Cutting trees with chainsaw. Blade got stuck. Turned off. When EE pulled the chainsaw out, it swiped EE's leg, resulting in a small cut, requiring three stitches and no loss time.

EE1 was getting in the man basket. EE2 was already in man basket and moved it pinning EE1 leg between basket and concrete wall

G&R employee was guiding sheet metal roofing into a dumpster. The sheet metal swung into the employee's gloved hand which was resting on a cross beam of the dumpster, cutting the pinky finger. Medical treatment was 3 stitches to the finger.

While honing employee's skills practicing welding in the maintenance shop the employee was exposed to the arc flash from the welding. Employee was wearing PPE and others had used the same PPE without injury.

EE was cleaning the preheater with a water blaster along with a co-worker. EE was using the shorter 6' lance. EE lost control of the lance and the tip struck EE on the left thigh. The co-worker let off the pressure release pedal shutting it down. EE was wearing supplied PPE. The high-pressure water resulting in a puncture and laceration wound on EE's left thigh.

Miner stepped into a pile of hot material allowing hot material to enter their boot causing burns around the upper ankle.

While removing a large approximately 50 LB chunk of material from the D3 fly ash conveyor belt's east side the chunk fell in a way that pinned the miners right ring finger to the belt frame crushing the fingertip.

The Cat 349 Excavator was on a bench pile of rock in southwest corner when the rock below the tracks gave way causing the 349 to slide down about 15ft and lay over on its side. Operator received 10 staples in the right side of the head, 6 cracked ribs and bruising to the face. Employee returned to work without lost time and is currently on restricted duty.

Employee was trying to clear a pipe when they got cement dust in their eye.

Employee slipped and fell, cutting head.

Employee was cutting off a bolt on a hatch on the inlet chute of the raw mill. There were 2 bolts in the door holding it in place. The one bolt that was holding it from rotating did not have a nut on it. As the employee began the work, the bolt without the nut somehow moved allowing the door to swing down and impact employee's leg, cutting it.

Employee was walking toward clinker breaker platform to retrieve tape for the vac hose. There was a hoe propped up against the platform. The employee stepped on the head of the hoe causing the handle to strike EE in the left side of EE's mouth.

Employee was going to bring the locomotive back into the plant. EE threw the wrong switch and was instead on the track that has a bump and stop. EE did not realize the mistake until EE looked up and saw the stop. EE decided to jump off instead of running into the stop EE was afraid that the locomotive was going to derail and crush EE.

Employee was in the process of building a Cardox tube for a preheater plug. Employee was filling the tube with CO2 and noticed it leaking. Employee put the tube in the vice to change the head. Employee did not bleed the pressure from the tube as trained to do. When the head came off, the CO2 violently released.

EE was helping remove a Cement Ring out of a finish mill. EE was laying on EE's left side with EE's knee in cement dust. EE received minor chemical burns to the left side of EE's neck, creases of both elbows & EE's left knee. After finishing EE's work, EE washed off and continued with EE's day.

Two employees were moving a shaft. One end slipped causing one employee to lose their grip. The shaft fell onto the other employees right hand, resulting in a broken ring finger and the nail on the middle finger needing to be sewn on.

Employee entered the 2nd floor Preheater MCC to work on the feed shelf analyzer. When employee went in, the room was hot because the air conditioner wasn't operating. Employee went to the bucket to put power back on the AC breaker and it exploded.

Checking haul truck. After cranking truck, employee got out of the cab to go down the steps to the ground. When employee stepped on the first step a pain went through employee's hip to employee's back.

Loading a shot, stepped into a hole, fell to the ground injuring left knee.

Employee was cutting a piece of vacuum hose while holding it between legs. The box cutter EE was using cut the hose then the right leg.

injured was lancing on preheater vessel when a large chunk fell on lance causing employee to unexpectedly twist his arm.

Employee was positioning a piece of drag conveyor when it fell amputating the end of his left pinky finger just below the first joint.

Employee was lifting a 95# cement bag off a conveyor and felt a pain in his abdomen.

Employee was installing a liner in secondary crusher. The liner slipped and pinched his hand causing a laceration in his palm

Employee was cutting a piece of rubber flashing with a utility knife when the blade broke and cut the back of his hand that was holding the belt.

