Miner fell stepping off excavator track

Over-exertion in lifting objects

Employee was removing rocks by hand from the mouth of the jaw crusher when EE felt pain in the right bicep. EE sought medical attention on April 7, 2021. The MRI revealed a torn bicep muscle. Employee is currently working restricted duty until EE's surgery date, April 20, 2021, which will then become a lost time injury.

Fall onto or against objects

While assisting in the removal of the barge pump from the sump, the employee fell into a razor blade that had been removed from EE's utility knife causing it to penetrate into the palm of EE's hand. EE sought medical attention and is currently working on restricted duty.

Employee was installing a steering cylinder on a 988F wheel loader when a metal splinter dislodged from a pin head into his right thumb. The splinter was surgically removed on July 23, 2012. He has remained on full duty, no lost time or restrictions. This incident is being reported within 10 days of employee receiving reportable medical attention.

Fall to the walkway or working surface

EE was helping to change a conveyor belt by pulling on a strap. The strap slipped off the conveyor, he fell backwards and tried to break his fall with his left arm. The employee's left arm was fractured as a result of the fall. This injury is reportable because a bone was fractured.

Employee was observed by Plant Manager as showing signs of a heart attack. Upon completion of the Plant Mgr's assessment of the employee's condition, the employee became faint and discolored. The Plant Mgr immediately transported the employee to the Hospital where he was diagnosed as having a heart attack. Surgery was performed and he is in stable condition.

Employee was using a #16 hammer to beat the side of the chute to dislodge material. As he was swinging the hammer he noticed something pulled in his shoulder.

Employee was getting off of a Komatsu 220 hammer when feet slipped and he fell. It was raining at this time. **Received 8 Sutures**

Employee was cleaning trash out of cab of haul truck when he felt something stick his finger. The next morning his finger was swollen up. Sent him to emergency room to have it checked out.

WORKING ON TAIL PULLEY ON CLASS ONE BELT WHEN EMPLOYEE GOT A PIECE OF DEBRIS IN HIS LEFT EYE. THE WIND WAS BLOWING PRETTY HARD AT THE TIME. EMPLOYEE EYE WAS WASH OUT AND LATER ON THAT NIGHT HE HAD TO GO THE DOCTOR.

THE EE WAS GETTING OUT OF MAINTENANCE TRUCK. THE WIND BLEW THE TRUCK DOOR CLOSED, CATCHING THE EE'S SHIRT IN THE DOOR CAUSING HIM TO LOSE HIS BALANCE & FALL.

EE WAS CLEANING OUT A STOPPED UP CHUTE. HE WAS USING A FLAT BLADE MOUNTED ONTO A ROUND ROD. WHILE PUSHING OUT MATERIAL BUILD UP HE HIT HIS PINKY FINGER ON RIGHT HAND BETWEEN ROD & TOP OF CHUTE . HE CUT HIS FINGER REQUIRING 7 STITCHES TO CLOSE IT UP.

EMPLOYEE WAS HELPING TO INSTALL ROLLER ON CONVEYOR BELT. WHILE TRYING TO REMOVE THE BAD ROLLER HE WAS HOLDING WHILE ANOTHER EMPLOYEE WAS HITTING IT WITH A HAMMER. THE ROLLER TURNED AND THE HAM MER HIS HIS LEFT INDEX FINGER. THE FINGER NAIL WAS REMOVED AND TWO STITCHES PUT IN THE END OF THE FINGER.

EMPLOYEE WAS HELPING TO DISLODGE A PIECE OF STEEL HUNG IN JAW CRUSHER. THEY WAS USING A DRIFT PIN AND A SLEDGE HAMMER. INJURY EMPLOYEE WAS WATCHING ANOTHER EMPLOYEE HIT THE DRIFT PIN WITH HAMM ER WHEN A PIECE BROKE OFF & FLEW INTO CORNER OF LEFT EYE. THE PIECE WAS HOT & BURN EYE.

EMPLOYEE WAS HELPING TO PULL SLEEVE OFF OF AN ENGINE. THE SLEEVE PULLER FELL & HIT EMPLOYEE ON LEFT HAND CAUSING A LACERATION ON RING FINGER AT THE SECOND JOINT. 4 STITCHES.

EE WAS REMOVING A BUSTED HYD. HOSE ON DRILL. THERE WAS OIL WHERE HE WAS WORKING. EE FEET SLIP AND HE GRAB HOLD TO KEEP FROM FALLING. WHEN HE GRAB HOLD HE STRAIN HIS BACK.

EE WAS HELPING TO TIGHTEN BOLTS ON CRUSHER WHEN HE TURN AND PULL MUSCLE IN HIS BACK.

EMPLOYEE WAS TIGHTENING BATTERY TERMINAL ON TRUCK WHEN THE WRENCH HE WAS USING MADE CONTACT WITHMETAL BOX. THE WRENCH WHEN SHORTENED MADE CONTACT WITH EMPLOYEE RING AND HE RECEIVED THIRD DEGRE E BURN ON RING FINGER.
