Employee was cutting zip ties off a set of new loppers and as they were pushing on the box cutter, it went through the zip tie cutting a finger.

While moving equipment, the EE pick up an electrical box and swung it to the opposite side of the room. While swinging the box, their glove became stuck. The momentum carried them to the wall, and they jammed their neck. EE was put on restricted duty on 2/12/25.

While blowing lime dust from the lime cooler access door with air lance, EE got lime in eyes.

The employee was cleaning the roof on the top lime silo with a pressure washer. After EE finished part of it, EE went downstairs between the 5th and 4th floor and slipped on a step. EE was holding onto the handrail with right hand.

Temp was helping to prepare the vacuum truck hose. While holding the end of the hose, EE's arm was sucked into the vacuum hose.

EE was carrying 5-gallon fuel cans up top the warming heaters and strained shoulder.

Employee was removing/installing a motor base for 3B screen. When employee bent over to grind a weld employee felt back pop with an onset of pain.

EE stepping down off a ladder and missed a rung causing EE to strain shoulder.

After changing out conveyor return rollers the injured when to dispose of the rollers. While putting the rollers in the dumpster the injured stated that they felt a strain in right arm. The injured did not report the possible injury to management. Around two weeks later, the injured reported the injury to their Manager. Injured had surgery on 4/3/2023.

Field Technician was attempting to remove debris (dirt, mud, blasting agent residue) from pants leg by using a personal open blade pocket knife. In the process of pulling pants leg tight while scrapping the pants leg with a knife, the Field Technician lacerated their left forearm requiring two stitches.

While cleaning the silo, the employee felt light-headed from the high heat index.

Employee was walking between rows of drilled pattern on a 21 degree slope (approximate) when EE stepped on a loose stone with EE's right foot causing EE to fall to the ground resulting in a right ankle sprain and right knee contusion.

Employee was loading emulsion blasting agent into drilled holes on a 21 degree slope pattern by manually pulling a 1.5 inch hose from hole to hole pumping product into holes when EE stepped on a loose softball sized rock causing EE to lose EE's balance. Employee regained EE's balance before falling to the ground, however, in doing so, employee strained EE's lower back.

Employee walked through a puddle of water in the lime handling area. EE's boots were in disrepair and lime water got in the boots.

an alarm for high temp came up on kiln 2 shell scanner. The supervisor on duty immediately inspected the kiln and found that a fire had ignited on K2 drive pier on the main drive gear.

A bulk loader bent over at the waist to install a tamper seal on a railcar. Once the seal was attached the employee was raising back up when EE felt a sharp pain in lower back. The employee was diagnosed with a muscle strain in the lower back. EE was placed on restricted duty.

EE arrived at the Annual Refresher class and later could not rise out of the seat. EE told the Safety Manager that the back was hurting due to pot holes in the haul road. On the initial doctors visit there were no restrictions. When the EE returned to the doctor on 10/22/18 for a follow up and was still in pain, the doctor gave EE a back brace.

EE hurt hand. EE when to the Dr. received meds. Meds made ill. Next day, EE sent a text to super. was not coming to work, ill. Called EE asked them to come, go to the Dr. EE said they were 5 hrs. away and was not coming back to see Dr. Manager called Denver office for guidance. We received notice on 9/7/18. The Denver office considered this injury reportable

Employee injured hip while climbing out of the lime cooler ductwork access door.

EE discovered a knot on EE's left hand on 6/15/18, EE went to the company doctor on 6/18/18. it was determined that there was a piece if metal in the left hand. EE was put on light duty.

A lightning storm caused a break in power that led to the coal mill on Kiln 2 going down. While trying to restart the coal mill fan, the attendant noticed a issue with the starter. An electrician was notified to report to the plant to check the starter. At approximately 12:45am the coal mill expansion joint caught on fire.

Employee spent the day walking within the kiln area and received blisters to the right foot, which required medical treatment. A preexisting condition prevented the employee from being aware of the injury. The employee was placed on restricted duty 2/1/18.

Employee was pulling bungee strap on Dump Trailer. The bungee cord broke and ee fell and hurt left wrist.

EE was cleaning out a railcar and re-injured EE's back

Employee was removing build up of material on surface of refractory brick inside of kiln #4 to install a refractory brick patch when a piece of a metal shim broke off and entered the employees right upper arm requiring medical treatment to remove.

