The operator of the Volvo EC380 Excavator was on the ground fueling the machine with a cord (24v-used to fuel machine as needed) and the male end was not covered to prevent contact with metal. The male end came in contact with the metal frame causing a arc flash. The operator was scared, jumped back and tripped falling to the ground.
While wearing safety glasses, employee was working around grinding, cutting and welding during the work day, causing dry irritated eyes. Medical treatment by diagnostic eye exam and prescription eye drops for employee.
While resetting # 2 soft start motor worker placed forearm on or near energized electrical circuit and was shocked.
Employee was descending stone pile to measure face, slipped on loose outer stone and slid to ground, catching his weight with his left hand, causing pain to his left shoulder.
Was walking up catwalk to replace screen on shaker left leg got caught in a bar, bent down to move bar stumbled, took a step & felt sharp pain in left knee. Sprained left knee - went to Dr 6/6/11
Employee was getting into loader seat holding onto door. A strong wind caught door, closing it suddenly causing employee to strike his head on window latch inside cab of loader causing laceration to head.
