Employee was cutting zip ties off a set of new loppers and as they were pushing on the box cutter, it went through the zip tie cutting a finger.

Struck against stationary object

While moving equipment, the EE pick up an electrical box and swung it to the opposite side of the room. While swinging the box, their glove became stuck. The momentum carried them to the wall, and they jammed their neck. EE was put on restricted duty on 2/12/25.

The employee was cleaning the roof on the top lime silo with a pressure washer. After EE finished part of it, EE went downstairs between the 5th and 4th floor and slipped on a step. EE was holding onto the handrail with right hand.

After changing out conveyor return rollers the injured when to dispose of the rollers. While putting the rollers in the dumpster the injured stated that they felt a strain in right arm. The injured did not report the possible injury to management. Around two weeks later, the injured reported the injury to their Manager. Injured had surgery on 4/3/2023.

Field Technician was attempting to remove debris (dirt, mud, blasting agent residue) from pants leg by using a personal open blade pocket knife. In the process of pulling pants leg tight while scrapping the pants leg with a knife, the Field Technician lacerated their left forearm requiring two stitches.

Employee was walking between rows of drilled pattern on a 21 degree slope (approximate) when EE stepped on a loose stone with EE's right foot causing EE to fall to the ground resulting in a right ankle sprain and right knee contusion.

Employee was loading emulsion blasting agent into drilled holes on a 21 degree slope pattern by manually pulling a 1.5 inch hose from hole to hole pumping product into holes when EE stepped on a loose softball sized rock causing EE to lose EE's balance. Employee regained EE's balance before falling to the ground, however, in doing so, employee strained EE's lower back.

Accident type, without injuries

an alarm for high temp came up on kiln 2 shell scanner. The supervisor on duty immediately inspected the kiln and found that a fire had ignited on K2 drive pier on the main drive gear.

A bulk loader bent over at the waist to install a tamper seal on a railcar. Once the seal was attached the employee was raising back up when EE felt a sharp pain in lower back. The employee was diagnosed with a muscle strain in the lower back. EE was placed on restricted duty.

EE arrived at the Annual Refresher class and later could not rise out of the seat. EE told the Safety Manager that the back was hurting due to pot holes in the haul road. On the initial doctors visit there were no restrictions. When the EE returned to the doctor on 10/22/18 for a follow up and was still in pain, the doctor gave EE a back brace.

EE hurt hand. EE when to the Dr. received meds. Meds made ill. Next day, EE sent a text to super. was not coming to work, ill. Called EE asked them to come, go to the Dr. EE said they were 5 hrs. away and was not coming back to see Dr. Manager called Denver office for guidance. We received notice on 9/7/18. The Denver office considered this injury reportable

EE discovered a knot on EE's left hand on 6/15/18, EE went to the company doctor on 6/18/18. it was determined that there was a piece if metal in the left hand. EE was put on light duty.

A lightning storm caused a break in power that led to the coal mill on Kiln 2 going down. While trying to restart the coal mill fan, the attendant noticed a issue with the starter. An electrician was notified to report to the plant to check the starter. At approximately 12:45am the coal mill expansion joint caught on fire.

Rubbed or abraded

Employee spent the day walking within the kiln area and received blisters to the right foot, which required medical treatment. A preexisting condition prevented the employee from being aware of the injury. The employee was placed on restricted duty 2/1/18.

An EE noticed flames and smoke coming from a dust collector. The EE called the supervisor and tried to put the fire out. The flame continued to reignite. Around 1:56AM the collector caught fire and set several conveyors on fire as well. The fire department was contacted and arrived on-site to put out the fire. They left the site at approximately 3:45AM.

EE was exiting the man elevator of the lime house. When he attempted to close inner door it temporarily hung and when it broke loose his right hand slipped off of the door and in between the top and bottom half of the door smashing his right hand and thumb.

EE was cleaning the windows on a haul truck with equipment wash solution. As he was shutting the pump off the pre-clamped hose separated from the wand and hose end struck him in the face knocking his safety glasses off his face, hitting him in the eye.

EE alleges that he was bitten by a spider while sitting in the plant laboratory. A tetanus shot and antibiotics was given to EE as precautionary measures.

Employee was working with two other employees on the 3A screen bottom deck. A co-worker noticed that the injured was getting hot and starting to slow down. The co-worker had the injured come out of the screen and helped him down the stairs and to the maintenance break room to cool off. Injured employee was taken to the ER where he was diagnosed with heat exhaustion.

After completing his task, EE returned to the shop prior to quitting time. He was feeling nauseous. EE stated that he clocked out and started driving home. On the way home he stated that he started getting chills and stopped sweating. EE texted his supervisor and was taken to the doctor the next day. EE was diagnosed with heat exhaustion.

At 8:00am EE was helping install a brush on the sweeper. While installing brush, EE experienced pain in back and shoulder. EE reported the injury to his supervisor at 1:00pm and stated he was OK. On Saturday,4/11/2015 EE called his supervisor and asked to see a physician. EE was diagnosed with a pulled back muscle.

EE was checking on a cooler feeder that wasn't working properly. EE removed the covers from the feeder to troubleshoot. EE used his middle finger to feel if the lime was packed behind the coil. While sweeping lime out EE felt a pop in the tip of his middle finger. When he pulled his hand out he noticed the tip of his finger was slightly bent and swollen.

Employee washing out preheaters and he took off his PPE while moving between preheaters. When the job was set up and ready to start it was the employee's turn to wash and he did not his PPE on. As a result when he injected water into the preheater which had hot debris inside, hot steam and dust blew out of the preheater causing burns to the employee's exposed hand.

