EE stated he had been to #3 rock bin to inspect the chute of #4 refuse belt. Witness: EE was on the 1st or 2nd step of a small ladder w/backward fall protection when he ""blacked out"".*He slipped from ladder landing on feet & fell on his back*. He never lost consciousness. We are waiting for medical records to determine the cause.*On 12/18/07 he had surgery & he died on 12/19/07*

Employee was replacing rollers when his finger was caught between two rollers, causing a laceration that required sutures.
Original accident: EE was pulling air doors open & felt a pop in both wrist & shoulders & neck. EE has been working since accident & was taken off 3/12/14 due to surgery on neck. As of 9/1/13 EE was working for North River Mine (0100759). North River Mine has since closed & EE was transferred to Mine #7 (0101401). I'm filing this report under the mine the acc originally occurred.
Employee complained with back pain after normal work duties. A MRI was performed which indicated degenerative disc damage. A previous MRI from 2005 showed many similarities. Employee did not lose time until 09/09/2013 due to doctor's exam.
Employee fell while going through mandoors.
Employee was walking his fireboss rounds when he stepped in a hole and felt pain in lower back. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT AND DID NOT START LOOSING TIME UNTIL 7/9/14 WHEN HE WENT OFF WORK DUE TO SURGERY.
Employee was walking off face when a rock struck him in the mouth, breaking two teeth.
Employee stated that he was getting socket sets out of tool box when a set fell out of his hand and struck his finger. This caused a laceration that required sutures.
The Cedar Creek Elevator was downed at approximately 8:45 pm due to a faulty door lock switch.
Employee stated that he was moving an oxygen bottle, when the top came off and struck him in the mouth.
Employee stated that while he was putting torches into the scoop bucket, he felt a pain in his belly. His first lost workday did not occur until 03/01/2013 due to surgery.
A fire was discovered in the engine compartment of #2 dozer. It lasted approximately 35 minutes.
Employee stated that he was using a pry bar when the bar slipped and struck employee in the left side of his bottom lip. This resulted in a laceration that required sutures.

A roof fall has occurred in the intersection of Flat Creek Main #6 entry between spad #33+25 and #36+05. The fall measured 180 ft long, 20 ft wide, and 8 ft high.
The Cedar Creek Elevator was down from 1:40 am until 4:00 am due to motor brake release fault.
Employee stated that he was stepping over a retaining wall when he twisted his knee. An MRI showed a torn meniscus. His first lost work day did not occur until 10/29/2012.
Employee stated that he was moving mantrip when batteries shifted, catching his finger between frame of mantrip and batteries.
A fire occurred in the engine compartment of a diesel lo trac. The fire burned for approximately 12 minutes.
Employee stated that while changing a top frame, he pinched his finger between the frame and the belt structure.
Employee stated that he was carrying a 5 gallon bucket of oil to dozer and felt a pain in his back. He went to the doctor a couple of times and was taken off of work on July 27, 2012.
Employee stated that he was replacing a rubber hose. When he cut through the hose, the knife cut his arm. It required 4 sutures to close the laceration.
A roof fall was discovered in Flat Creek Main return. It was approx 10' x 8' x 15'. Some of it was above the anchorage point.
Employee was sent a letter stating that he had evidence of pneumoconiosis. Employee, MIIN M00423747, had waived his rights as a Part 90 miner.
Employee stated that he was removing a broken metal band when it came out of the tool and struck the employee in the hand.

A roof fall has occurred in the intersection of Flat Creek 8 #2 entry in the intersection at spad #97+10. The fall measured 35 ft long by 28 ft wide by 5 1/2 ft high.
Employee was working bent over. When he raised up, he felt a pain in his back.
Employee was walking down slope, slipped and fell causing compound fracture of right ankle
A roof fall occurred behind the wash down bay of the motor pit. It was 3 ft wide by 8 ft long by 8 ft high.
The slope hoist was down from 6:00 am on 8/14/2011 until 12:40 pm on 8/15/2011 due to the tachnometer on the drive.
Employee stated that he was stepping down when all of his weight shifted to the edge of his foot.
A rock fall occurred on Flat Creek 6 Section. It was in the intersection of spad 7+32. It measured approximately 19 ft wide by 20 ft long by 7 ft high.

