The injured miner & the supervisor were attempting to light the #2 kiln's gas flame. While the miner was holding a flare attached to a metal pole at the end of the gas pipe, the supervisor turned on the gas. In the process of lighting, the gas ignited & blew back through the front of the kiln causing burns to the right side of the miner's body. On 5/28/18 the EE passed away.

Employee was sweating due to humid environment and while using a grinder EE attempted to wipe sweat out of EE's eye and the grinder wheel cut EE's hand. Employee was taken to Shelby Baptist Medical center and received 8 stitches. EE returned to work the following day.
Employee was working in basement area, around the pan conveyor, at Lhoist Alabaster Plant. Employee rose from a crouched position & bumped the wall & wire that was hanging from the ceiling of basement with shoulder. This caused calcium hydroxide that was attached to the wire to fall & make contact with employee's left eye, going around the safety glasses employee was wearing.
On 5/11/2019 the employee was raking grates during the shift and then went to the top of K2 coal bin to clear a plugged chute. While bending over the chute with a rod that was used to clear the opening, the employee felt pain in EE's back. After prolonged pain in EE's back the employee sought medical attention on 6/19/19.
Employee was working inside an open hopper when rock was dumped into the hopper. This caused rock to contact employee's legs resulting in bruising and hyper extension of the right ankle.

Employee was cleaning out from under the rail scales, he was shovelling lime into a 5 gallon bucket to be dumped. Lime fell down into the employee's pants and he suffered 2nd degree burns across his buttocks and one of his legs.
EE was going to plug the kiln, at first he put the plug in the feed pipe backwards so he had to reposition it. When he went to re position the plug the t-handle on the plug caught his hand between the conveyor belt and the bar. Causing burns and breaking 2 fingers.

Employee was removing a damper blade from kiln 2 scrubber and pulled his lower back. He didn't go to the doctor until 12/31/08 and was placed on restricted work duty.
Employee was re-installing trunion roll housing cap, using bars to roll it over to align bolts and holes. The cap rolled and smashed employee's hand and middle finger.
Employee was removing build-up from a pulley with a bar when the bar caught in the belt and struck his left arm.
Employee was closing the door on the bottom of a railcar and twisted the lower part of his back. He did not go to the doctor until 4/2/08, at which time he was placed on restricted work duty.

Injured was pulling on a rail switch that had been run over and broken when he strained his back. Accident became lost time on 9/16/05 when it was discovered that he has a bulged disc in his back.
While working on a water line in the ground, lifted a 3 inch piece of pipe, put it on shoulder, his boss had the other end, injured his neck.

Injured was attempting to put a drawhead back on a railcar. While holding the drawhead to put into position it slipped causing a strain to injured's left groin. Became reportable on 11-2-04 when injured was placed under restriction of no lifting by physician.
DUMPED WHEELBARROW HALF FULL OF COAL REJECT. DID NOT FEEL CATCH IN HIS BACK UNTIL HE STARTED UP THE STEPS.
Injured was working off of a ladder removing bolts from a pipe flange. Pipe came loose from flange striking injured and knocking him off of the ladder.

INJURED AND CO-WORKER WERE REPLACING MID-KILN THERMALCOUPLE ON #1 KILN. WHEN PUTTING THE PROBE IN THE KILN, HE WAS ABOUT TO TURN THE PROBE TO LINE IT UP WHEN CO-WORKER PUSHED THE PROBE SMASHIN G FINGER BETWEEN FLANGES.