Employee was using his foot to help push a pallet jack. As the jack moved his knee popped.

Employee had a 1 ton ratchet fall from his hand to his little toe. Ratchet hit little toe directly behind the steel toe in his safety work boot.

Employee was climbing stairs when his right foot slipped no fall occurred just felt a pain in his heal.

Employee was cleaning pre heater tower when he bent down his knee twisted

Employee was descending a vertical ladder when his foot slipped and he fell backwards hitting ground (fall from 3rd step from bottom leg stuck in ladder and he fell uncontrolled to the ground.

Injured employee was getting off maintenance truck and his foot got caught on a burning torch hose, causing him to fall on his back.

Employee was carrying a tugger and he hit his head on a plastic vacuum hose causing him to strain his lower back.

Employee lost his balance, tripped and hit his head under the back of his hard hat on a beam.

Employee was walking down stairs when he felt a pop or pull in his groin area.

Employee was working and felt dizzy and light headed. Was sent to Hospital for symptoms of heat exhaustion.

EE was attempting to hook up a come-a-long to a steel pipe truss section when he got his right middle finger caught between the latch on the hook and the steel pipe truss section. This resulted in a laceration to the tip of the finger that took seven stitches.

Employee was working and felt dizzy and light headed. Was sent to Hospital for symptoms of heat exhaustion.

Employee placed a 5ft rolling ladder at a 45 degree angle to work area. He then proceeded to lance the preheater from the top platform level, which caused the ladder to tip over, causing him to fall to the floor.

Employee was cleaning out a concrete hose when he strained the lower back.

EMPLOYEE WAS CHANGING BLADES IN THE SEPARATOR. HE WAS USING A RATCHET WRENCH TO LOOSEN BOLTS AND STRAINED HIS BACK PULLING UP ON WRENCH.

Employee was walking No 1 belt conveyor when he slipped on some loose rock injuring his right knee. Employee was diagnosed with a meniscal tear on 9/25/2009.

EMPLOYEE WAS PUSHING LOADED WHEEL BARROW IN MILL ROOM, WHEN HE STRAINED HIS CHEST.

Employee was hanging tires on conveyor system. He picked up an oversized tire weighing 70 lbs and strained his left shoulder.

Injured was lancing preheater when a chunk fell striking wand on end. The injured was holding wand which jerked causing him to strain his right arm. Employee was sent to Physician on 7/30/09 and diagnosed with right shoulder strain.

Employee was moving protective cage in kiln when he strained his upper back.

Employee was helping install kiln feed sampler. He was holding sampler in place, when he felt a pull in his lower back.

Employee was standing behind other employee who was using a pnuematic hammer. Employee bent over and a piece of material came between the top of his safety glasses and struck him in the left eye.

employee was shotting kiln gun and caught is elbow between door frame of cooler and hand handle injuing his right wrist (He was released to full duty on 5/12/09 but continued to have problems with the wrist and was place back on restricted duty on 6/29/09 by 220091870039)

No 1 fines belt hung. Employee was standing on ground level pulling down on belt to free belt. The belt broke free and the employee lost his balance and fell on the ground injuring his lower back.

EMPLOYEE WAS CLEANING SPILL WITH VAC TRUCK. SIX INCH HOSE FILLED WITH MATERIAL. EMPLOYEE TRIED TO LIFT HOSE CHEST HIGH AND DUMP MATERIAL OUT END. WHEN HE DID HE STRAINED HIS LOWER BACK.

EMPLOYEE WAS CLIMBING A STRAIGHT LADDER WHEN IT SLIPPED FROM UNDER HIM. HE FELL ON THE LADDER LACERATING HIS LEFT ELBOW.

Employee got dust in both eyes when working in the coal mill. He irrigated both eyes and thought they were ok. On 2/17/2008 he complained of both eyes still being irritated. He was sent to the Dr. and diagnosed with an infection.

Employee was passing 35 lb cooler grate to the floor level of cooler. He turned to set the grate in the cooler and struck his left index finger on the platform handrail.

Employee was moving metal locker up stairs when his co-worker miscommunicated and set the locker down on his right middle finger.

Employee missed rung climbing vertical ladder and strained lower left abdomen.

Employee tried to remove 1 1/2 inch nut from bolt using 3/4 inch socket wrench. Nut was placed on with impact wrench. He strained his lower back pulling up on wrench.

