Over-exertion (Not Elsewhere Classified)

The employee was clearing rocks jammed in between the grizzly bars on the grizzly bar screen with a sledge hammer and strained lower back. The employee cleared a few rocks and went to bend down to reach for the sledge hammer and felt a ""pop"" in his lower back. The employee didn't report the injury until 10/27/2008.

Employee was ascending vehicle steps - heard ""pop like a balloon"" and began experiencing severe pain. This injury was not reported in a timely manner. Employee notified plant personnal on 10/14/2005.

Contact with hot objects or substances

While cutting bolts from liner plates the torch burned back into the tip. Employee purged torch and let it cool off. When employee re-ignited torch, it burned through his glove. Employee had recently removed a piece of hot slag that had fallen into his glove - liner could have been smoldering and oxygen from torch could have accelerated it.

Struck by falling object

EMPLOYEE WAS USING A BAR TO REMOVE A ROCK (APPROX 10 LBS) FROM A CONVEYOR TRUSS. THE ROCK CAME LOOSE, BOUNCED OFF THE CATWALK STRIKING HIM ON THE SHIN CAUSING A SMALL LACERATION.