Employee was replacing V-belts on a screw conveyor and got lime in EE's eye.

Driver was disconnecting unload hose from pump on tanker using manual force and under no pump pressure. When hose dislodged, employee struck self in the upper lift with the fitting on the end of the hose causing laceration to upper lip, resulting in 5 stitches to lip.

Passenger elevator was coming down hit boom of manlift and it fell approx 20'

On 7/2/2016, EE was removing buckets from a bucket elevator chain. While completing this task, the EE hit left thumb with a hammer. EE reported the injury on 7/5/2016.

Employee was conducting an inspection inside Kiln # 2 when, looked up and got lime dust in eye.

EE was helping other EE's move an electric motor from it's base. After completing the task, the injured EE felt tightness in EE's shoulder.

EE was helping to un-plug a dust chute and got dust in his right eye.

Employee exited the south doorway from the quarry maintenance shop and twisted his ankle while stepping from the door's threshold concrete pad to the ground.

An EE noticed flames and smoke coming from a dust collector. The EE called the supervisor and tried to put the fire out. The flame continued to reignite. Around 1:56AM the collector caught fire and set several conveyors on fire as well. The fire department was contacted and arrived on-site to put out the fire. They left the site at approximately 3:45AM.

EE experienced heat exhaustion

EE was exiting the man elevator of the lime house. When he attempted to close inner door it temporarily hung and when it broke loose his right hand slipped off of the door and in between the top and bottom half of the door smashing his right hand and thumb.

Employee was adjusting the knuckle on a railcar and strained his lower back.

Employee was clearing rocks from the south haul road at the 480 Bench entrance road with a motor-grader. LHoist employee was traveling on same haul road and when turning right onto the 480 Bench road struck the motor-grader.

EE was cleaning the windows on a haul truck with equipment wash solution. As he was shutting the pump off the pre-clamped hose separated from the wand and hose end struck him in the face knocking his safety glasses off his face, hitting him in the eye.

EE go debris in his eye on 7/11/15. EE reported the injury aroud noon on 7/12/15. EE does not know how and where the injury occurred.

EE alleges that he was bitten by a spider while sitting in the plant laboratory. A tetanus shot and antibiotics was given to EE as precautionary measures.

Employee was working with two other employees on the 3A screen bottom deck. A co-worker noticed that the injured was getting hot and starting to slow down. The co-worker had the injured come out of the screen and helped him down the stairs and to the maintenance break room to cool off. Injured employee was taken to the ER where he was diagnosed with heat exhaustion.

After completing his task, EE returned to the shop prior to quitting time. He was feeling nauseous. EE stated that he clocked out and started driving home. On the way home he stated that he started getting chills and stopped sweating. EE texted his supervisor and was taken to the doctor the next day. EE was diagnosed with heat exhaustion.

At 8:00am EE was helping install a brush on the sweeper. While installing brush, EE experienced pain in back and shoulder. EE reported the injury to his supervisor at 1:00pm and stated he was OK. On Saturday,4/11/2015 EE called his supervisor and asked to see a physician. EE was diagnosed with a pulled back muscle.

EE was checking on a cooler feeder that wasn't working properly. EE removed the covers from the feeder to troubleshoot. EE used his middle finger to feel if the lime was packed behind the coil. While sweeping lime out EE felt a pop in the tip of his middle finger. When he pulled his hand out he noticed the tip of his finger was slightly bent and swollen.

While raking the kiln cooler grates the employees finger was caught between the rake handle and the platform's handrail.

Opening Roll Up Door Lube Truck. Strained His Lower Back.

Employee was shoveling lime out of a door on the passenger side of the vacuum truck. While performing the task lime flushed out of the compartment hitting the employee on the right side of the face. The employee felt lime contact his eye.

Driller climbed into a drill to ask a co-worker a question. When the driller stepped down to exit the drill, he twisted his left ankle.

employee was closing a slide gate on a LKD (lime kiln dust)tank while a particle of lime got in his eye.

While cutting conveyor skirt rubber, the employee punctured himself in the left abdomen with a razor knife.

Employee arrived at work and went to his locker to retrieve his boots. As he bent over to put his boots on, he felt a pain in his lower back.

While riding as a passenger in a on-site service truck, the service fuel truck run through a pot hole, causing the miner to hit his head on top of roof.

Employee says he slipped off back of fuel truck and landed on his feet wrong, jarring his back.

