Struck by flying object

The employee and a contractor had placed the left track back onto an Hitachi Excavator and were in the process of fastening the ends of the track with a pin. While the pin was being driven into place a fragment of steel stuck our employee in the left arm.

Struck by flying object

EE were attempting to walk the track back onto the right track.Two slivers of metal flew off & hit the ee. 1 piece hit his shirt & other stuck into upper portion of his left forearm.Piece of metal was approx. 1/4"" long & embedded into his arm. he was taken immediately to primed where his arm was x rayed and the metal removed. he received 3 stitches in his arm.

LEFT OPERATORS CAB TO CHECK HYDRAULIC AT REAR OF MACHINES WALKED TO BACK ON TRACKS, RETURNING TO CAB WALKED ON TRACKS ATTEMPTING TO ENTER CAB,MISSED GRAB BAR FEET FELL FROM UNDER HIM, FELL 3 FT TO GROUND LANDING ON BACK

EE WAS CLEANING THE SHOP AND STEPPED IN A SPOT WHERE WATER HAD BEEN STANDING AND FOOT SLIPPED AND HE FELL CAUSING INJURY TO LEFT KNEE.
