Struck by falling object

ON OCT.7, 2002, A 31 YEAR OLD MINER WAS FATALLY INJURED BY THE FALL OF A SINGLE ROCK/CLAY MASS FROM LOOSE UNCONSOLIDATED MATERIAL LOCATED ABOVE THE HIGHWALL. RAINFALL HAD OCCURRED DURING THE EARLY MORNING HOURS & WAS REPORTED TO HAVE BEEN HEAVY JUST PRIOR TO THE START OF THE 6AM SHIFT. MINER WAS HAND SHOVELING RAW COAL FROM THE BASE TO SCOOP INTO LOADER. NOT WEARING A HARD HAT.

Employee was steam cleaning truck # 212 while climbing front ladder on truck. He fell backwards approx. 4 feet landing on Tail & Back complaining of back pain.

Employee was on a drill and was having trouble with the chain guide. His left third and fouth fingers were smashed between the chain and the chain guide.

Employee was placing diesel in his truck. He inserted the nozzle into the tank and turned on the switch. The nozzle popped out and sprayed into his right eye.

Employee was helping place an 8"" pump suction hose in the water. He lifted it to his shoulder and felt a pain in his back.
