Miner states was bit by a tick on the back of the neck while inspecting fire extinguishers at the pug mill located at the O'Neal Quarry on 06-13-2024. EE was absent from work on Monday 06-24-2024 and has not returned to work.

Fall down stairs

As EE was exiting the tower using the stairway, EE rolled right ankle off the edge of the stair tread causing EE to fall backwards hitting head.

Employee was installing a motor on the primary feeder. As the motor was being positioned onto the motor base, it shifted and pinched the tip of the employees left ring finger between the motor and an adjacent beam.

Employee was coupling railcar airlines. When air was applied to the airline it separated at the coupling. The airline coupling swung back and hit his left middle finger causing a cut that needed 10 stitches.