Employee stated that he struck his head against T board. An MRI showed calcium spurring on several discs and disc protusion. Employee was placed off work on 8/5/2011 due to surgery.
Employee stated that he was tramming a scoop when he hit a hole and struck his head on the canopy.

Employee stated that he was holding a bar up that holds cylinders in place on the refuge chamber cylinder racks while another employee tightened the bolts with an impact wrench. The employee's finger got caught in a pinch point.
Employee stated that he bolting a strap on a locomotive when his hand slipped and cut his thumb on a piece of metal.
Employee stated that he had his hand on side of the canopy while shield was being functioned. This caused him to mash his left fingers, resulting in sutures.
Employee stated that he was in the process of moving a chain fall from above a chute when he slipped and straddled the chute.
Mine examiners discovered a small fire at crosscut 7 on North Main 3 belt which was extinguished with fire extinguishers and fire hose.

Employee stated that he got his feet tangled up in the booster pump hoses and fell, twisting his knee.
Employee stated that he tripped over a water hose & fell. An MRI showed moderate degenerative changes & ACL partial tears. This will be the third surgery on his knee due to degenerative changes & ACL issues. Employee's first lost work day did not occur until 9/8/2010 due to surgery.
Employee stated that he was walking by the power center and tripped. He fell, striking his face on the ground.
As the hoist operator was bringing two supply cars out, they became uncoupled and dropped into the slope bottom.
Employee stated that he was working on the feeder when a ram car trammed near him. The oil compartment caught his foot between the compartment and the coal floor. His FLWD did not occur until 03/17/2010 due to surgery.
The hoist operator had droped the Barney car into the bottom. The right rear wheel had came off and caused the car to wreck. When discovery of the wheel missing, all employees were notified to evacuate the mine. The hoist was our secondary mechanical means of escape.
The supervisor of the long-wall crew was walking down the #2 entry and had started back to the face area. He saw a dribble of rock, looked up and a rock fell, stricking him on his left side head, ear, and shoulder. Employee recieved lacerations, requiring sutuers.
The Cedar Creek elevator was out of service from 7:00 am until 11:30 am due to a faulty contactor.
Employee stated that when they let an end of the monorail down, the other side slid off. The employee's thumb was caught on top of the motor.

Acceptable conditions- Employee was removing equipment from belt structure and got his finger caught between two pieces of equipment causing a fracture to the left pinky finger.
Employee was assisting in replacing the boom lift jack on the Fletcher Roof Bolter when the boom fell fracturing his right arm.
The Cedar Creek Elevator was out of service from 6:53 am until 9:46 am due to an auxilary brake contactor fault.
The Cedar Creek elevator was out of service from 8:20 PM until 11:50 PM due to a faulty switch.
During Longwall recovery, employee was helping pull hoses off face with the use of a scoop and chain. The chain slipped off the end of the hose, causing the hose to spring back and strike the employee in the leg.
Employee stated that he was opening airlock travel doors when his knee ""gave out"" & he fell into the door. The treating physician says that his knees were ""bone to bone"". His first lost work day did not occur until 9/24/09 due to doctor's orders.
Employee stated that he was about to start pinning when a piece of draw rock fell and struck the canopy. It slid off the canopy and struck the individual.
Employee stated that he was loading a Kennedy brattice when he stepped on a rail and twisted his ankle. Employee was placed on restricted duty on 08/20/09.
Employee stated that he was walking down a overcast stairway when his foot slipped off of one of the steps. An MRI was taken and the doctor placed the employee on restricted duty 8/10/09.
Employee stated that he was assisting another employee with a wrecked jeep when the jack they were using, came out from under mantrip and struck the employee on the left elbow. EE FLWD was 7/13/09.