EMPLOYEE WAS LIFTING A TIRE TO HANG ON TIRE CONVEYOR WHEN HE FELT A PAIN IN HIS RIGHT SHOULDER.

Employee was cleaning up spilled material in long drag area. clinker dust leaked from platco valve and blew in his right eye.

Employee was placing 3.5 x 5 five foot angle iron on back of flat bed truck and caught his left thumb between the angle iron and the tie down rail of the truck.

Employee was moving two 95 lb steel rails with skytrack lift. The rail was not centered on tines which caused it to be unbalanced. He exited the lift and attempted to push the 1400 lb rail on the tine. He then felt a pain in his left shoulder.

EMPLOYEE WAS REPAIRING BENT SUPPORT AND STRAINED RIGHT SHOULDER WHILE USING HAMMER OVERHEAD.

Employee was lifting a 85-90 pound edge form with the assistance of another employee. He felt a slight pull in his groin area. Employee reported it but did not feel that he needed medical attention at the time. On September 23, the employee stated that he could still feel soreness in the groin area so the job superintendent directed the employee to see our medical provider.

Employee was lifting a six inch diameter pump hose when he felt something pull in his left shoulder and bicep. Resulting injury is a torn bicep muscle.

EMPLOYEE WAS USING 5 GALLON BUCKET TO CLEAN UP MATERIAL. SHE PULL LEFT SHOULDER LIFTING BUCKET

EMPLOYEE WAS REMOVING GUARD OF LARGE SPROCKET, SLIPPPED, STRAINED LOWER BACK - DIAGNOSIS ON 4/28/2008 LOWER BACK STAIN.

EMPLOYEE STRAINED LOWER BACK WHEN HE BENT OVER AT THE WAIST TO INSTALL SEPERATOR BLADE ON 5 MILL

EMPLOYEE SLIPPED ON WET MUDDY SURFACE INJURING RT LOWER LEG

EMPLOYEE WAS REPAIRING AN IDLER ROLL IN THE SHOP. HE BENT OVER AT THE WAIST AND PICKED UP ONE END OF ROLL WEIGHING 110 LBS. HE STRAINED HIS LOWER BACK.

EMPLOYEE WAS CHANGING LINER PLATE IN FINISH MILL AND CAUGHT RT 5TH IN PINCH POINT LACERATING FINGER REQUIRING 5 STITCHES.

EMPLOYEE WAS LANCING MATERIAL FROM BIN WHEN IT SLIPPED AND HE CUT HIS LEFT THUMB OM PIPE CLAMP.

THERE WAS A FIRE IN THE ELECTRICAL CABLE TRAY LOCATED ON THE BURNER FLOOR. AN ISOLATION VALVE LOCATED ABOVE THE CABLE TRAY LEAKED ALLOWING COAL DUST TO ACCUMULATE ON THE CABLE TRAY. IT IS BELIEVED THE COAL WAS IGNITED BY HIGH TEMPERATURE ON THE COOLER BAG HOUSE DUCT DURING A PUSH. THE FIRE WAS EXTINGISHED. LOCAL FIRE DEPARTMENT WAS NOT CALLED TO THE SCENE.

EMPLOYEE WAS OPENING HINGED DOOR ON RAW MILL BODY AND PINCHED FINGER BETWEEN DOOR AND BAR.

EMPLOYEE WAS LANCING PREHEATER WHEN A CHUNK FELL FROM ABOVE STRIKING THE LANCING WAND CAUSING IT TO STRIKE THE EMPLOYEE ON THE RIGHT SIDE OF THE HEAD.

EMPLOYEE WAS CLEANING D10 SUMP WITH A VAC TRUCK WHEN HE BENT OVER AND REINJURED HIS LOWER BACK. EMPLOYEE HAD RETURNED TO WORK FROM BACK SURGERY ON 7/16/2007, DOCUMENT #403092.

EMPLOYEE WAS CLEANING D-10 TUNNEL WHEN HE LIFTED HEAVY HOSE AND STRAINED HIS LOWER BACK.

EMPLOYEE HAD RETURNED FROM BACK SURGERY ON 7/30/2007. ORIGINAL INJURY WAS ON 9/6/2006 #382016. EMPLOYEE COMPLAINED OF LOW BACK PAIN CONTACTED PHYSICIAN AND WAS PLACED OFF WORK ON 8/9/2007. NO NEW INCIDENT OCCURRED.