Employee lost control of the waste truck he was driving and jumped from the truck while it was moving backwards. Employee received a sprained right ankle and an abrasion to his right elbow when he jumped from the moving truck.

while using a rivet buster to break refractory brick, the brick broke loose, allowing the rivet buster to swing freely. This motion caused the rivet buster to smash employee finger against a scaffold pole

Employee was attempting to remove a impact socket that was stuck in a nut with a hammer. When the socket start to come free, the employee grabbed the socket and struck in again hitting his finger with the hammer.

Employee was changing bit on the drill and pinched his right hand with bit. His skin was pinched but he did not require sutures.

Injured was pushing an insert back into a control knob with his right pinky. His finger rolled forward causing a injury. Injured saw a doc on 9/12/12. Doc put a splint on finger to protect from impact. Injured saw Doc again in two weeks. Doc wanted him to see a surgeon. Injured saw surgeon on 10/12/12. Surgeon recommended out patient surgery. Surgery was performed on 11/5/12.

While blowing bridged lime from a chute, the employee got lime in his eye.

Employee went to the emergency room on his own after his shift was over. He claimed that while working outside he became dehydrated. Employee claims he was experiencing severe muscle cramps, nausea, and vomiting, none of which was reported or observed at work.

While preparing to walking away from a completed task, the employee turned and injured his knee.

Employee allegedly injured himself on 7/2/2012 and did not report the injury until 7/4/2012. The employee reported that he slipped while descending a incline after completing an environmental inspection at the south retention pond and injured his ankle.

Employee was attempting to repair an oxygen hose on a cutting torch. while cutting the oxygen hole to splice it, the employee cut his left thigh with the razor knife. The laceration required four stitches.

Employee was retrieving a material sample from a tanker truck with a sample scoop. When the scoop entered the material stream, it jerked the employee's arm.

Employee was attempting to clear a bridged pre-heater ram using a high pressure water jet. When employee started clearing the bridged material, dust and steam blew out of the access port

Employee was cleaning up at the Primary Crusher with the Dingo skid steer. The employee slipped and fell while operating the skid steer and the machine pinned him against the cement retaining wall.

Employee was troubleshooting an issue at the tail pulley of the BC-3104 conveyor belt. While removing a back plate on the feed chute, employee twisted his right ankle.

While climbing a flight of stairs, the employee stumbled and hurt him left foot. It was determined that the employee had a hair line fracture at his first metatarsal.

was walking through building. was wet lime on the floor. His left leg slid forward causing pain in the knee. regained footing and assessed that it hurt but he was fine and finished shift. Continued to work full capacity until 12/15/2011.he originally thought he could possibly have gout. been waiting on a diagnosis (which I received today 1/12/2012) that said injury was due to slip.

While closing the door on the lime handling man elevator the employee shut the doors on her hand.

Employee washing out preheaters and he took off his PPE while moving between preheaters. When the job was set up and ready to start it was the employee's turn to wash and he did not his PPE on. As a result when he injected water into the preheater which had hot debris inside, hot steam and dust blew out of the preheater causing burns to the employee's exposed hand.

While removing a guard from a pre-heater ram, the employee strained his lower back.

Employee was stepping down from the operator platform of a boom truck and twisted his ankle.

EE was grinding metal with an electrical grinder, when a piece of metal lodged in his right eye. He was using PPE. We were informed on 9/19/11 that the metal was removed with a tool by the MD.

Employee was flipping a rail switch and dislocated his finger. The Employee did not report the accident until 8/31/2011. The Safety supervisor accompanied the employee to the ER on 9/6/2011.

While repairing the air knife in the 2411 rotary feeder, the miner got lime in his boot causing lime burns to his ankle

The employee was walking up the # 3 surge conveyor catwalk and stepped on a loose section of floor grating. The grating slide from its supports and fell, causing the employee to fall to the ground.

Employee was moving rail-cars in the rail yard. Employee was crossing the rail track and slipped, hurting his upper thigh.

A piece of metal fell on EE's upper left arm. He was scratched and bruised on his left wrist.

Employee was entering skid steer when footing slipped, causing him to fall to ground striking right shoulder against ground, resulting in dislocating right shoulder.

Injured dumped a loader of waste and pulled up to re-attach his tailgate chains. The chain was caught between the bed and the tailgate. The injured pulled the tailgate out with his right hand, when he moved the chain, the tailgate closed on his fingers.

