Over-exertion (Not Elsewhere Classified)

As employee was exiting the wash plant they jumped over a drainage ditch and felt a pop in their left knee. On February 16th employee was diagnosed with an ACL tear and is currently on restricted duty.

Struck by flying object

Employees were using a come-a-long to move a water tank over a few inches to sit in place properly. EE walked up on the job to give assistance. The chain that was around the structure's leg broke, striking the employee in the left eyebrow area. EE received 3 sutures and was released to full duty.

Struck by flying object

Employee was welding liners into a discharge chute when they felt something hit their face. Later in the day they realized a piece of debris was in their eye. They sought medical attention the next day and the debris was removed.

Employee was performing maintenance to the toggle system on the jaw crusher when a foreign body entered EE's eye causing irritation. EE sought medical attention on August 29th, prescribed eye drops and released to full duty.

Employee was stripping the outer jacket off of motor wire to install in a pump when the Hawkbill knife EE was using slipped out of the insulation and ran across the top of EE's leg resulting in a laceration. EE sought medical attention, received sutures and is currently on restricted duty.

Employee injured hand while dislodging oversized rocks from a chute. EE went to the ER, received 5 sutures and is currently working on restricted duty.

Employee was loading customer trucks with a John Deere 350 excavator. As they were exiting the machine they lost footing, injuring their right ankle.

Employee was repairing a tarp arm on a customer's truck when the rod bracket broke and struck EE between the rib cage and abdomen. EE sought medical attention and is currently on restricted duty.

Employee's finger was injured when it was caught between the tailgate and bed of a pickup truck. EE is currently working restricted duty.

Fall from ladders

Employee was entering an ingredient bin to perform maintenance when the ladder shifted causing employee to fall to the ground. Employee sought medical attention and was diagnosed with a fractured ankle. Employee is currently working on restricted duty.

Employee stepped on a rock and twisted ankle while walking to the crusher. Employee did not seek medical attention until December 10, 2020 and was released to full duty. Employee went back to the doctor on May 6, 2021 and was given restrictions, which the plant could not accommodate so this will be a reportable lost time injury as of May 6, 2021.

Employee was attempting to close the door on a rail car when the employee felt something pull in back. Employee sought medical attention and is currently on restricted duty.

Employee was inserting springs on a screen when the screen shifted and the springs compressed, resulting in a laceration and fracture to finger. EE sought medical attention, received six stitches and was released to full duty.

On July 16, 2020 employee reported that EE experienced a shoulder injury in June of 2017 and EE wanted to seek medical attention (employee did not report injury in 2017). On August 24, 2020 MRI results revealed a labrum tear consistent with an old injury. EE is scheduled to have surgery on September 11, 2020. EE is currently working full duty until surgery date.

While assisting in the removal of the bottom deck of a screen tower, a pry bar slipped and caused a laceration on the employee's thigh. EE received sutures and was released to full duty.

Contact with heat

Employee was performing maintenance on a crusher when he reported to his supervisor that he felt dizzy. He sought medical attention and was diagnosed with strep throat and heat exhaustion.

Struck by flying object

EE's were replacing broken links on the Dozer. EE was using a sledgehammer to drive the pin into the link. EE swung,immediately felt like he had been hit by a rock in the scrotum. EE observed blood,felt sore and went home. On 3/24 ee felt like something was in the scrotum area,co,sent ee for medical attention.EE had surgery on 3/31/15 to remove the metal splinter from his scrotum.

Fall from machine

Employee jumped from a Grove crane as it overturned. Employee was attempting to move water pipes and a pump. The injured employee sustained torn ligaments in his hip and 3 broken ribs. Investigation is on-going.

Employee was repairing a coolant leak on a RTV Kubota when he cut his hand on the 9"" plastic coolant fan. He sought medical attention on 7/16, required sutures and was placed on restricted duty.

Employee was dismounting a locomotive when he felt a sharp pain in his right hip and tingling in his right foot. He sought medical attention on May 6, 2013. He was given a Medrol dose pack prescription and released to full duty.

Employee slipped while exiting fuel truck causing a strain to his right knee. He sought medical attention on 9/14 and was placed on restricted duty. This incident is being reported within 10 days of seeking medical attention.

EE was sitting at her desk performing her duties as a scale clerk when she notified her supervisor that she wasn't feeling well and there was pain in her chest. He called 911 and an ambulance transported her to the hospital. She was diagnosed the next day with having a mild heart attack. This incident was immediately reported to MSHA.

Employee received a laceration, requiring staples and stitches, to the back of his right thigh when his right leg stepped through a grating. He was in the process of washing rock off of a walkway and grating at a rail clean out pit.

Fall to the walkway or working surface

An Ensley Central employee was fitting two sections of pipe together. The employee straddled one of the pipes to put weight on it to get the flanges to align. The pipe bucked and threw the employee off the pipe. When the employee hit the ground he used his left hand to break his fall. This caused him to hurt his L hand.

