Struck by flying object

Employee was lying on his back, micing bores on 12CM12 mainframe CM97. A gust of wind blew debris from yard into his right side of face and into his eye. Received prescription.

Employee was assisting in the turning over of the stage loader drive assembly. Employee had his hand on the drive as it turned. The drive was approximately 3/4 ways turned over when the unbolted deck plate turned loose, striking employee's left middle finger.

Employee was burning with a torch. He was not wearing a welding jacket or apron. He was bent over a job when the oxygen line broke and saturated two layers of clothing and cutting sparks or flame ignited, causing second degree burns on abdomen and neck.

EE lifted cover improperly with chain hook in the hole and it would not fit in cover had already fell off once and he hooked it the same again. Then get cover in place and let the crane down, relasing tension. Cover folded and fell on his. where he was standing in front of the cover. He was knocked down and the cover landed in his leg, the hinge area striking him.