The employee was attempting to dislodge a piece of metal from the southeast cooler feeder of kiln number one lime cooler. The pry bar he used had an L-shaped hand grip. While thrusting the bar through a four inch by four inch clean out port, the bar missed the metal and the employee's left middle finger was smashed between the bar and the clean out port.

Miner was changing a drill bit on a DX 800 track-mounted slope drill. While walking back to the pickup truck with the old bit, a loose rock slipped from under his left foot causing him to twist his left ankle. Miner report the injury and told his supervisor that he was OK. On 10/11/2010 miner informed me that he needed to go to the doctor.

On 8/11/2010, The injured was replacing a water line on the oxygen analyzer at the feed-end of #1 kiln. when the injured reconnected the hose and turned the water back on, the hose blew off spraying hot water on the employee's face, stomach area and left thigh area.

Employee was using the portable toilet when he was bitten on the testicles by a black widow spider.

Employee was lifting his tool bag into truck and sprained his sternum.

Lime in eye while cleaning cooler grates on kiln 1. Employee was taken to doctor and given a prescription for eye.

Employee jammed left knee when he stumbled, descending metal stairs.

Driller stepped out of the back of pickup truck and slipped on the bumper of the truck. He cut his elbow on the rocks on the ground.

Maintenance worker jammed left finger while closing door on a blower motor. Splint was put on finger by doctor.

EE was whole watch at bag house, ee removing bags. EE was warned 3 times to put respirator back on, was sent home after 3rd warning. 7:30pm ee could not breathe and went to emergency room. ""See attached letter"".

Employee injured his back while installing a DC drive motor in the reclaim tunnel on October 28, 2008. This injury was filed in October of 2008. This is a reoccurrence of an old injury.

Working with sledge hammer. Index finger on left hand got caught/smashed between hand rail of manlift and sledge handle while swinging hammer.

Worker was either breaking castable or welding on elevator buckets when something got in his eye. Employee did not know that something was in eye until he awoke to find his right eye matted shut and puffy. Worker was taken to eye doctor and given a prescription for eye.

Employee was mucking out the settling ponds at the mill using a longreach trackhoe. He caught a ride back to the quarry garage on a waste truck. When he exited the waste truck, he slipped on the steps, lost his hand hold and fell to the ground on his rear. The fall was from the top step approximately three feet from ground level.

Removing coal ring from kiln when strained neck and shoulder occured.

Employee was inside the framework of #2 surge conveyor preparing to cut an roller bracket off with a oxygen/acetylene torch. When employee lit the torch, he heard a gurgling sound. Employee attempted to the gas and air off. before he could get it off, the oxygen hose blew into causing 1st degree burn to the employee.

Employee was dumping mud from a 773 Rock Truck. The mud became stuck in the bed, as the employee put the truck in drive gear and started forward, the front end of the truck rose off the ground about 6 ft then falling back to the ground, jarring the employee's lower back.

Employee driving a 773B rock truck. While the truck was being loaded with stone, the trackhoe bucket and stone jarred the truck, causing employee to experience neck and shoulder pain.

Employee was troubleshooting a problem with the #3 tertiary crusher. Fellow employee came to help. After finding the crusher plugged, the fellow employee attempted to de-energize the circuit breaker. Employee was standing behind fellow employee in the doorway. As fellow employee switched off the breaker, it exploded causing an arc flash.

Employee shutdown the crushing plant from the computer in the control room, and left the crusher control room to help troubleshoot a problem with the #3 tertiary crusher. After finding the crusher plugged, the employee attempted to de-energize the circuit breaker. As employee was switching off the breaker, it exploded causing arc flash.

Back dragging with dozer from the B2 stock pile when he backed off in a dip from a previous drag. This caused a jar, causing his neck to start hurting.

EE was attempting to cut broken V-belt from the drive pulley of the grizzly feeder. He was using a utility knife. His first cut was directed away from his body. That cut went half way through the belt. He turned the utility knife toward his body and attempted to finish the cut. The knife slipped off the belt and cut his left thumb.

EE was driving a Ford L-9000 tandem truck from the settling ponds to the east waste pile. After driving for two hours, he experienced sharp pain in his right shoulder. He took two pain tablets and continued to drive until approximately 2:45 pm. EE told supervisor that the pain had gotten worse and he needed to see a physician.