Ensley Central Service employee 01-03226 was drilling holes in steel floor at switch house at Calera Quarry 01-00050; drill caught and twisted left and right wrists. Employee was applying a large amount of force onto a drill without enough leverage to keep drill under control.

Employee, along with co-workers were changing screen wire on the bottom deck of a triple deck screen. At time of injury, employee was laying down on the floor underneath the screen's sheave guard tightening a nut and felt a pain in his back.

Employee was cleaning rail car. Grabbed door lock with hand to unlock when door dropped down catching his finger.

After cleaning out rail car; employee was using a metal bar to close the rail car door. He said something let loose in his lower back. The door was defective and hard to close.

Employee was closing a rail car door at the cleanout pit when the bar slipped and struck him across the face.

Over-exertion in lifting objects

It was a clear day, but cold. The employee was lifting a 2 3/8 screen wire to replace the existing screen when he twisted his back. When the employee realized that he may have hurt his back, he went to sit down and rest to eat lunch and sat on a small nail. Sitting on the nail made him jump and causing additional pain to the back. No lost time from work.

Employee was fueling loader, spout slipped out and fuel spilled on his left arm and upper body.

Employee was coupling railcars to locomotive. When he noticed at the last minute that the draw heads were not lining up, used his left foot to push the draw head on the railcar. His foot slipped and was caught in the couplings, crushing his toes. Three toes were surgically moved later that night. (Little toe and next 2 amputated - all proximal phalange.)

Three employees were building a new feed tube for the 302 crusher. As they were attempting to flip the tube over the injured employee's finger slipped off the tube and his right index finger was smashed by the 150 - 175 pound tube.

Employee was crossing rail cars and did not use ladders. He stepped on draw head and jumped off on the ground.

Employee sprain his right ankle as he dismounted loader and stepped on a 3"" rock. He was fueling and changing air filters on 992G loader. Employee chose to not see a doctor until 10/22/2004.

THREE EMPLOYEES WERE CHANGING LOWER CHECK PLATES ON JAW CRUSHER. ONE WAS USING A PRY BAR TO LINE UP HOLES FOR BOLTS. THE BAR SLIPPED & HIS LEFT KNEE POPPED. PRY BAR DID NOT HIT KNEE. EMPLOYEE TOOK IT EASY OVER THE WEEKEND, BUT ON MONDAY HIS KNEE STARTED SWELLING. HE WAS TAKEN TO THE DOCTOR. AN MRI CONFIRMED TORN MEDIAL MENISCUS AND OUTPATIENT SURGERY SCHEDULED.

EMPLOYEE WAS CLEANING UP AT THE PRIMARY HE HAD BEEN WASHING OUT. AS HE CAME DOWN TO JAW LEVEL OF OPERATOR'S HOUSE, HE SLIPPED AND FELL ON WET STEP.

EE WAS REMOVING TROUGH ROLLERS PREPARING FOR NATIONAL BELT TO CHANGE A CONVEYOR BELT. THE NATIONAL BELT EE'S WERE PLACING A LARGE TOOL BOX ONTHE CATWALK. WHEN ONE OF THE HOOKS WAS TAKEN LOO SE, THE TOOL BOX SLIPPED DOWN THE CATWALK AND STRUCK EE ON THE LEG AND WAIST.

EMPLOYEE WAS TAKING BELT SAMPLES FOR RAIL LOADOUT AND SPLITTING THEM DOWN AT QC LAB. A 5 GALLON BUCKET WITH ABOUT 35-40 LBS MATERIAL WAS LIFTED TO POUR IN SPLITTER WHEN EMPLOYEE FELT SOMETHING PULL IN HIS BACK LEFT SIDE.

EE WAS DOING ALIGNMENT ON MOTOR. HE WAS TURNING COUPLING AND WATCHING INDICATURE. EE DID NOT REALIZE HE HAD HIS FINGER IN COUPLING WHOLE. WHEN EE TURNED MOTOR HIS FINGER WAS MASHED BETWEEN THE COUPLING. CAUSED LACERATION TO FINGER THAT REQUIRE STITCHES.

EE WAS TAKING A SHORT CUT BACK TO HER WORK AREA.SHE STARTED DOWN A HILL THAT HAD LOOSE GRAVEL & ROCKS IN THE PATH. HALF WAY DOWN SHE STEPPED ON A ROCK WHICH CAUSED HER TO TWIST HER LEFT ANKLE, WHICH ALSO CAUSED IT TO FRACTURE.

EMPLOYEE REPORTED HAVING BACK PAIN TO HIS SUPERVISOR. AN INVESTIGATION BEGAN 4-30-01. EMPLOYEE STATES HIS HAUL TRUCK RIDES VERY ROUGH WHEN PASSING OVER RAILROAD TRACKS AND THE SHOCKS ARE WORN OUT. HE WAS SENT TO DOCTOR FOR AN EVALUATION BY COMPANY PHYSICIAN. HIS CONDITION WILL BE MONITORED AND TREATMENT GIVEN AS PRESCRIBED BY DOCTOR.