Employee was raking on Flat Creek Main beltline when he felt a burning in his left shoulder. Employee's first lost work day was 9/25/09 due to surgery.
The Cedar Creek Service Elevator was taken out of service due to wear on the ropes of the elevator.
Employee stated that he was pulling the miner cable over with a cable hook and felt a pain in his lower back. The employee was diagnosed with two herniated discs in his back and placed on restricted duty on April 20, 2009.
Employee stated that he was moving a pump with a metal bar when he stepped on some loose coal and lost his footing. The pump shifted and caused the bar to jerk his arm. Employee's FLWD did not occur until April 15, 2009.
Employee stated that he was pulling 100 ft of suction line over the overcast when he felt his right knee pop. Employee first LWD did not occur until 5/28/09 due to surgery.
Employee stated that he was jacking out the cat tracks on the mobile tailpiece with a grease gun when the grease fitting broke and struck employee in the face.
Employee stated that he was helping move the miner cable when he felt a strain in his right shoulder. Employee's FLWD was not until 4/29/09 due to surgery.
Supervisor was making prestart face exam and found low volume of air on face. Went to bleeder and found two brattices damaged, most likely from fall of gob. Notified CP person who began withdrawal of Miners, except those needed to repair brattice.
Employee was assisting another employee lift a part from a platform to a loading dock. Employee's foot became entangled in the overhead hoist chain, which resulted in employee tripping and falling backwards off the platform.
Employee stated that he was picking up a bucket of plaster with both hands when he felt a pain in both arms. Employee's FLWD did not occur until 2/23/09.
The Longwall experienced the first fall of roof in the gob behind the shields. The fall damaged a few ventilation devices behind the set up entry. A brattice, a set of run through doors, a regulator, and 9 curtains were damaged.
Employee was using a com-a-long to raise a pipe in order to tighten support chain. A clevis that was supporting the com-a-long spread apart, allowing the com-a-long and pipe to drop, pinching employee's finger between pipe and support chain.
Employee was in the process of preparing base lift jack for shield removal. The saddle that holds the base lift jack vertical fell, striking/pinching employee's finger between the saddle and the base lift jack.
Employee was operating scoop in the face area. Coworker saw scoop not moving and went to check. Found operator unresponsive with shallow breathing and pulse. Activated EMS system. Crew began CPR and transported out of the mine. Employe was transported to hospital where he was pronounced dead of Myocardial infarction (MI)
Two persons were trapped on the elevator from 12:05 to approximately 1:35 due to a fault in the PLC of Cedar Creek Elevator.

The Tyro Creek Service Elevator was out of service from 9:00 am to 1:45 pm due to power outage.
The Cedar Creek elevator was out of service from 6:15 am to 8:30 am due to a problem with operating in the inspection mode.
The hoist rope for the Flat Creek Main Emergency Escape Capsule broke during a functionality test. Ice had accumulated on the guide ropes which stopped the capsule's descent, allowed slack to gather in the rope, and allowed the capsule to drop the corresponding distance of the slack in the rope, which resulted in breakage of the hoist rope.
Employee stated that he was lifting top idlers for the belt set up and loading them on a skid. When he tried to lift a roller, two rollers were struck together, causing a sharp pain in his right shoulder. Employee's FLWD did not occur until 1/7/09 due to surgery.
Employee stated that he and a contract employee were moving eicotrac when the contract employee turned the eicotrac and caught the employee's finger between the panline and the eicotrac.
As employee was riding mantrip, he raised his left arm up to shield himself from the run through track drops. As mantrip travelled through, his left arm was pulled back over his head.
Employee stated that he got off the ram car and was walking to the dinner hole when he tripped over a waterline. He put his arms out to catch himself and felt a pain in both shoulders. Employee's FLWD did not occur until 1/7/09 due to surgery.
Employee stated that he was installing a Kennedy stopping, when a rock fell from the roof and struck the employee on right hand and leg just above the ankle. Employee's FLWD did not occur until 10/11/08 due to surgery.
Employee stated that he was helping hang the miner cable in # 1 entry of Flat Creek 3 section, when he felt a burning sensation in both shoulders. Employee's FLWD occurred on 10/24/2008 due to surgery to correct the condition.
Employee was conducting a fan check. As he was walking around a concrete form in the early morning light he walked in to and struck some spare fan housing panels, resulting in his two front teeth being chipped.
Injured employee and three other coworkers were preparing to build a brattice. Injured employee was taking the plastic shrink wrapping from a pallet of blocks. As employee was removing wrapping, a block became dislodged from the pallet, fell, and struck employee on the top of the right foot, resulting in a contusion.
Employee stated that he grabbed the chain to a valve block and it was not secure. The block fell on the employee's leg causing a laceration. This resulted in 10 sutures.
Employee stated that he was loading roof jacks onto rail car when he felt a pain in his back. He was diagnosed with disc herniation in his back. His FLWD was not until July 1, 2008 due to surgery.

