G&R employee was guiding sheet metal roofing into a dumpster. The sheet metal swung into the employee's gloved hand which was resting on a cross beam of the dumpster, cutting the pinky finger. Medical treatment was 3 stitches to the finger.

While honing employee's skills practicing welding in the maintenance shop the employee was exposed to the arc flash from the welding. Employee was wearing PPE and others had used the same PPE without injury.
EE was cleaning the preheater with a water blaster along with a co-worker. EE was using the shorter 6' lance. EE lost control of the lance and the tip struck EE on the left thigh. The co-worker let off the pressure release pedal shutting it down. EE was wearing supplied PPE. The high-pressure water resulting in a puncture and laceration wound on EE's left thigh.
Miner stepped into a pile of hot material allowing hot material to enter their boot causing burns around the upper ankle.

While removing a large approximately 50 LB chunk of material from the D3 fly ash conveyor belt's east side the chunk fell in a way that pinned the miners right ring finger to the belt frame crushing the fingertip.
The Cat 349 Excavator was on a bench pile of rock in southwest corner when the rock below the tracks gave way causing the 349 to slide down about 15ft and lay over on its side. Operator received 10 staples in the right side of the head, 6 cracked ribs and bruising to the face. Employee returned to work without lost time and is currently on restricted duty.

Employee was cutting off a bolt on a hatch on the inlet chute of the raw mill. There were 2 bolts in the door holding it in place. The one bolt that was holding it from rotating did not have a nut on it. As the employee began the work, the bolt without the nut somehow moved allowing the door to swing down and impact employee's leg, cutting it.
Employee was walking toward clinker breaker platform to retrieve tape for the vac hose. There was a hoe propped up against the platform. The employee stepped on the head of the hoe causing the handle to strike EE in the left side of EE's mouth.

Employee was going to bring the locomotive back into the plant. EE threw the wrong switch and was instead on the track that has a bump and stop. EE did not realize the mistake until EE looked up and saw the stop. EE decided to jump off instead of running into the stop EE was afraid that the locomotive was going to derail and crush EE.

Employee was in the process of building a Cardox tube for a preheater plug. Employee was filling the tube with CO2 and noticed it leaking. Employee put the tube in the vice to change the head. Employee did not bleed the pressure from the tube as trained to do. When the head came off, the CO2 violently released.
EE was helping remove a Cement Ring out of a finish mill. EE was laying on EE's left side with EE's knee in cement dust. EE received minor chemical burns to the left side of EE's neck, creases of both elbows & EE's left knee. After finishing EE's work, EE washed off and continued with EE's day.
Two employees were moving a shaft. One end slipped causing one employee to lose their grip. The shaft fell onto the other employees right hand, resulting in a broken ring finger and the nail on the middle finger needing to be sewn on.
Employee entered the 2nd floor Preheater MCC to work on the feed shelf analyzer. When employee went in, the room was hot because the air conditioner wasn't operating. Employee went to the bucket to put power back on the AC breaker and it exploded.
Checking haul truck. After cranking truck, employee got out of the cab to go down the steps to the ground. When employee stepped on the first step a pain went through employee's hip to employee's back.

No 1 fines belt hung. Employee was standing on ground level pulling down on belt to free belt. The belt broke free and the employee lost his balance and fell on the ground injuring his lower back.
EMPLOYEE WAS CLEANING SPILL WITH VAC TRUCK. SIX INCH HOSE FILLED WITH MATERIAL. EMPLOYEE TRIED TO LIFT HOSE CHEST HIGH AND DUMP MATERIAL OUT END. WHEN HE DID HE STRAINED HIS LOWER BACK.
EMPLOYEE WAS CLIMBING A STRAIGHT LADDER WHEN IT SLIPPED FROM UNDER HIM. HE FELL ON THE LADDER LACERATING HIS LEFT ELBOW.
Employee got dust in both eyes when working in the coal mill. He irrigated both eyes and thought they were ok. On 2/17/2008 he complained of both eyes still being irritated. He was sent to the Dr. and diagnosed with an infection.
Employee was passing 35 lb cooler grate to the floor level of cooler. He turned to set the grate in the cooler and struck his left index finger on the platform handrail.

Employee was placing 3.5 x 5 five foot angle iron on back of flat bed truck and caught his left thumb between the angle iron and the tie down rail of the truck.
Employee was moving two 95 lb steel rails with skytrack lift. The rail was not centered on tines which caused it to be unbalanced. He exited the lift and attempted to push the 1400 lb rail on the tine. He then felt a pain in his left shoulder.

Employee was lifting a 85-90 pound edge form with the assistance of another employee. He felt a slight pull in his groin area. Employee reported it but did not feel that he needed medical attention at the time. On September 23, the employee stated that he could still feel soreness in the groin area so the job superintendent directed the employee to see our medical provider.
Employee was lifting a six inch diameter pump hose when he felt something pull in his left shoulder and bicep. Resulting injury is a torn bicep muscle.

