IE was completing a pre-op on a rock truck when they slipped and fell. When the employee fell, they felt a pain in right shoulder. The employee finished the shift and requested treatment on 8-7-25
The conditions was hot, damp and clean. The injury was to the left foot at the toe, the damage was a miner cut and bruising followed by increased pain and swelling later that day.
The 980M Cat loader was discovered to be on fire at the Preparation Plant. After investigation it was determined that the fire did not burn for more than 30 minutes from discovery deeming it a non-reportable. MSHA was notified by precaution and investigated by MSHA. The fire appeared to have started from an electrical issue in the cab of the machine.

IE was walking to a pump on the 1st floor of the prep plant when IE stepped into a drain hole that the grating had moved from. The IE hit knee on the edge of the concrete.
IE was carrying flume screens down steps when IE missed the bottom step and fell to knees. IE did not seek medical attention until 5-25-21.
IE was switching from the D9N dozer to the D9L dozer when EE felt a pain in their left shoulder. IE was taken to OHC and is set up to follow up. IE has not returned to work.
A D-10 Dozer was discovered on fire at the Prep Plant. The dozer could not be extinguished by employees at the Prep Plant. The local fire department came and extinguished the fire. There were no injuries. MSHA was notified via the hotline and they also investigated the fire.

IE had just cleaned the scalping screen and was coming down the steps. IE slipped down the last 4 stairs and fell. IE was taken to be checked out.
IE was cleaning the material from the top of the scalping screen. IE was swinging a pick and an angle due to the chute of the reclaim belt to dig the material from the screen and IE felt a sharp pain in lower back. IE wanted to be taken to checked out.
IE was changing rollers on a beltline when EE felt a pain in EE's lower and middle back. IE did not request attention until the end of EE's shift.
IE was running a rock truck. IE went over a rut and bounced up hitting head on the top of the truck. IE finished shift and waited until 3-31-2020 to be checked out by a Dr.
IE was changing an underflow pump when IE stepped in a hole. The IE then fell and twisted knee. IE finished shift then decided to seek medical attention.

A contract employee was helping set-up a winder. A second contract employee was in an elevated position when a metal plate slipped from EE's hand. The plate struck the injured employee on the shoulder. The following were not a factor: rules, regs., systems, equipment, PPE. Attitude may have been a factor. Did not lose time until September 4, 2019.
IE was turning a splash guard on the 6th floor of the Concord prep Plant. As EE was pushing the splash guard EE felt a pain in EE's left shoulder. IE missed no work until 8/16/19. After IE visited the Dr on 8/16/19 for EE's shoulder pain, the Dr took EE off of work but found no damage in EE's shoulder. Possibly a strained muscle that needed to heal.
IE was trying to break some bolts loose with ratchet. As IE was straining IE felt pain in left shoulder. IE was taken to the Dr & was taken off of work for 1 week.
IE was removing a piece of metal out of a chute. As IE pushed the piece of metal off of the side of the belt, IE felt something pull in IE's left shoulder. Still waiting on the Dr's report to see what is wrong with IE's shoulder.

While replacing filter sector, the sector slide back from its resting spot. Employee attempted to grab it but while doing so felt a pull and pop in right shoulder.
EE was walk down stairs from 10th to 9th floor. When stepping onto the landing of 9th floor EE ""rolled"" ankle because of uneven grating.

Injured was washing under the coal storage silo when the hose separated. The injured ran to get away from hose but struck head on a piece of steel. Injured suffered a strain of muscle, fascia and tendon at neck level. Injured became reportable on 7/18/2016.
Injured was monitoring the thermal dryer when a hot spot developed. The injured called to the control room to turn the fan down. Injured thought the fan had turned off and opened the access door. When the door was opened, the injured suffered burns to the face, neck and wrist as the fan was still running at a lower speed.

Carrying five gallon buckets of coal samples down stairway to elevator.
Injured was changing screen panel in vibrating screen. The vibrating screen was de-energized and locked out. As injured exited the screen deck the injured pulled on the drive mechanism vee belt. This caused the sheave wheel to rotate pulling his right hand between the belt and sheave wheel resulting in a laceration to the palm and fracture of the 5th finger .

Employee was in squatting position and knee gave out when standing up.
Employee was walking across Sump Walkway and tripped cutting his knee.
While walking down stairs employee misjudged the distance of the last step and fell. Employee has an Inguinal Hernia. Medical evidence shows this may be a re-occurrence of a non work related previous injury.
Injured and coworker were replacing a pump. there was not enough clearance for the pump to slide over the pump well hand rail. The new pump became caught on the hand rail. Injured lifted up the pump. The pump slid across the hand rail catching injured' s left thumb between the pump base and hand rail resulting in a severe laceration. Hospital personnel amputated the thumb.

