Caught in, under or between a moving and a stationary object

Employee adjusted the grizzly bars at the primary crusher and failed to close the access door before the feeder started back up. While closing it a rock fell onto the door and injured EE's finger, requiring two stitches.

On April 21, 2016 employee reported to supervisor that ee injured back the prior week when ee was pulling on a wrench while installing a fan motor on a drill.

Employee was helping with maintenance on feeder. While climbing on the second step of a four foot , the ladder shifted and caused him to fall back against the hitch on the welding truck. Employee suffered from a cracked rib and contusion to his back.
