Drill operator was fatally injured by the drill rig he was operating. The likely scenario is that the drill rig began to roll forward while he was at the back of the rig operating the controls for the drill. The victim may have attempted to stop the drill rig and was caught up under the rig and suffered fatal injuries. There were no witnesses to the accident.

Fall to the walkway or working surface

IE clocked out went to EE's car and changed out of work boots and put on slides, went to the back of EE's car, stepped off asphalt into muddy area and fell. Broke 2 bones in EE's ankle.

Absorption of radiations, caustics, toxic and noxious substances

EE was reaching to grab a rain jacket in the bed of his pickup. He felt a sharp pain when he grabbed it and pulled his hand back, and his finger started to swell. EE went to the Doctor and they said he was stung by some sort of insect.

Caught in, under or between two or more moving objects

EE holding one side of a suspended roll for conveyor belt, that was being held by an overhead crane with a bar running through the middle. Another EE was prying nails out and the bar in the middle failed and the EE's hands were caught between the hook and the bar.

Employee was attempting to clear boulders from a clogged chute and suffered a back strain.

Employee slipped on 1/9/2010, employee refused medical treatment at the time.On 1/14/2010 the employee reported pain in rib cage and was sent to the doctor. Employee was found to have a cracked rib and was given a prescription and returned to work after the examination.

Operator woke up morning after shift that started 7th of October 2009, with neck pain (strain), went to medical facility morning after his shift and was issued pain relievers and muscle relaxants. Returned to work for the PM shift on the 8th of October 2009. Employee did not immediately report incident to supervisor prior to leaving site after shift was over.

While walking down ramp to the pump, employee slipped on rocks and tumbled down hill. He put his hand out to catch his fall and sprained it.

Employee was adding a suction hose on a pump. While clamping up to pump, the clamp fastener lever caught his right hand. Employee did not go to the doctor until 3/24/08.

Employee was lifting trailer onto back of truck and dropped trailer on foot causing a fracture.

While attempting to lift a 60 lb screen and move it out of the way, employee felt sharp pain in lower back.

Co-worker backed a Cat 773 rock truck into the cab of another Cat 773 rock truck. MSHA was contacted the night of the accident and an investigation followed.

Employee was walking from crusher house to jaw crusher and walked into a metal plate on boom truck.

Over-exertion in pulling or pushing objects

While moving a plate in a loader bucket, the plate hung up behind the bolts that hold the bucket edge on. Injured reached to hold his left arm on top of bucket and right arm and hand on plate. When he pulled to dislodge the plate a sharp pain was felt in his left shoulder area.

EE REACHED DOWN AND BEHIND WITH HIS RIGHT HAND TO FILL THE HEATER BLOWER FAN BECAUSE HE FELT IT WASN'T BLOWING VERY HARD. HE FAILED TO LOOK AT THE BLOWER FAN BEFOR PUTTING HIS HAND IN THIS ARE