Employee was operating longwall shearer when he was struck on left side of face by rock, fracturing tooth.
Employee stated that he was cutting a metal band on a roll of belt at Flat Creek No. 2 belt header when a loose end of the band struck him in the face.
Employee stated that he was pulling a fly pad down. While pulling the pad down, his hand slid across a nail resulting in a laceration that required sutures.
Employee was loading belt wiper on to end of supply car. When he set it down on the car it slid over into the drive motor that was already on the supply car and pinched his pinky finger on right hand.
Employee was helping splice West Main belt and changed the blade in his cutting knife, which he thought he had closed the blade in the knife. While he was working on the belt splice his right hand came in contact with the exposed blade cutting the side of his hand where his pinky finger is.

Employee was helping other employees maneuver outby Pinner cable and plug over other cables near the section power center. As he was pulling the plug over another cable he felt a sharp pain in his abdomen. Later diagnosed as a hernia.

Employee stated that he was loading bags of gravel onto a material car when one bag slipped. He attempted to catch it and felt a pull on his shoulder. Employee's first lost work day was 01/17/08 due to surgery.
At approx 6:00pm, a water inundation occurred, due to the breaching of Don 25 seal
Employee stated that he was kneeled down beside the track switch leading into flat creek 1 section. When ee attempted to stand up, his foot slipped and his right knee came into contact with the switch or rail. This caused a laceration to the side of his knee which required sutures.

Employee stated that he was moving the feeder cable to repostion the remote cable when he felt his shoulder pop. Employee's first lost work day was not until March 4, 2008 due to surgery.
Employee stated that she was kneeling down while she replaced bits on the shearer. When she rose up, she felt a pain in her lower back. She was placed on restricted duty on October 4, 2007.
Employee was walking from H-Main Seal area where the walkway was very muddy. Employee slipped, twisting left knee.
Employee stated that he was making up pins when a rock fell between the pins and struck the employee. His first lost work day was 9/4/07.
Employee stated that he was stepping across a pile of ""gob"" when his foot struck on piece of rock. He was placed on restricted duty on 9/7/07.
EE stated that he was trying to move a flight ahead to help release tension. As he picked up the filght, it slipped catching the employee's finger between the flight and the stage loader flight. The employee returned to work full duty the next day.

Employee was swapping out supply cars. He was standing near the rail to uncouple the cars. When the cars separated, the car rolled forward taking the slack out of the chain which had the cars secured and caught the employee's left foot. He was placed on restricted duty 7/24/07.

Employee stated that he was attempting to remove bolts from ranging arms. He used a wrench as a cheater bar. The wrench slipped & struck him in the nose. He received 11 stitches.

Employee was walking in the motor pit when he hit his head on a roof bolt. He was diagnosed with cervical strain. He was placed on restricted duty on May 18, 2007.
Employee was trying to loosen a roof bolt plate that hung under a rock while doing overcast work. Employee exerted extra force to free plate and stated he felt pain in left arm. When he released the roof bolt his arm continued back striking rib rock.

Employee stated that while loosening a plug on gear box, he hit his head on side of gear box causing a laceration to the left ear.

Employee has been diagnosed with bilateral Carpal Tunnel Syndrome. He is currently off of work pending surgery. Employee FLWD was 2/5/2007.
The Cedar Creek elevator was out of service from 8:00 am on 1/31/07 to 2/1/07 at 8:35 am due to a problem with the doors not functioning properly. From the Kemper was on site and adjusted the doors and instabled/a new operator which corrected the problem.

