Cutting trees with chainsaw. Blade got stuck. Turned off. When EE pulled the chainsaw out, it swiped EE's leg, resulting in a small cut, requiring three stitches and no loss time.

G&R employee was guiding sheet metal roofing into a dumpster. The sheet metal swung into the employee's gloved hand which was resting on a cross beam of the dumpster, cutting the pinky finger. Medical treatment was 3 stitches to the finger.

Flash burns (welding)

While honing employee's skills practicing welding in the maintenance shop the employee was exposed to the arc flash from the welding. Employee was wearing PPE and others had used the same PPE without injury.

EE was cleaning the preheater with a water blaster along with a co-worker. EE was using the shorter 6' lance. EE lost control of the lance and the tip struck EE on the left thigh. The co-worker let off the pressure release pedal shutting it down. EE was wearing supplied PPE. The high-pressure water resulting in a puncture and laceration wound on EE's left thigh.

Miner stepped into a pile of hot material allowing hot material to enter their boot causing burns around the upper ankle.

While removing a large approximately 50 LB chunk of material from the D3 fly ash conveyor belt's east side the chunk fell in a way that pinned the miners right ring finger to the belt frame crushing the fingertip.

Struck against a moving object

The Cat 349 Excavator was on a bench pile of rock in southwest corner when the rock below the tracks gave way causing the 349 to slide down about 15ft and lay over on its side. Operator received 10 staples in the right side of the head, 6 cracked ribs and bruising to the face. Employee returned to work without lost time and is currently on restricted duty.

Employee was cutting off a bolt on a hatch on the inlet chute of the raw mill. There were 2 bolts in the door holding it in place. The one bolt that was holding it from rotating did not have a nut on it. As the employee began the work, the bolt without the nut somehow moved allowing the door to swing down and impact employee's leg, cutting it.

Employee was walking toward clinker breaker platform to retrieve tape for the vac hose. There was a hoe propped up against the platform. The employee stepped on the head of the hoe causing the handle to strike EE in the left side of EE's mouth.

Fall from machine

Employee was going to bring the locomotive back into the plant. EE threw the wrong switch and was instead on the track that has a bump and stop. EE did not realize the mistake until EE looked up and saw the stop. EE decided to jump off instead of running into the stop EE was afraid that the locomotive was going to derail and crush EE.

Employee was in the process of building a Cardox tube for a preheater plug. Employee was filling the tube with CO2 and noticed it leaking. Employee put the tube in the vice to change the head. Employee did not bleed the pressure from the tube as trained to do. When the head came off, the CO2 violently released.

EE was helping remove a Cement Ring out of a finish mill. EE was laying on EE's left side with EE's knee in cement dust. EE received minor chemical burns to the left side of EE's neck, creases of both elbows & EE's left knee. After finishing EE's work, EE washed off and continued with EE's day.

Struck by falling object

Two employees were moving a shaft. One end slipped causing one employee to lose their grip. The shaft fell onto the other employees right hand, resulting in a broken ring finger and the nail on the middle finger needing to be sewn on.

Flash burns (electric)

Employee entered the 2nd floor Preheater MCC to work on the feed shelf analyzer. When employee went in, the room was hot because the air conditioner wasn't operating. Employee went to the bucket to put power back on the AC breaker and it exploded.

Checking haul truck. After cranking truck, employee got out of the cab to go down the steps to the ground. When employee stepped on the first step a pain went through employee's hip to employee's back.

Employee placed a 5ft rolling ladder at a 45 degree angle to work area. He then proceeded to lance the preheater from the top platform level, which caused the ladder to tip over, causing him to fall to the floor.

No 1 fines belt hung. Employee was standing on ground level pulling down on belt to free belt. The belt broke free and the employee lost his balance and fell on the ground injuring his lower back.

EMPLOYEE WAS CLEANING SPILL WITH VAC TRUCK. SIX INCH HOSE FILLED WITH MATERIAL. EMPLOYEE TRIED TO LIFT HOSE CHEST HIGH AND DUMP MATERIAL OUT END. WHEN HE DID HE STRAINED HIS LOWER BACK.

Employee got dust in both eyes when working in the coal mill. He irrigated both eyes and thought they were ok. On 2/17/2008 he complained of both eyes still being irritated. He was sent to the Dr. and diagnosed with an infection.

Employee was moving two 95 lb steel rails with skytrack lift. The rail was not centered on tines which caused it to be unbalanced. He exited the lift and attempted to push the 1400 lb rail on the tine. He then felt a pain in his left shoulder.

Employee was lifting a 85-90 pound edge form with the assistance of another employee. He felt a slight pull in his groin area. Employee reported it but did not feel that he needed medical attention at the time. On September 23, the employee stated that he could still feel soreness in the groin area so the job superintendent directed the employee to see our medical provider.
