight lower back strain.

Over-exertion in pulling or pushing objects

EE was pulling cloth on #5 filter. They bent over and with both hands began to pull cloth. They felt sharp pain in middle of lower back that radiated down to legs preventing them from walking. EE was taken to clinic on 02/05/2023 and was issued restrictions. Taken off work.

Struck by falling object

EE was cutting off the center cutting edges with a torch. The edge slid off the moldboard and came down and struck the EE on the left forearm. EE turned off the torch then checked their arm. No broken skin or scrapes. On 1/23, EE went to clinic and had x-rays on forearm. Discovered they sustained a fracture to forearm and was placed on restrictions.

Struck against a moving object

EE was operating a dozer on Pushback 53 dump. They made contact with a large rock while reversing, jarring the dozer and EE. This caused discomfort in their back. EE was taken to clinic to be evaluated and issued restrictions.

Contact with hot objects or substances

EE was cutting a bolt off with a grinder at the #6 Ball Mill Underflow tub while performing a PM. A piece of the metal fell into the EE's left boot and onto their foot, causing a burn to the outside of their left foot. EE sustained a 2nd degree burn to the left lateral ankle. Their pants were tucked into their boot. EE was issued restrictions.

EE was called to the 45 shovel. EE got clearance and began to approach the shovel. As EE was walking towards the boarding ladder on the right side EE lost EE's footing and fell forward, hitting EE's head and falling on EE's right elbow/arm. On 12/21/22 EE was issued restrictions.

Inhalation of radiations, caustics, toxic and noxious substances

EE was found sitting inside EE's pickup truck unconscious responsive. First Aid was rendered & EE was transported via helicopter to a hospital for further medical treatment.

EE was working on #2 Moly Larox making repairs on a guide plate. EE was attempting to drill a hole to reinstall the guide when the drill bit became jammed. The drill spun around hitting the EE in the face causing a laceration to EE's mouth and loosening a tooth.

EE was going to replace the plate on the ball float of #2 column cell. The ball float shaft slipped and the ee went to catch it and EE had gotten a laceration to EE's finger. The ee received stitches to EE's finger and given restrictions.

An operator of the manvan was going too fast & hit a washout behind the Crusher tiedown. This caused the EE in the back seat to be bounced up & down. EE's back & neck became jarred. As they were headed back to Room 100 once again the EE became jarred by the speed bump. EE sustained neck & back strain. Restrictions issued.

Struck against stationary object

Three employees were attempting to unplug the drop chute on 2-1 & 2-2 hammermill. The EE was hitting the chute about 10 times with a pry-bar. They hit their left pinky finger against a piece of angle iron that is attached to the hammermill drop chute. The EE noticed blood on the piping. They received a laceration to the finger. EE was given 5 stitches and restrictions.

On 9/09/22, a EE reported to supervision that they were feeling discomfort in their back as a result of operating a motorgrader on 09/05/2022. They were taken to the clinic & issued restrictions & physical therapy.

Contact with heat

EE was sitting on the barricade, a coworker walked by & asked EE how EE was doing. EE did not respond, coworker became concerned & started asking EE questions. EE finally responded by saying EE did not feel well. EE was taken to clinic for medical evaluation then transported offsite for further care. EE was diagnosed with Heat Exhaustion.

EE began to not feel well due to the heat. They became dizzy and vomited. EE reported to supervision and then EE was taken to the clinic. The clinic transferred them to elevated care at an ER. EE was diagnosed with Heat Exhaustion.

EE was performing housekeeping duties at the Tailings booster containment area. EE had slipped on a patch of algae that had formed near the sump pumps. EE sustained an injury to the left knee. On August 6th EE was issued restrictions.

During the task of cathode sampling, an employee felt a pop in lower back while they were pushing the cathode into the saw blade. EE notified supervision and was taken to a medical facility. Employee was diagnosed with lumbar strain. Employee was given restrictions.

Accident type, without injuries

Three contract employees became stuck in the freight elevator. Electricians were called and they could not reset the elevator. Notifications were made. ERT rescue team was contacted. Attempted to open doors and hatch would not open. CE's were finally able to be released from the elevator at 10:52 a.m.

Contact with heat

EE was priming on the 403 pattern. EE became unable to walk or stand without assistance. EE was transported to the crusher ready line. EE was then taken off site to a medical facility and treated for Dehydration and Heat Exhaustion.