Miner was helping to set a portable diesel pump. While lifting the pump off of the towing ball, the pump moved forward and miners left ring finger was mashed between the tow ball on the truck and the ball receiver hitch on the portable pump.

Closing door on a railcar using a bar. The bar gave way pinning fingers between the bar and the ground. Mashed the tips of two fingers ( middle finger & ring finger) right hand.

Employee was tightening a bolt on kiln 1 with a wrench. His hand got caught by the ram causing the wrench to go through his hand.

The uphill drive side bearing was being replaced. The new bearing was being held in place with a chain connected to a chainfall. Employee began pushing the uphill drive side bearing housing on to the drive shaft and sustained a hernia. Employee had surgery on 10/17/08 and began working restricted duty on 10/21/08.

Employee was cleaning the hydraulic room. There was approximately one inch of water, hydraulic fluid and glycol on the floor, and employee received burns to his left foot.

Employee was looking at driver's side mirror with her head leaning to the left. Large boulder loaded onto rock truck. Boulder shifted causing employee to hit her head inside the cab.

Employee was climbing down from the D8 dozer and slipped on the step hitting his knee on the tracks of the dozer. Employee developed cellulitis in the knee.

Worker was using a 12 lb. sledge hammer to break up a concrete wall. When he stopped and began to clean up the area, he experienced intense pain and was temporarily unable to use his right shoulder. The injury was diagnosed as a shoulder strain. He was prescribed anti-inflammatory and pain medication for one week.

Employee was climbing off of a 980 loader and twisted his left ankle.

Employee was working on kiln 2 dump gate, heard someone walking on grate above and looked up, lime fell in eyes.

Employee was cleaning feeders on kiln 1, when hot lime dust blew out of the port hole, under safety glasses and into his eyes.

Employee was training on the tunnel belt, using a 16 lb hammer. He was adjusting the roller by tapping on it. He missed the roller and smashed his left thumb against the tunnel belt frame.

Employee was climbing into the motor grader when the step broke and he fell onto his back on the ground.

Employee noticed pain and swelling in his right wrist after working an extended shift.

Employee fell from elevated platform injured shoulder and ribs. Was not wearing mandatory fall protection

employee was struck by a piece of steel wedge that sheered off while striking it.

Employee was walking with a lit torch and it brushed his leg and burned him.

Employee was cleaning grates when rake slipped sending him back into the hand rails hitting his lower back on the rail. Employee did not go to the doctor until 8/25/2007.

Attempting to attach a weld trailer to the hitch on a pick up truck, trailer rolled forward and caught his hand between the hitch and the truck.

Employees were pulling bags out of the baghouse while holding the cage up, the cage slipped and fell on one of the employee's fingers.

Cleaning out dust collector on top of #16 bin and lime shot out into eye. Employee was given prescription medication.

While punching a hole through the screen to install a patch a piece of lime bounced up off of the screen and into his eye. Employee went to the doctor on 06/21/2007 and was given prescription medication.

EE was welding when he turned to get a part when another EE began grinding. The grinding debris went under EE's safety glasses & struck his left eye. He suffered a corneal abrasion and was treated with an eyewash & antibacterial cream from the physican. No futher problems have been noted.

Trying to unstop tip of pressure wand. When injured thought it was unstopped pressure was added and the wand twisted around and smashed fingers on a hydraulic line.

Cleaning K-1 ram with lance, injured felt pain in lower back.

While assisting on coal mill project, left thumb was pinched under a large cone.

Employee was cleaning the mirror on a mobile crane when he slipped and fell landing on his elbow.

EE was on a crane platform instructing a worker what to do when he slipped on iced over platform and fell backwards off the platform about 10 to 15 feet into a puddle of ice water.

Handle of sledgehammer broke and head of hammer hit employee in the forehead.

Injured was helping clean rams pulling on wand and hurt his back.

EE was pulling on a tagline which was attached to a steel pipe hanging from a crane. The tagline came lose and the employee fell backwards and struck his elbow on a steel beam.

EE was rigging beams and instructed the forklift operator to retract the forks of the lift. When he retracted the forks they caught on the ground causing the beam to turn over onto the EE's foot causing his big toe to be broken.

While working on kiln pier employee tripped over welding lead and fell on his right shoulder and his left knee causing a contusion injury to his left knee. Employee received treatment for his injury and will have followup at the doctor's office to determine when he will be able to return to his regular duties.