EMPLOYEE WAS SHOVELING LOOSE MATERIAL IN DUMP HOPPER, WHEN HE FELT A PAIN IN HIS LOWER BACK. EMPLOYEE RE-AGGRAVATED BACK ON 8/17/2007 WHEN HE BENT OVER TO OPEN A RAIL CAR DOOR. HE WAS SENT TO THE DR. AND DIAGNOSED WITH A LOWER BACK STRAIN ON 8/17/2007.

EMPLOYEE WAS WELDING ON ABOVE STRUCTURE WHEN HOT SLAG ENTERED RT EAR CANAL. EMPLOYEE WAS SEEN FOR MEDICAL TREATMENT AND RELASED FULL DUTY ON 7/26/2007. EMPLOYEE HAD OUT PATIENT SURGERY TO REMOVE A PIECE OF SLAG FROM HIS EAR AND REPAIR EAR DRUM ON 10/30/2007.

EMPLOYEE STEPPED OVER AIRSLIDE AND FELL INJURING RIGHT SHOULDER

EMPLOYEE WAS MOVING 55 GALLON DRUM OF MATERIAL TO BUCKET OF BOBCAT LOADER WHEN HE CAUGHT HIS LEFT 2ND AND 3RD DIGIT FINGER BETWEEN BUCKET AND LIP OF BARREL.

EMPLOYEE REPORTED: HE WAS HAVING PAIN IN SHOULDER DUE TO INSTALLING CRANE CABLES. EMPLOYEE WAS SENT FOR MYELOGRAM AND DIAGNOISED WITH DEGENERATIVE CHANGES AND BULGE ON 6/22/07.

EMPLOYEE COMPLAINED LEFT WRIST PAIN DUE TO USING JACKHAMMER AND OPERATING CRANE JOYSTICK.

EMPLOYEE WAS HANDING BOX DOWN FROM PLATFORM TO INJURED EMPLOYEE ON GROUND LEVEL WHEN INJURED EMPLOYEE STRAINED LEFT THUMB.

EMPLOYEE WAS WORKING ON TROLLEY HOIST. HOIST WAS STUCK ON RAIL. EMPLOYEE WAS GOING TO ATTACH HOOK TO HAND RAIL AND WENCH TROLLEY FREE. EMPLOYEE GOT HIS RIGHT LITTLE FINGER CAUGHT BETWEEN HANDRAIL AND HOOK

EMPLOYEE HAD STEPPED OFF VERTICAL LADDER AND TURN HIS RT ANKLE

EMPLOYEE WAS TORQUEING A BOLT ON THE CLOSING HOIST WITH A HEX HEAD SOCKET WHEN THE TIP BROKE CAUSING HIM TO STRIKE HIS RT THUMB.

EMPLOYEE WAS FABING DOOR IN SHOP WHEN LIFTED THE DOOR AND STRAINED HIS LOWER BACK

EMPLOYEE WAS MOVING BAGS JAMMED ON BAG CONVEYOR. LATER THAT DAY HE FELT PAIN IN HIS RIGHT SHOULDER. SENT TO DOCTOR ON 1/26/2007 DIAGONOSIS WITH RT SHOULDER STRAIN

EMPLOYEE WAS REMOVING EXPAND METAL GUARD ON F18 FAN WHEN GUARD HUNG ON SHAFT GUARD. WHEN GUARD CAME LOOSE IT PINCHED LT THUMB BETWEEN TWO GUARDS HALFS.

EMPLOYEE WAS USING PNUEMATIC HAMMER FOR CLEAN UP WHEN HE DROPPED HAMMER ON RIGHT FOOT INJURING SMALL TOE.

Employee was inspecting dust collector screw. He failed to check to see if hopper was empty. When he opened inspection cement flushed out entering both eyes.

Employee was installing an electric motor and strained his right lower back. He was diagnosed on 11/17/2006.

EMPLOYEE WAS CLIMBING VERTICAL LADDER WHEN HIS FOOT SLIPPED OFF A RUNG CAUSING HIM TO PULL HIS RIGHT FOREARM.

