Employees were removing empty oil drums from service truck. Injured employee was stepping down from bed onto bumper step. As he was stepping down form the bumper, his foot slipped and he fell to the ground landing on his hand.

Employee and co-worker were attempting to load a full acetylene bottle onto the racks of the boom truck. Injured employee was lifting the bottom of the bottle when he heard his back pop.

EE WAS CLEANING ROCK BUILD UP FROM UNDER FEED TUBE. THE TUBE WAS RAISED UP TO CLEAN ROCK FROM TOP OF ADJUSTMENT RING COVER. THE TUBE WAS HOOKED TO BOOM TRUCK WITH 2 WAY SPREADER CHAINS. WHEN ANOTHER EE CLEARED A ROCK THE TUBE ROCKED FORWARD AND STRUCK EE'S LEG, PINNING HIS LEG BETWEEN FEED TUBE AND ADJUSTMENT RING COVER.

EE HAD WORKED IN THE WELDING SHOP GRINDING ON A PIECE OF STEEL. HE HAD ALSO WELDED FLANGES ON 8""PIPE. EE LEFT WORK AT 4:30PM. AROUND 9:00PM LT. EYE STARTED TO HURT BAD ENOUGH THAT EE FELT HE NEEDED TO GO TO E.R. DR. REMOVED A PIECE OF METAL FROM LT. EYE.
WHILE TOSSING A WRENCH TO ANOTHER VULCAN EE ABOVE HIS HEAD, INJURED EE STATED THAT HE PULLED OR STRAINED HIS KNEE. EE WAS INSTALLING USED SECTION OF CONVEYOR.