Employee squatted down to glue PVC conduit & felt his knee pop. An MRI on 6/30/2006 showed a torn ligament. Surgery performed on 7/5/2006.

The loadout chute center spout broke. Injured was underneath it trying to line it up. Some lime dust came loose and hit him in his face.

Injured was attempting to break loose a plugged lime chute using a bar and hammer. When lime broke loose, it came through the 5"" port hole and hit injured in the face. It knocked the dust mask loose causing injured to breathe in a large amount of lime.

EE was 7' above ground with safety harness on. He secured beam w/rope to forklift to bring beam down. He used cut torch to release beam from building. Untied from the building and came down ladder. Forklift held beam in position. Beam made a noise as it settled on forklift arms and EE thought beam would fall on him so he jumped off ladder 5' above ground. Result: fractured tibia.

Injured pulled the wrong lever and caught his thumb in between the wrench and the drill. Reportable because of sutures only.

INJURED WAS REMOVING GUARD RAILING FROM AN ABANDONED SECTION OF CATWALK. WEEDS WERE GROWING UP THROUGH THE CATWALK GRATING. INJURED STEPPED INTO A HOLE IN THE CATWALK CAUSING A KNEE STRAIN WHEN HIS LEG & FOOT WENT THROUGH THE HOLE. REPORTED BECAUSE OF RESTRICTED DUTY.

INJURED WAS REACHING IN AN AWKWARD POSITION. HE EXPERIENCED A SHARP PAIN IN HIS BACK CAUSING HIM TO FALL, INJURING HIS ELBOW AND BACK. COULD POSSIBLY HAVE BEEN AN AGGRAVATION FROM AN OLD BACK INJURY OCCURRING 5/8/97. BECAME REPORTABLE ON 6/21/04 BECAUSE OF LOST TIME.

USING DRILL PRESS IN SHOP TO DRILL HOLES IN A PIECE OF PLATE. WHEN THE BIT TWISTED THE PLATE, INJURED GRABBED THE C-CLAMP CAUSING HIS HAND TO BE BINDED SUSTAINING A LACERATION TO HIS THUMB. RE PORTABLE BECAUSE OF SUTURES.

INJURED WAS HOLDING A BAR. ANOTHER EE WAS STRIKING THE BAR WITH A HAMMER. WHEN THE EE'S STRUCK THE BAR WITH THE HAMMER, THE HAMMER GLANCED OFF THE BAR AND STRUCK THE INJURED ON THE CHIN OF HI S FACE. REQUIRING STITCHES.

TIGHTENING LOCKNUT ON THE BOTTOM OF A GRINDING ROLL USING A HAMMER. A SMALL PIECE OF METAL STRUCK THE INJURY IN THE LEG. REPORTABLE BECAUSE OF SUTURE LEG.

INJURED WAS HELPING A MECHANIC REMOVE THE DRIVE SHAFT FROM A 773 B TRUCK. CAP CAME OFF OF U-JOINT AND HIT INJURED INT HE HEAD DIRECTLY OVER THE LEFT EYE. REPORTABLE BECAUSE OF SUTURES ONLY.

INJURED PARTY WASD RIVING TRACK MOBILE DOWN TRACK #2 WHEN INJURED REACHED CARS ON TRACK #2 WITH TRACK MOBILE HIS SPEED WAS TOO GREAT TO STOP BEFORE TRACK MOBILE COLLIDED WITH CARS, INJURED HAD HIS HEAD OUTSIDE OF A WINDOW AT IMPACT, CAUSINGHEAD TO HIT WINDOW FRAME RESULTING IN A LACERATION TO HEAD REQUIRING STITCHES.

WHILE DUMPING LIME INTO A LOADER BUCKET WITH A BOBCAT LIME CLOUDED UP OPERATOR COULD NOT SEE OPERATOR TRIES TO EXIT BOBCAT & SLIPPED ON BUCKET CAUSING HIM TO FALL. OPERATOR TRIED TO CATCH HIM SELF HIS AND SLID THRU LIME ONTO HARD SURFACE CAUSING A BURN ON RIGHT HAND AROUND THUMB/WRIST AREA.

INJURED WAS WORKING UNDER RAIL CAR, INSTALLING ADOOR. WHEN EXITING FROM UNDER CAR, HE FELT PAININ LEFT KNEE. NOTE: WAS NOT REPORTABLE UNTIL SCOPE WAS PER- FORMED.