EMPLOYEES WAS WORKING IN MILL WITH VAC TRUCK WHEN HE CUT HIS RT SIDE ON A SHARP OBJECT

EMPLOYEE WAS MAKING A GUARD OF EXPANDED METAL. HE WAS USING A BAR TO BEND THE METAL WHEN HE CAUGHT HIS RT 5TH FINGER BETWEEN THE GUARD AND BAR.

EMPLOYEE WAS COLLECTING OIL SAMPLE FROM SULLAIR COMPRESSOR. HE OPENED THE VALVE TO COLLECT WHICH HAD A HOSE CONNECTED TO IT. HE DID NOT HOLD THE HOSE AND THE PRESSURE WHIPPED THE HOSE AROUND AND SPRAY OIL IN HIS LEFT EYE.

Employee was removing clinker screw housng when he slipped in spilled clinker from the screw injuring his lower back.

Employee and co worker were lifting an electric motor when it slipped causing the employee to strain his wrist, shoulder and back.

EMPLOYEE WAS UNCLOGGED PLUG AIR WHEN THE PLUG BROKE LOOSE AND RUST MATERIAL STUCK LEFT HAND

EMPLOYEES HIT HIS LEG ON THE SISSOR LIFT WHILE WALKING ON THE PREHEATER

Employee was trying to remove a lump of clinker from a belt feeder using a bar when he strained his lower back. (Aggravated previous injury.)

EMPLOYEE WENT TO TOOL ROOM TO GET A PNUEMATIC WRENCH. THE WRENCH WEIGHED 65LBS. HE BENT OVER AND PICKED IT UP STRAINING HIS LOWER BACK.

EMPLOYEE WAS INSTALLING A 10' SECTION OF 12 INCH SCREW CONVEYOR HOUSING WHEN IT SLIPPED, PINCHING HIS RT. INDEX FINGER BETWEEN THE HOUSING AND A 2"" FLANGE.

Employee was using a pnuematic hammer and strained left wrist

EE WAS WALKING AND STEPPED ON PLYWOOD COVERING OPENING TO BELT CONVEYOR. PLYWOOD BROKE, CAUSING EE TO FALL INTO HOLE. HE CAUGHT HIMSELF WITH HIS ELBOWS WHEN HE WAS WAIST DEEP, STRAINING HIS LEFT SHOULDER.

Employee got right thumb caught between jook and 55 gallon drum. Lifting 55 gallon drum of balls with forklift.

EMPLOYEE WAS REMOVING DUST COLLECTER BAGS WHEN ONE FELL STRIKING HIM ON THE LEFT KNEE

Employee strained right shoulder pulling pneumatic hammer up to second level

Employee reported his back was hurting on 10/26/05 from shoveling material at raw mill. He was sent for an examination on 11/2/05 and was diagnoised with a lower back strain.

Employee and coworker were carring a 102 lb. electric motor up stairs. Employee strained lower back.

Injuried was trying to up choke feed chute to #6 cement mill using a 3 lb hammer. He missed the chute with a swing and strained his right forearm

Employees has been diagnoised with an allergic reaction to cement dust.

Employee was on top of dust collect and turned and tripped over door latch striking left knee

Employee was water blasting preheater. He turned and struck his left tiba on the pressure relief valve of an air cannon. Incident reported to company on 8/8/2005; diagnoises recieved on 8/10/05.

Dust feel from overhead beam. Employee jumped out of the way. He twisted his right ankle and right arm when he turned. Employee was sent for medical treatment on 7/1/05 and diagnosis with a strain and placed on restricted duty.

Employee's audometric test results indicated a STS

Employee working on dust collector when dust fell off a beam and hit on the left side of face and dust went in eye

Employee was crossing over the cross screw when he tripped on the steps. As he was falling, he caught himself with his right hand injuring his right ring finger.

Employee was using a hacksaw to cut 2"" connector and 2 1/2"" bushing when the hacksaw slipped causing employee to cut his left hand.

Standing on movable frame - pulling on chainfall removing beam and beam popped up knocking EE off frame onto heels & slipped onto buttocks then back.

Employee was pulling a motor to be serviced in the coal mill area when he hurt his left wrist while trying to break a base bolt loose. Employee did not seek medical attention until 3/15/05.

Employee was water blasting when platform he was standing on gave way causing employee to fall 8 feet to next level.