The cedar creek elevator was out of service on 1/24/07 for 3:05am to 5:30am due to problem with the door not functioning properly. Bagby made the necessary repairs & placed the elevator back in service at 5:30am 1/24/07.
The cedar creek elevator was out of service from 10:15pm to 11:15pm on 1/22/07 due to problem with the e-stop. Bagby reset the elevator drive system and placed elevator back in service at 11:15pm
Employee stated the weld broke that was used to pull slack out of belt causing the eye and chain to strike him in the head. This resulted in a laceration and a chipped tooth.
The Cedar Creek elevator was down from 11:05 pm on 1/7/07 until 2:30 am on 1/8/07 due to a problem with the outer door rollers.

The Tyro Creek elevator was down from 9:55 pm to 10:45 pm due to thrown breaker.
The Tyro Creek elevator was down from 12:20 pm to 11:20 pm as a result of a power outage. When power was restored, Bagby elevator was called to make some adjustments & check-out the systems.
Employee stated that he was attempting to cut a plastic tie when the knife slipped, cutting his wrist. The laceration was closed with sutures.
The Tyro Creek Elevator was down from 3 pm to 7:15 pm due to the brakes being out of adjustment causing them to lock up. Bagby was called & repaired the problem & tested the elevator & everything checked out okay.
The Tyro Creek elevator was down from 10:30 pm on 10/17/06 to 1 am on 10/18/06 due to insufficient airgap on brake causing the brakes to overheat. Bagby was called & corrected the problem, tested & placed back in service.
Employee stated he was helping another employee put up racks in the storage building, the other employee fell. While helping him up the racks fell against the EE, he fell to the ground causing him to hit his left shoulder against the rail. His FLWD did not occur until Nov. 10, 2006.
Employee stated he was helping another ee install shelving at the prep plant when some of the boards he was standing on fell through. He has been diagnosed with cervical degeneration of the spine, arthritis and a cyst in his lt. shoulder that was aggravated by his fall. His first lost work day did not occur until 11/22/06.
Cedar Creek elevator is out of service as of 9/13/06 due to electrical problems. Bagby Elevator Company examined the service elevator, but did not have the part needed to repair it. We are presently still in search of part to repair elevator.
A roof fall has occurred in the intersection of L Main entry #6 at spad 57 + 38. The fall measured 21' long x 19' wide and approx 8' thick.
EE stated that while releasing a belt clamp by lowering the belt clamp support jack, the belt clamp did not lower because it became ""hung up"". EE placed right hand under belt clamp and belt clamp ""freed"" and fell apprx 6"", striking EE's hand. This resulted in two sutures.
Employee stated that he was repositioning a high voltage cable when he felt a pain in his shoulder. The employee's first lost work day was January 18, 2007 due to surgery.
The Cedar Creek Service Elevator was out of service on 7-31-06 from 9:00 pm until 11:15 pm due to slack in the braking system. Bagby Electric Company adjusted the braking system and the elevator was put back in service at 11:15 pm on 7-31-06.
The Tyro Creek elevator was down from 2:30 until 4:15am due to limit switch.

The cedar Creek Service Elevator was out of service on 7-19-06 from 8:45 pm to 9:40 am due to electrical problems. The cedar creek elevator was also out of service on 7/20/06 from 1:30 am to 4:30 am due to door limit switch problem. Bag by elevator co. was called and repaired both items
The typro creek service elevator was out of service on 7/17/06 from 10:47 pm until 7-18-06 at 1:40 am due to slack in braking system. Bag by elevator co. adjusted the braking system and then elevator was pit pack in service at 1:40 am on 7-18-06.
Employee stated that he was washing on the slope. He stepped on a mandoor that gave way causing him to fall. He was diagnosed with a rotator cuff tear. His first last work day did not occur until 7/20/06 due to surgery.
The Cedar Creek service elevator was out of service from approx 4:15 am to 8:00 am due to problems with power supply to drive.