Struck against a moving object

An operator was backing up to a shovel to get loaded. Operator pulled forward to realign with dipper. As they started to back up again the operator felt contact with shovel that was on the left side of the shovel. EE in shovel felt some pain in neck like whiplash. EE was issued restrictions.

Bodily reaction, NEC

Employee stood up from kneeling position and felt strain to right knee. Medical diagnosis MRI determined tear to the meniscus.

Absorption of radiations, caustics, toxic and noxious substances

EE was attempting to close drain valve on #2 Leach vessel. EE tried to exit the area. EE's rubber boots became stuck in the moly concentrate under the vessel. EE fell forward putting hand out to break the fall. EE's feet came out of the boots & EE landed in the ferric chloride solution. It splashed EE's face, hands knees. EE crawled 20 ft to safety shower.

On 7/2, ee was installing a pump at Butler substation. EE stepped over the pipe and did not see the valve. EE tripped and fell, landing on left wrist and knee. On 7/21, EE's MRI was positive for a fractured wrist. EE was issued a brace.

Contact with heat

EE was walking near the shop and felt dizzy/lightheaded then collapsed to the ground. Mayday was called and EE was given cool water and cold towels. EE was helped to a chair. EE was given IV fluids and transported to facility offsite. Diagnosed with Heat Exhaustion.

Over-exertion NEC

EE was dismounting a water truck to fuel the truck. EE did not realize EE was not in the parking ditch all the way. The front of the truck was higher than usual. As EE stepped down as normal, EE's knee popped and hyperextended. EE was taken to the clinic and on 06/02/2022 when EE returned to the clinic EE was taken off work.

EE was operating a loader. The AC unit in the loader went out some time during the day. The EE began to not feel well. EE called supervision and ERT members were called to check EE out. EE was exhibiting signs of Heat Stroke. EE was placed in a vehicle and given water. EE was taken to an outside medical facility where they gave EE IV fluids.

EE began to feel dizzy. They went to lean on railings & collapsed to the ground. EE lost consciousness for a second. They regained consciousness immediately. EE was take to the clinic & diagnosed with Heat Exhaustion.

Over-exertion in lifting objects

EE attempted to pick up a 10hp electric motor to discard into the recycling roll off dumpster. EE noticed discomfort/tightness in lower back. EE's partner then offered assistance to offload the motor. EE was taken to clinic and restrictions were issued.

EE was loading truck to start work day on fuel island. As EE was getting out of the cab of vehicle, EE stepped on rocks & a broken piece of concrete. EE twisted ankle & fell to ground hitting head. EE was taken to medical facility & sustained a fractured ankle.

EE conducting daily use of hands on the job. Has constant pain in both wrists. Unable to use wrists normally. Trying to utilize double jack when felt pain in both wrists. Has been occurring approximately 6 months.

Struck against stationary object

EE was cleaning around the drumline when EE went to shut off the air valve. EE twisted EE's body position and while doing so EE stepped on the air hose, and rolled EE's ankle. EE was taken to clinic for Xrays. EE was issued restrictions.

Struck by... NEC

EE was opening the door to the E/I shop. The door was jammed and EE attempted to pull the door when the handle broke off and cut EE's finger. EE was taken to clinic where EE received stitches to finger. EE was released to work.

Over-exertion NEC

On 3/23 EE was working on 11 Mill performing normal tasks. Began feeling lower back pain. On 3/24 EE informed supervisor & was sent home. EE on STD. On 5/2 EE informed HR EE wanted to file claim. On 5/3 EE was issued restrictions from clinic.

Over-exertion in pulling or pushing objects

On 3/12/2022 a EE went to open a valve at the Tailings Dam but it was stuck. EE tried to open it & felt a pop in the left side of chest. On 3/14/2022 EE was issued restrictions due to the pop on 3/12/22.

Struck by... NEC

EE was holding a piece of steel support while co-worker was cutting steel with cordless grinder. The grinder became caught and ran up the frame, making contact with EE's L pinky finger, causing a laceration. EE had 4 stitches.

Struck against a moving object

EE was operating Haul Truck 65 & pre-spotted on the right side of 43 shovel. Shovel operator stated that HT 65 was too far out & at an angle. Shovel operator released the trip & the corner of the dipper door hit the side of the bed. HT operator began to feel sharp pain in back area when this happened. EE was taken off work due to muscle strain.