There was a PLC system failure which caused the primary air fan to stop. This caused ground coal to settle which was shortly thereafter ignited causing an explosion.

Employee was walking across a catwalk, lost his footing, fell and cut his right knee.

Employee was operating a milling machine cutting a key-way in a shaft. He was racking the vise that holds the shaft to the west with his right hand on the rack lever and the handle on the vise pinched his hand between rack lever and the vise handle.

Employee was putting a bearing on the drive end of the clinker breaker and strained his left heel in the process.

The coal mill system shut down & on restart we experienced a coal mill explosion. The explosion was attributed to smoldering coal under the bowl of the coal mill.

EMPLOYEE WENT TO USE BLOW OFF STATION. WHEN HE TURNED ON THE TOGGLE SWITCH IT SHORTED OUT CAUSING THE EE TO RECEIVE AN ELECTRICAL SHOCK & BURNS TO HIS RT. THUMB.

Glowing ember in rehect compartment was caused by broken scraper. The result was an explosion that blew out 4 explosion doors in the circuit. The scraper was replaced and doors were replaced.

Employee walking through plant when dust got into eye.

Employee was changing a seal on the no.4 raw mill cylinder while another employee let the pressure off of the system. He tried to remove the seal when some fluid squirted him in the face.

Ee was attempting to move a metal floor plate when the metal floor plate slipped causing a contusion to his left middle finger.

EMPLOYEE WAS ABOUT TO WELD A PATCH ON THE CHUTE COMING DOWN TO THE PAN CONVEYOR WHEN HE SLIPPED ON CLINKER & CAUGHT HIMSELF WHILE HITTING HIS RIGHT SIDE ON THE HANDRAIL.

EE WAS PUTTING WEIGH IDLER BACK ON SCALE AFTER CHANGING LOAD CELL. IDLER SLIPPED AND CUT HIS FINGER.

EE did not have an additional accident but aggrevation to back due to previous injury dated 6-16-02.

Employee was working on air line to unclog it. He pinched off one end to disconnect other end and pressure built up in line causing cement to blow into his eyes.

EMPLOYEE WAS COMING DOWN STAIRS AT JAW CRUSHER & STEPPED DOWN ON A PIECE OF ANGLE & TWISTED HIS RIGHT KNEE.

EE WAS COMING DOWN STAIRS WHEN HE SLIPPED ON LOOSE CLINKER & TWISTED HIS KNEE.

EMPLOYEE WAS WORKING ON A PNEUMATIC OIL DRUM PUMP. HE HAD PUMP OUT OF THE DRUM AND WAS TRYING TO GET IT TO WORK. HE WAS HOLDING THE PUMP WITH HIS LEFT HAND AND TAPPED ON THE PUMP WITH HIS RIGH T HAND (TOOL USED). THE AIR WAS ON AND IT STARTED THE PUMP. HE CUT THE HAND THAT WAS HOLDING THE PUM (LEFT HAND).

REBOUND FROM SHOOTING GUNITE MATERIAL , GOT INTO EYE.

EE WAS CLIMBING UP THE LADDER TO DO AN INSPECTION OF THE CHUTE UNDER CLINKER BREAKER. WHEN HE GOT TO THE TOP RUNG HE FELL FORWARD SHIFTING ALL OF HIS RIGHT LEG & PUTTING EXCESS PRESSURE ON THE RIGHT ANKLE.

EMPLOYEE WAS HELPING PRY ROLLER OUT OF DRAG HOUSING WITH A BAR. BAR SLIPPED AND HIT HIS CHIN. HE WAS DIAGNOSED BY A DENTIST AND SOUGHT MEDICAL TREATMENT ON 3/10/04.

EMPLOYEE WAS WALKING ON BUILD UP ON PLATFORM AROUND THE TOP OF THE ELEVATOR WHEN HE SLIPPED & TWISTED HIS RIGHT KNEE. ORIGINAL ACCIDENT 01/28/04, AGGREVATED INJURY 04/08/04, SURGERY 05/19/04.

EMPLOYEE WAS USING A CLAY DIGGER TO PUSH OUT BOLTS IN THE #7 MILL WHEN THE LINER FELL STRIKING HIS LEFT FOOT.

