ON OCT.7, 2002, A 31 YEAR OLD MINER WAS FATALLY INJURED BY THE FALL OF A SINGLE ROCK/CLAY MASS FROM LOOSE UNCONSOLIDATED MATERIAL LOCATED ABOVE THE HIGHWALL. RAINFALL HAD OCCURRED DURING THE EARLY MORNING HOURS & WAS REPORTED TO HAVE BEEN HEAVY JUST PRIOR TO THE START OF THE 6AM SHIFT. MINER WAS HAND SHOVELING RAW COAL FROM THE BASE TO SCOOP INTO LOADER. NOT WEARING A HARD HAT.
Employee was climing ladder onto equipment, slipped and fell to ground landing hard on left foot.
Employee was helping other employees reattach a bull hose to a pump using a crow's foot which became unlodeged letting the bull hose swing around & knock employee to the ground hurting his side, shoulder & back.
EE was climbing up on his boom truck to get blocks to crib up truck and he slipped and fell.
While operating Dozer, employee ran over rock with blade up and it threw him into door handle, where he hit his face. Contusion to nose & mouth.
Employee was pulling fuel hose out of truck & pulled something loose in Right Shoulder.

Employee pulled ladder down on equipment in order to climb up. Ladder did not go all the way down, when employee attempted to go up ladder, his foot slipped and caused his Left Knee to hit ladder.
Employee was climbing off of equipment & fell off ladder landing to the ground. He complained of Lower hip & back pain.

EE WAS TRAVELING IN A WATER WAGON TOWARDS CARTER #2 MINE AND WAS TRYING TO SLOW DOWN. THE BRAKES LOCKEC UP AND THE TRUCK ROLLED OVER.
EE HURT KNEE CRAWLING OUT FROM UNDER A TRUCK.
EE STEPPED DOWN FROM A DOZER AND SPRAINED HIS LEFT ANKLE.
EE WAS LOADING WOOD BLOCKS. ON 6'6"" FELL ON HIS LEFT FOOT/BIG TOE.