Over-exertion in lifting objects

An employee was conducting clean up at the liners when the employee went to lift a motor. EE began to feel some pain and discomfort in EE's back later that day. EE was taken to the clinic and was issued restrictions for muscle strain.

An ee was switching lime pots & left a drain valve open. This caused the lime to spill out & fill the spill pad. The EE had a hole in boot & lime got into the boot & made contact with EE's foot. The lime caused multiple burns to the foot. EE was issued restrictions.

EE was haul truck mentor. Operator backed in on left side to get loaded. First dipper was loaded onto HT with no issue. The shovel swung onto HT on the second pass the dutchman made contact w/ bed tail. Mentor felt some stiffness. On 1/3 the mentor stated that their back was hurting still from the contact. Mentor was issued PT & attended first session on the 13th of January.

Shovel operator honked the horn and released the brakes that is when the 48 shovel lost power resulting in the dipper to fall into the back of the haul truck 66 bed. The operator was shaken around. EE did not seek medical treatment. On Dec 1st, EE informed supervision that EE had back pain. EE was taken to a medical facility and was taken off work.

Accident type, without injuries

Two EE's utilized the man elevator to come from the 5th floor, when the elevator approached the landing for lower floors the door limit switch would not activate.

Bodily reaction, NEC

EE was inspecting the Tailings Booster station containment area. EE bent over to access a sump pump and EE began to feel pain in EE's back and under EE's right shoulder blade. EE was seen at clinic and was issued restrictions.

Struck by rolling or sliding object

EE was assembling a 6"" knife valve. The EE was rebuilding valve parts that were staged next to the valve that was being rebuilt. The valve, which was leaning against a wooden crate, slid over and pinched the ee's finger between the parts and valve. The ee sustained a fracture to EE's finger and was issued restrictions.

Over-exertion NEC

EE BOOT CAUGHT IN LAST STEP OF GRATING CAUSING EE TO ROLL ANKLE. EE WAS ISSUED RESTRICTIONS.

Struck by falling object

Employee was tightening bolts overhead using an impact wrench. Employee looked up and debris fell into face and behind safety glasses, resulting in a foreign body entering right eye. Employee's eye was flushed but irritation continued. Employee was transported to eye clinic on 09/24/2021 where a metal foreign body was removed and antibiotic drops applied.

Struck by falling object

EE was removing nose cone and pin assembly from damaged pull yoke on Haul Truck. As EE was attempting to remove the nose cone bearing a piece of the broken bearing fell out of the assembly & struck the back of their hand. The caused a deep laceration. EE was issued restrictions & had 7 stitches in hand.

Struck against a moving object

EE was being loaded at the 43 shovel when a large boulder was dropped in EE's haul truck. The truck shook and the EE felt a pinched nerve/muscle in EE's neck along with EE's thumb going numb. EE was taken to the clinic the next morning and issue restrictions.

EE was conducting a WPE of the XS leach pad when EE stepped out of the truck and onto a rock. This caused the EE to roll their ankle & fall to the ground. After getting back in the truck EE stated that EE was in pain. EE was taken to clinic & diagnosed w/fracture to fibula. EE was taken off work.

Employees were attempting to remove an inspection door on the PAC Cell. The EE was descending down a set of stairs when a scaffolding pipe aprx 5 ft in length, that was leaning against the handrail, fell over and struck the EE on the lower part of the neck and upper back. EE was placed on restricted duty.

Employee slipped and reached out to steady self causing a wrist sprain.

EE was sweeping material into a pile for cleanup. They stepped backwards onto a softball size rock, rolling their ankle and spraining it. EE was issued restrictions.

EE was closing a valve on the tailings delivery line. EE stepped down and did not notice the cribbing supporting the pipe. EE stepped on cribbing and rolled ankle. EE was issued restrictions.

EE was getting loaded at the 43 shovel on the right side. The HT operator felt a slight pinch on the right side of neck while getting loaded. On 7/16/2021 EE was issued restrictions due to the rough loading incident.

EE was walking behind the secondary crusher and had reached down to remove wheel chocks. EE fell down injuring lower lumbar, right hip and left shoulder. Medical assistance was called and the EE was transported to a hospital for treatment.

EE was cutting some rope for a tagline. They had cut thru the rope and sliced the edge of right index finger. EE was taken to the clinic and issued restrictions.