IT IS BELIEVED THE COAL WAS IGNITED BY HIGH TEMPERATURE ON THE COOLER BAG HOUSE DUCT DURING A PUSH. THE FIRE WAS EXTINGISHED. LOCAL FIRE DEPARTMENT WAS NOT CALLED TO THE SCENE.

Employee reported his back was hurting on 10/26/05 from shoveling material at raw mill. He was sent for an examination on 11/2/05 and was diagnoised with a lower back strain.
Employee and coworker were carring a 102 lb. electric motor up stairs. Employee strained lower back.
Injuried was trying to up choke feed chute to #6 cement mill using a 3 lb hammer. He missed the chute with a swing and strained his right forearm
Employees has been diagnoised with an allergic reaction to cement dust.
Employee was on top of dust collect and turned and tripped over door latch striking left knee
Employee was water blasting preheater. He turned and struck his left tiba on the pressure relief valve of an air cannon. Incident reported to company on 8/8/2005; diagnoises recieved on 8/10/05.

The coal mill system shut down & on restart we experienced a coal mill explosion. The explosion was attributed to smoldering coal under the bowl of the coal mill.
EMPLOYEE WENT TO USE BLOW OFF STATION. WHEN HE TURNED ON THE TOGGLE SWITCH IT SHORTED OUT CAUSING THE EE TO RECEIVE AN ELECTRICAL SHOCK & BURNS TO HIS RT. THUMB.
Glowing ember in rehect compartment was caused by broken scraper. The result was an explosion that blew out 4 explosion doors in the circuit. The scraper was replaced and doors were replaced.
Employee walking through plant when dust got into eye.
Employee was changing a seal on the no.4 raw mill cylinder while another employee let the pressure off of the system. He tried to remove the seal when some fluid squirted him in the face.
Ee was attempting to move a metal floor plate when the metal floor plate slipped causing a contusion to his left middle finger.

REBOUND FROM SHOOTING GUNITE MATERIAL , GOT INTO EYE.
EE WAS CLIMBING UP THE LADDER TO DO AN INSPECTION OF THE CHUTE UNDER CLINKER BREAKER. WHEN HE GOT TO THE TOP RUNG HE FELL FORWARD SHIFTING ALL OF HIS RIGHT LEG & PUTTING EXCESS PRESSURE ON THE RIGHT ANKLE.

EMPLOYEE WAS WALKING ON BUILD UP ON PLATFORM AROUND THE TOP OF THE ELEVATOR WHEN HE SLIPPED & TWISTED HIS RIGHT KNEE. ORIGINAL ACCIDENT 01/28/04, AGGREVATED INJURY 04/08/04, SURGERY 05/19/04.
EMPLOYEE WAS USING A CLAY DIGGER TO PUSH OUT BOLTS IN THE #7 MILL WHEN THE LINER FELL STRIKING HIS LEFT FOOT.

COAL DUST ACCUMULATED ON NEWLY INSTALLED HOT AIR DUCT. THIS COAL WOULD EVENTUALLY IGNITE & DROP INTO MILL INLET RESULTING IN AN EXPLOSION. NO MAJOR EQUIPMENT DAMAGED, MILL RUPTURE DISK RELEA VED PRESSURE AS DESIGNED.
HORIZONTAL DUCT FROM NEW HEATER HAD ACCUMULATED COAL DUST WHICH CAUGHT ON FIRE. FIRE INSIDE DUCT WAS IGNITION POINT FOR COAL DUST TO EXPLODE AND REPTURING THE 2 RELIEF DISKS.
EMPLOYEE WAS INSTALLING BELTS ON WATER PUMP. HE ROLLED BELT ON PULLEY & GOT FINGER BETWEEN BELT & PULLEY.
EE WAS INSTALLING A NEW DRIVE BELT ON BAG PALLETIZER. HE HAD TO PRY ONE OF THE ROLLS OVER TO STRETCH BELT OVER PULLEY. LATER IN THE DAY HE FELT SOME DISCOMFORT GOING DOWN HIS LEFT SIDE OF HIS NECK AND DOWN THE ARM AND IT GOT WORSE DURING THE NIGHT.
EMPLOYEE WAS CLEANING OUT MATERIAL FROM A CLOGGED FEED CHUTE. AFTER CONTINUOUS PUNCHING WITH AIR LANCE IN AWKWARD POSITION, SHOULDER STARTED HURTING.
EE WAS LIFTING A BAG OF OIL DRY WHEN HE FELT A PULL IN HIS CHEST.
EE WAS LIFTING MATERIAL WITH A COME A LONG AT LURGI SCREW. THE COME A LONG SLIPPED A GEAR WHILE HE WAS CRANKING THE HANDLE. THIS IS WHEN HE FELT SMOETHING PULL IN HIS RIGHT SHOULDER.
EE WAS REMOVING DUST COLLECTOR BAGS FROM COLLECTOR WHEN HE FELT A PAIN IN THE LEFT SIDE OF HIS NECK. SOME OF THE BAGS WERE HALF FULL OF DUST.
EE WAS CLEANING OFF WELL OF PREHEATER WITH A BAR WHEN A CHUNK FELL HITTING THE END OF THE BAR CAUSING THE BAR TO STRIKE EE'S RT. SHOULDER.

