Struck against stationary object

Employee was loading a broken heel plate from a loader into the back of a service truck. EE cut left pinky finger on the plate as EE tossed it into the bed of the truck. EE received 8 sutures and was released to full duty.

Employee felt something in eye when EE and others were in the process of installing a vibrator on a sand bin. A foreign object was removed from EE's eye at the emergency room and EE was returned to full duty.

On 6/28/18 the police requested our permission to search the property at our Trinity Quarry to look for a person that had recently gone missing from a nearby home. Around 11:30 a.m. the police found the deceased body of the missing person in a heavily wooded buffer on VMC property (inactive mining area). The police stated the person had left home on foot and overdosed on drugs.

Fall from machine

Cold weather contributed to frozen material in a hopper resulting in the under hopper conveyor to be inoperable. The employee climbed on the machine to investigate and slipped and fell to the ground resulting in shoulder contusion

Employee was tightening driveline bolts with a hand ratchet and it slipped hitting EE in the upper lip causing a laceration to the lip.

EE was performing maintenance on the liner in the surge tunnel feeder. While using a pry bar to loosen the liner he felt a sting in his back. He did not request medical attention until April 9, 2012. He was diagnosed with a lumbar strain and placed on restricted duty.

EE was tightening screw w/ratchet, felt slight strain in lower stomach, but didn't feel it necessary to see Dr. In March '07 he requested to see a Dr, who thought it was a recurrence of pre-existing hernia. He referred EE to surgeon, who placed EE on app. 6 wks restricted duty. Once surgeon's medical notes were rec'd (4/5/07) & reviewed it was determined this was a new hernia.

EE WAS WATERING ROADS IN THE PIT. HE STARTED TO MAKE A LEFT HAND TURN AT THE END OF THE RAMP. HEBEGAN COUGHING AND BLACKED OUT. THE TRUCK STRUCKA BOULDER.