EE was being loaded at the 45 shovel pit. A large rock came out of the bucket, jarring the truck. EE began to experience some back pain. EE was taken to clinic and issued restrictions.

Struck by falling object

Epiroc employee was welding a square stock steel brace in place to support one side of 81 Drill A-frame for shipping. The other brace was leaning against the A-frame to be welded next. The brace fell and hit the Epiroc technician on the back while they were welding the first brace. The square stock measurements are 3""X1/4""X8'. They received a Transverse Process Fracture.

The Tailings Thickener tunnels were full of slurry. An EE entered a tunnel with PPE on to close lines and got slurry into their boots. The EE began experiencing a burning sensation inside the boots on the legs. On 3/15/2021 EE was issued restrictions.

Accident type, without injuries

An operator reported that there was a brushfire burning in the canyon below the Half Million Tank. This is on the west side of the pit in an unmanned/unmined area. Fire cause to be determined however it appears that two power line touched and sparked from the excessive winds.

EE was walking down a dirt ramp when their right foot lost traction causing the EE to slip. EE attempted to break their fall by putting their right hand out to the side. EE made contact with the ground and sprained wrist. EE was issued restrictions.

Over-exertion NEC

EE was assisting in cleaning area in the center sump. EE began talking with another employee. A metal covering plate became dislodged and the EE fell into the hole as EE stepped on it. EE's foot fell in hole and twisted EE's ankle.

Struck by flying object

EE was throwing away a damaged pop up tent from the clinic screening area. The wind picked up the pop up tent and it flew back and struck the EE in the mouth. The EE sustained a chipped tooth that was repaired.

Fall onto or against objects

EE was removing a ladder from an empty cell on top of the cell line. When they turned around to move the EE slipped on one of the bars on the adjacent cell. Their leg came into contact with the concrete cell wall. EE scraped their leg. On 2/11/2021 EE was issued restrictions for leg contusion.

Over-exertion NEC

On 2/1/21 EE was standing to the side of the cathode unloading platform when several cathodes began to fall back. Employee stopped the movement of the cathodes with right hand & struggled against the weight of the cathodes. On 2/9/2021 EE reported to supervisor that their back hurt from that day. EE was issued restrictions on 02/09/2021.

Over-exertion NEC

While working inside scalping screen, EE was in an awkward position crawling inside the scalping screen. As EE got out, EE noticed some discomfort on EE's lower back. EE was taken to the clinic and issued restrictions for back pain.

Hearing loss deemed a mixture of occupational exposure and personal medical related issue.

Over-exertion NEC

On 12/3/2020 an employee approached a pickup truck and was trying to watch EE's footing as EE was getting in the truck. EE's foot slipped and EE twisted EE's knee. On 12/04/2020 ee was seen for knee pain and was issued restrictions for knee strain.

Over-exertion in lifting objects

EE began to feel stiffness in right lower back during shift around midnight. It began to get progressively worse throughout the night. EE was not seen until 12/07/2020 and issued restrictions for back strain.

EE was walking to the change room before shift. EE walked around the jersey barriers to avoid having to walk thru a group of Crusher personnel gathered on the walkway. EE stepped on parking lot pavement and stepped in a pothole. EE fell striking nose, mouth and knee. EE issued sutures for mouth laceration.

Over-exertion NEC

EE was attempting to close a bad valve on the cyclone skid that was releasing slurry. The EE slipped on the material the valve had been releasing, causing the EE to reach out and try to catch self on the cyclone skid frame. The EE felt sharp pain in the arm. EE was issued restrictions.

Over-exertion in pulling or pushing objects

An employee was moving some brass and metal steps used in the Tertiaries. EE began to feel sharp pain in their back. EE was taken to clinic and issued restrictions.

An ee was taking tools from the locker. The ee stepped backwards and ee's foot made contact with the dolly. The ee lost balance, began to fall and twisted ankle in the process. The ee was taken to the clinic and sustained a fracture to the leg.

After eating lunch, an ee was picking up lunch trash. Ee had a can of green beans with a pull top lid. When ee picked up the trash ee's thumb slid across the sharp edge of the lid and ee sustained a laceration to the bottom of thumb. Was given stitches and restrictions.

Struck against a moving object

On 09/24/2020 an operator was loading with a loader and jarred self on some heavy rock causing lower back pain. EE did not inform site. On 10/06/2020 EE notified supervision of lower back pain. Was issued restrictions on 10/06/2020.