EE WAS INSTALLING NEW LINERS INT HE FEED END OF THE NO. 6 FINISH MILL WHEN LINER SLIPPED WHILE PUTTING IT IN PLACE AND PINCHED HIS THUMB.

COAL DUST ACCUMULATED ON NEWLY INSTALLED HOT AIR DUCT. THIS COAL WOULD EVENTUALLY IGNITE & DROP INTO MILL INLET RESULTING IN AN EXPLOSION. NO MAJOR EQUIPMENT DAMAGED, MILL RUPTURE DISK RELEA VED PRESSURE AS DESIGNED.

HORIZONTAL DUCT FROM NEW HEATER HAD ACCUMULATED COAL DUST WHICH CAUGHT ON FIRE. FIRE INSIDE DUCT WAS IGNITION POINT FOR COAL DUST TO EXPLODE AND REPTURING THE 2 RELIEF DISKS.

EMPLOYEE WAS INSTALLING BELTS ON WATER PUMP. HE ROLLED BELT ON PULLEY & GOT FINGER BETWEEN BELT & PULLEY.

EE WAS INSTALLING A NEW DRIVE BELT ON BAG PALLETIZER. HE HAD TO PRY ONE OF THE ROLLS OVER TO STRETCH BELT OVER PULLEY. LATER IN THE DAY HE FELT SOME DISCOMFORT GOING DOWN HIS LEFT SIDE OF HIS NECK AND DOWN THE ARM AND IT GOT WORSE DURING THE NIGHT.

EMPLOYEE WAS CLEANING OUT MATERIAL FROM A CLOGGED FEED CHUTE. AFTER CONTINUOUS PUNCHING WITH AIR LANCE IN AWKWARD POSITION, SHOULDER STARTED HURTING.

EE WAS LIFTING A BAG OF OIL DRY WHEN HE FELT A PULL IN HIS CHEST.

EE WAS LIFTING MATERIAL WITH A COME A LONG AT LURGI SCREW. THE COME A LONG SLIPPED A GEAR WHILE HE WAS CRANKING THE HANDLE. THIS IS WHEN HE FELT SMOETHING PULL IN HIS RIGHT SHOULDER.

EE WAS REMOVING DUST COLLECTOR BAGS FROM COLLECTOR WHEN HE FELT A PAIN IN THE LEFT SIDE OF HIS NECK. SOME OF THE BAGS WERE HALF FULL OF DUST.

EE WAS CLEANING OFF WELL OF PREHEATER WITH A BAR WHEN A CHUNK FELL HITTING THE END OF THE BAR CAUSING THE BAR TO STRIKE EE'S RT. SHOULDER.

EMPLOYEE WAS CLEANING SPILLED ROCK OUT OF #2 BELT WALKWAY. HE LIFTED A LARGE ROCK AND FELT A PAIN IN HIS LEFT ABDOMEN.

EMPLOYEE WAS INSTALLING PUMP AND HEAT EXCHANGE TO COOL OIL IN BEARING AT #2 KILN PIER. WHEN PUMP STARTED RUNNING EMPLOYEE WAS CHECKING RETURN LINE FOR FLOW WHEN A FLAME CAME OUT OF THE PORT IN THE TOP OF THE BEARING, BURNING HIS RIGHT FOREARM.

EMPLOYEE WAS DISMOUNTING A WA-500 FRONT END LOADER. WHEN STEPPING FROM THE LADDER TO THE GROUND HE STEPPED ON A ROCK CAUSING HIM TO LUNGE TO THE RIGHT IN AN AWKWARD POSITION.

EMPLOYEE WAS JACKHAMMERING #6 MILL FEED ELEVATOR CHUTE. HE WAS BENT OVER IN CHUTE BREAKING OUT MATERIAL. HE PULLED JACKHAMMER UP TO START ANOTHER CUT & FELT SOMETHING PULL IN HIS BACK.

INJURED WAS REMOVING MATERIAL FROM BEHIND LOSHE MILL ROLL WITH 4"" HOSE OFF OF THE GUZZLER TRUCK. THE FORCE FROM THE HOSE PULLING THE MATERIAL CAUSED A JERKING MOTION CAUSING A STRAIN TO THE LO WER BACK.