EMPLOYEE WAS INSTALLING PUMP AND HEAT EXCHANGE TO COOL OIL IN BEARING AT #2 KILN PIER. WHEN PUMP STARTED RUNNING EMPLOYEE WAS CHECKING RETURN LINE FOR FLOW WHEN A FLAME CAME OUT OF THE PORT IN THE TOP OF THE BEARING, BURNING HIS RIGHT FOREARM.
EMPLOYEE WAS DISMOUNTING A WA-500 FRONT END LOADER. WHEN STEPPING FROM THE LADDER TO THE GROUND HE STEPPED ON A ROCK CAUSING HIM TO LUNGE TO THE RIGHT IN AN AWKWARD POSITION.
EMPLOYEE WAS JACKHAMMERING #6 MILL FEED ELEVATOR CHUTE. HE WAS BENT OVER IN CHUTE BREAKING OUT MATERIAL. HE PULLED JACKHAMMER UP TO START ANOTHER CUT & FELT SOMETHING PULL IN HIS BACK.
INJURED WAS REMOVING MATERIAL FROM BEHIND LOSHE MILL ROLL WITH 4"" HOSE OFF OF THE GUZZLER TRUCK. THE FORCE FROM THE HOSE PULLING THE MATERIAL CAUSED A JERKING MOTION CAUSING A STRAIN TO THE LO WER BACK.
EE WAS CLEANING OFF RETURN ROLL ON D-10 BELT WITH A METAL BAR. THE BAR WENT THROUGH THE MUD, CAUSING HIM TO STRIKE HIS HAND AGAINST THE METAL FRAME OF THE BELT.
EE WAS PULLING ELECTRICAL CABLE FOR MOTORE AT #1 BELT. HE STEPPED DOWN ONE STEP TO MAKE ANOTHER PULL AND STEPPED ON A PIECE OF ROLLED STEEL, CAUSING HIM TO TURN HIS ANKLE.

INJURED WAS WALKING UP STEPS GOING TO TOP OF SILOS. AFTER THE FIRST LEVEL OF STEPS HE TURNED ON THE PLATFORM TO THE NEXT LEVEL OF STEPS. THIS IS WHEN HE TWISTED HIS KNEE.
EE WAS PLACING A O.5 LB LEDGER BOOK IN OVERHEAD BIN WHEN HER LEFT SHOULDER GAVE WAY.
DIGGING OUT TOE WITH WA700 FRONT END LOADER WHEN BUCKET JUMPED THE TOE AND HIT THE QUARRY FACE OF THE 370' WALL CAUSING HIM TO SNAP HIS NECK IN A WHIPLASH TYPE EFFECT.
AN UNPLANNED EXPLOSION OCCURRED IN THE COAL MILLDUCT WORK. TWO RELIEF VALVES WERE BLOWN, NO OTHER DAMAGE RESULTED. NO INJURIES OCCURRED.
THE PLANT EXPERIENCED AN UNPLANNED EXPLOSION OF COAL DUST INSIDE THE COAL MILL SYSTEM. DAMAGE WAS SUBSTAINED TO THE COAL CYCLONE, COAL MILL AND DUCT. NO INJURIES OR BUILDING DAMAGE. THE IGNITI ON SOURCE IS UNDER INVESTIGATION.
EE WAS BURNING BOLTS OUT OF GUARD. HOT BOLT BURNED HIS LEG. FIRST AID WAS GIVEN. LEG BECAME INFECTED & HE WAS SENT TO SPECIALIST 2/4/01, DIAGNOSED AS HAVING A REACTION TO THE MEDICATION HE HAD RECEIVED ON 1/26/01.

WHILE TAPING THE GUZZLE HOSE, THE AIR CANNON DISCHARGED BLOWING DRY CEMENT UNDER HIS SAFETY GLASSES INTO THE LEFT EYE.
INJURED WAS FILLING NO. 1 SULLAIR COMPRESSOR WITH OIL. INJURED REMOVED TOP VALVE AND FILLED SPOUT WITH OIL, SPLASHING SOME OIL IN HIS EYE.