Struck by flying object

EE was erecting a portable scaffolding platform right below the #1 cyclone when EE felt something go into eye. EE rinsed it immediately. Continued to feel something in eye. EE was taken to a clinic for evaluation. Was prescribed eye drops.

Over-exertion in wielding or throwing objects

EE was doing routine clean up at the Primary crusher basement when they felt a pop and sharp pain in shoulder. EE was shoveling rocks at the time. EE was issued restrictions.

Struck by falling object

EE was working in the Secondary crusher when debris went into EE's eye. The EE washed EE's eye out with water & continued working. EE went home & eye was irritated the next day. EE went to an optician and was prescribed antibiotics.

Accident type, without injuries

A fire was spotted by an equipment operator working near Special Place in the Half Million Gallon Tank area. A mine supervisor was notified and a mayday was called. Green Valley Fire Dept was contacted to extinguish the fire. A vulture had landed on a wire and went to ground. It sparked a fire that ignited the brush and caused the fire.

Contact with heat

An employee notified supervisor that employee was not feeling too well. Employee stated that employee was experiencing a headache and was a little dizzy. Employee was taken to the clinic and treated for heat exhaustion.

Fall from scaffolds, walkways, platforms

Injured Employee was working on a 32"" tall elevated platform without any handrail protection or fall protection. While hooking up a 1"" choker the tension of the choker pushed back and pushed the injured mployee off of the platform landing onto left side on the ground causing a fracture to the ribs.

Struck by... NEC

EE was removing a bolt from bearing housing. The impact was in place on the bolt. EE reached out to feel for the bolt head to set a backup wrench when EE lost EE's balance & accidentally pulled the trigger. This caused the bolt to turn grabbing EE's fingernail & ripping it off. EE was given restrictions

EE was walking from 6 Mill Cyclone Deck to 7 Mill for EE's rounds when EE stepped with EE's left foot & felt sudden pain as EE's foot bent inwards. The grating was bad which made EE step wrong. EE was taken off work.

EE was removing a pump from a barge for repairs. While removing the last bolt with an impact, the pump shifted downward and the impact came into contact with the metal structure. EE's left middle finger was pinched between the impact & metal support. EE fractured finger.

EE was ball charging #12 mill. EE found the ball bin was plugged with seized 3 1/2 inch steel balls. EE attempted to dislodge the plug by throwing a freed steel ball up the chute. EE's hand came into contact w/the steel chute plate. EE fractured the pinky finger.

Struck by flying object

EE was hydro blasting the FC-7 drain line. EE was hit with high pressure water to right index finger, middle finger and palm while snaking out the drain line. EE was taken to a specialist and bandaged. EE was taken off work and received restrictions.

EE was fueling a truck when they called a supervisor stating, wasn't feeling well and was dizzy. EE fell backwards and hit head on ground. EE had contusion on head.

Employee was assisting in moving the deposition spread. When employee was opening and closing valves, employee started to experience right shoulder pain.

An employee was entering the Cu Moly Thickener pad area when they stepped on a hose in a dimly lit area and rolled their ankle. The EE was taken to a medical facility for evaluation. EE was taken off work and given restrictions.

EE was shoveling fine ore material in the Secondary Crushing plant when they began to feel a warm sharp pain in the upper quadrant of rib cage. EE was taken to a medical facility where EE was issued restrictions.

The bleeder 195 had gone bad. The EE opened a bleeder & closed it. In the process of closing the bleeder the EE was sprayed with chemical. It went into their eye & passed their safety glasses. EE flushed eye with water. EE continued to rub eye; scratched retina.

EE was performing a pre-operational inspection on a drill when EE fell onto left shoulder. EE was taken to the clinic and given restrictions.

Caught in, under or between a moving and a stationary object

EE was rolling a stand out to the dozer push arm on a dolly. As EE was setting the dolly down the stand began to move forward toward the dozer push arm. The EE put EE's hand up to stop the stand and it became smashed in between the stand and the dozer push arm. EE sustained laceration to finger. Stitches were issued and EE put on restrictions.

Over-exertion NEC

EE was at the West Tiedown for a bathroom break. EE was descending the ladder. As EE's left leg touched the ground EE turned felt a pop in EE's left knee. EE notified supervisor & taken to the clinic for evaluation. EE sustained a meniscus injury. Was given restrictions.