EE WAS CLEANING OFF RETURN ROLL ON D-10 BELT WITH A METAL BAR. THE BAR WENT THROUGH THE MUD, CAUSING HIM TO STRIKE HIS HAND AGAINST THE METAL FRAME OF THE BELT.

EE WAS PULLING ELECTRICAL CABLE FOR MOTORE AT #1 BELT. HE STEPPED DOWN ONE STEP TO MAKE ANOTHER PULL AND STEPPED ON A PIECE OF ROLLED STEEL, CAUSING HIM TO TURN HIS ANKLE.

EE WAS OPENING A PLASTIC CONTAINER OF WEAR COMPOUND WITH A BOX CUTTER WHEN THE CONTAINER SLIPPED CUTTING HIS LT. HAND ACROSS THE PALM..

EMPLOYEE WAS PRYING CHAIN ONTO SPROCKET OF #6 MILL BUCKET ELEVATOR.

INJURED WAS PUTTING CABLE TRAY ONTO SCISSOR LIFT. CABLE TRAY BEGAN TO FALL AND EE ATTEMPTED TO CATCH IT STRAINING HIS LOWER BACK.

INJURED WAS WALKING UP STEPS GOING TO TOP OF SILOS. AFTER THE FIRST LEVEL OF STEPS HE TURNED ON THE PLATFORM TO THE NEXT LEVEL OF STEPS. THIS IS WHEN HE TWISTED HIS KNEE.

EE WAS PLACING A O.5 LB LEDGER BOOK IN OVERHEAD BIN WHEN HER LEFT SHOULDER GAVE WAY.

DIGGING OUT TOE WITH WA700 FRONT END LOADER WHEN BUCKET JUMPED THE TOE AND HIT THE QUARRY FACE OF THE 370' WALL CAUSING HIM TO SNAP HIS NECK IN A WHIPLASH TYPE EFFECT.

AN UNPLANNED EXPLOSION OCCURRED IN THE COAL MILLDUCT WORK. TWO RELIEF VALVES WERE BLOWN, NO OTHER DAMAGE RESULTED. NO INJURIES OCCURRED.

THE PLANT EXPERIENCED AN UNPLANNED EXPLOSION OF COAL DUST INSIDE THE COAL MILL SYSTEM. DAMAGE WAS SUBSTAINED TO THE COAL CYCLONE, COAL MILL AND DUCT. NO INJURIES OR BUILDING DAMAGE. THE IGNITI ON SOURCE IS UNDER INVESTIGATION.

EE WAS BURNING BOLTS OUT OF GUARD. HOT BOLT BURNED HIS LEG. FIRST AID WAS GIVEN. LEG BECAME INFECTED & HE WAS SENT TO SPECIALIST 2/4/01, DIAGNOSED AS HAVING A REACTION TO THE MEDICATION HE HAD RECEIVED ON 1/26/01.

EMPLOYEE'S NECK AND SHOULDER WAS HURTING FROM PICKING UP AUTO TIRES WITH RT ARM AND PLACING THEMON THE TIRE CONVEYER.

THE PLANT EXPERIENCED AN UNPLANNED EXPLOSION WHEN THE COAL MILL WAS IN THE PROCESS OF SHUTTING DOWN DAMAGE WAS SUSTAINED IN THE COAL MILL SYSTEMFAN, PRIMARY AIR FAN, COAL MILL CYCLONE & DUCT. THE IGNITION SOURCE IS UNDER INVESTIGATION NO INJUIRIES WERE RECORDED

INJURED DIAGNOSED WITH CARPAL TUNNEL SYNDROME TO LEFT WRIST.

INJURED WAS HELPING REMOVE A MOTOR FROM FAN ON PREHEATER FLOOR #3, WHEN HELPING HE TURNED AND FELT HIS BACK TWIST AND HURT.

WHILE WALKING IN THE QUARRY THE EE TRIPPED ON ROCK AND TWISTED ANKLE.

WHILE TAPING THE GUZZLE HOSE, THE AIR CANNON DISCHARGED BLOWING DRY CEMENT UNDER HIS SAFETY GLASSES INTO THE LEFT EYE.

INJURED WAS FILLING NO. 1 SULLAIR COMPRESSOR WITH OIL. INJURED REMOVED TOP VALVE AND FILLED SPOUT WITH OIL, SPLASHING SOME OIL IN HIS EYE.