Employee was dismounting haul truck on the off operator side (passenger) when he lost his balance when he missed reaching for the handrail. No three point contact was used. The employee then fell to the ground (Approx. six feet) landing on his left foot causing a broken bone in his foot.
EE was shoveling concrete. He reached too far with a shovel & felt a tingly feeling down his left leg. His supervisor placed him on light duty for 3 days. On 7/14/11, EE was taken for medical treatment. He was released with restrictions for seven days. EE is now on light duty at the Concord mine site. He will return to full duty 7/21/11 per doctors orders.
Moving material on a 4 wheel cart. Material shifted when cart caster was rolled into opening in existing grating causing cart to shift and strike EE in the face. He received laceration above left eye and left nostril the injury required stitches. EE is release to working modified duty.
Wire way was lowered to the floor and the wire way was in one piece with a flanged connection. EE was taking the connection apart to get it off the floor. When he took the last bolt out the forward piece hit the floor. EE thought that it wasn't as heavy as it was and smashed his finger between the flange and the floor.
Removing a piece of conduit from the building, employee grabbed the conduit and the sharp edge cut the palm of his right hand.

injured and co-worker were using a pipe wrench to free a pump sheave wheel. the sheave wheel suddenly free up causing injured to pinch his right ring finger between the sheaved wheel and the pipe wrench.
INJURED EMPLOYEE WAS CHANGING A DRIVE BELT ON #8 WEMCO DRYER. INJURED WAS ATTEMPTING TO ROLL THE DRIVE BELT ONTO THE PULLEY. THE PULLEY WAS BEING TURNED BY HAND. THE PULLY TURNED, CATCHING INJURED LEFT LITTLE FINGER IN A PINCH POINT BETWEEN THE HOUSING AND THE PULLEY RESULTING IN AMPUTATION OF END OF LITLE FINGER.

A subcontractor's employee was improperly crossing bracing when he lost his grip and fell. He suffered a fracture in his lower left leg.

HE LEANED TOWARD THE DOOR AS HE PULLED & WHEN THE TWO PARTS OF THE DOOR CAME TOGETHER HE GOT HIS MIDDLE, RING & LITTLE FINGER ON HIS RIGHT HAND BETWEEN THE DOOR HALVES.

INJURED WAS ATTEMPTING TO PULL A 5 GALLON BUCKET OUT OF MUD. HE PULLED ON THE HANDLE, THE HANDLE BROKE AWAY FROM THE PLASTIC. THE INJURED FELL BACK AND THE HANDLE PULLED THROUGH HIS HAND CAUSI NG A 3/4"" CUT BETWEEN HIS THUMB & FOREFINGER. HE WAS NOT WEARING HIS GLOVES. INJURY REQUIRED THREE STITCHES.
INJURED WAS USING A UTILITY KNIFE TO CUT RUBBER GASKET MATERIAL ON A FLAT TABLE IN THE SHOP. HE HAD THE KNIFE IN HIS RIGHT HAND CUTTING TOWARDS HIMSELF. THE KNIFE SLIPPED & HIT HIS LEFT INDEX FINGER, CUTTING IT. HE WAS NOT WEARING GLOVES. CUT REQUIRED TWO STITCHES TO CLOSE.

HE THOUGHT THE FLOOR WAS WET BUT IT WAS COVERED WITH A LAYER OF ICE. HE TRIED TO CATCH HIMSELF WITHHIS ARM. HE FRACTURED HIS RIGHT DISTAL RADIUS.

DROPS OF TROWEL-EASE FELL ON WORK LIGHT & IGNITED. EE SUFFERED 2ND DEGREE BURNS ON RT. FOREARM, 2ND DEGREE ON LEFT HAND & NOSE, 1ST DEGREE & 2ND DEGREE ON RIGHT HAND.
INJURED WAS INSPECTING THE INTALLATION OF A GRIZZLY UNIT 141. HE WAS STANDING ON UNIT 150, 34 INCHES ABOVE THE FLOOR. HE REACHED IN UNIT 141 TO GET A PAPER TAG. AS HE REMOVED THE TAG HIS ARM F ROM THE CHUTE HE FELL OFF THE EASTSIDE OF UNIT 150. INVESTIGATION SHOWS HE MAY HAVE HAD HIS FACE IN THE ATMOSPHERE WHICH CAUSED HIM TO FALL.
BARS OUT OF POSITION ON THE THERMAL DRYER BED CAUSED A DEAD SPOT. (NO AIR COMING THROUGH.) COAL LAYED ON THE DEAD SPOT AND CAUGHT ON FIRE. WHEN FEED TO THE DRYER GOT LIGHT A SPARK WAS PULLED U P FROM THE BED & IGNITED THE DUST.
INJURED HAD JUST REPLACED A SHEAVE ON THE #1 REFUSE SCREEN DRIVE. IN AN ATTEMPT TO PUT THE BELT SON TEH HOOKED TO THE LIFTING EYE WITH CHAIN FALLS, AND ATTEMPTED TO PULL THE MOTOR OVER TO GET ENOUGH SLACK TO PUT THE DRIVE BELTS ON. THE LIFTING EYE BROKE WHILE HIS LEFT HAND WAS ON THE SHEAVE. SHEAVE FLEXED ABOUT 6"" CUTTING HIS HAND. NOT WEARING GLOVES.