The employee was attempting to dislodge a piece of metal from the southeast cooler feeder of kiln number one lime cooler. The pry bar he used had an L-shaped hand grip. While thrusting the bar through a four inch by four inch clean out port, the bar missed the metal and the employee's left middle finger was smashed between the bar and the clean out port.

Miner was changing a drill bit on a DX 800 track-mounted slope drill. While walking back to the pickup truck with the old bit, a loose rock slipped from under his left foot causing him to twist his left ankle. Miner report the injury and told his supervisor that he was OK. On 10/11/2010 miner informed me that he needed to go to the doctor.

On 8/11/2010, The injured was replacing a water line on the oxygen analyzer at the feed-end of #1 kiln. when the injured reconnected the hose and turned the water back on, the hose blew off spraying hot water on the employee's face, stomach area and left thigh area.

Employee injured his back while installing a DC drive motor in the reclaim tunnel on October 28, 2008. This injury was filed in October of 2008. This is a reoccurrence of an old injury.

Employee was mucking out the settling ponds at the mill using a longreach trackhoe. He caught a ride back to the quarry garage on a waste truck. When he exited the waste truck, he slipped on the steps, lost his hand hold and fell to the ground on his rear. The fall was from the top step approximately three feet from ground level.

Employee was inside the framework of #2 surge conveyor preparing to cut an roller bracket off with a oxygen/acetylene torch. When employee lit the torch, he heard a gurgling sound. Employee attempted to the gas and air off. before he could get it off, the oxygen hose blew into causing 1st degree burn to the employee.

Employee was dumping mud from a 773 Rock Truck. The mud became stuck in the bed, as the employee put the truck in drive gear and started forward, the front end of the truck rose off the ground about 6 ft then falling back to the ground, jarring the employee's lower back.

Employee driving a 773B rock truck. While the truck was being loaded with stone, the trackhoe bucket and stone jarred the truck, causing employee to experience neck and shoulder pain.

Employee was troubleshooting a problem with the #3 tertiary crusher. Fellow employee came to help. After finding the crusher plugged, the fellow employee attempted to de-energize the circuit breaker. Employee was standing behind fellow employee in the doorway. As fellow employee switched off the breaker, it exploded causing an arc flash.

Employee shutdown the crushing plant from the computer in the control room, and left the crusher control room to help troubleshoot a problem with the #3 tertiary crusher. After finding the crusher plugged, the employee attempted to de-energize the circuit breaker. As employee was switching off the breaker, it exploded causing arc flash.

Back dragging with dozer from the B2 stock pile when he backed off in a dip from a previous drag. This caused a jar, causing his neck to start hurting.

EE was attempting to cut broken V-belt from the drive pulley of the grizzly feeder. He was using a utility knife. His first cut was directed away from his body. That cut went half way through the belt. He turned the utility knife toward his body and attempted to finish the cut. The knife slipped off the belt and cut his left thumb.

EE was driving a Ford L-9000 tandem truck from the settling ponds to the east waste pile. After driving for two hours, he experienced sharp pain in his right shoulder. He took two pain tablets and continued to drive until approximately 2:45 pm. EE told supervisor that the pain had gotten worse and he needed to see a physician.

Miner was helping to set a portable diesel pump. While lifting the pump off of the towing ball, the pump moved forward and miners left ring finger was mashed between the tow ball on the truck and the ball receiver hitch on the portable pump.

Employee was tightening a bolt on kiln 1 with a wrench. His hand got caught by the ram causing the wrench to go through his hand.

The uphill drive side bearing was being replaced. The new bearing was being held in place with a chain connected to a chainfall. Employee began pushing the uphill drive side bearing housing on to the drive shaft and sustained a hernia. Employee had surgery on 10/17/08 and began working restricted duty on 10/21/08.

Employee was cleaning the hydraulic room. There was approximately one inch of water, hydraulic fluid and glycol on the floor, and employee received burns to his left foot.

Employee was looking at driver's side mirror with her head leaning to the left. Large boulder loaded onto rock truck. Boulder shifted causing employee to hit her head inside the cab.

Employee was climbing down from the D8 dozer and slipped on the step hitting his knee on the tracks of the dozer. Employee developed cellulitis in the knee.

Worker was using a 12 lb. sledge hammer to break up a concrete wall. When he stopped and began to clean up the area, he experienced intense pain and was temporarily unable to use his right shoulder. The injury was diagnosed as a shoulder strain. He was prescribed anti-inflammatory and pain medication for one week.

While punching a hole through the screen to install a patch a piece of lime bounced up off of the screen and into his eye. Employee went to the doctor on 06/21/2007 and was given prescription medication.

EE was welding when he turned to get a part when another EE began grinding. The grinding debris went under EE's safety glasses & struck his left eye. He suffered a corneal abrasion and was treated with an eyewash & antibacterial cream from the physican. No futher problems have been noted.

Injured was helping clean rams pulling on wand and hurt his back.

Struck against stationary object

EE was pulling on a tagline which was attached to a steel pipe hanging from a crane. The tagline came lose and the employee fell backwards and struck his elbow on a steel beam.

Struck by rolling or sliding object

EE was rigging beams and instructed the forklift operator to retract the forks of the lift. When he retracted the forks they caught on the ground causing the beam to turn over onto the EE's foot causing his big toe to be broken.

Fall to the walkway or working surface

While working on kiln pier employee tripped over welding lead and fell on his right shoulder and his left knee causing a contusion injury to his left knee. Employee received treatment for his injury and will have followup at the doctor's office to determine when he will be able to return to his regular duties.

Employee squatted down to glue PVC conduit & felt his knee pop. An MRI on 6/30/2006 showed a torn ligament. Surgery performed on 7/5/2006.

Injured was attempting to break loose a plugged lime chute using a bar and hammer. When lime broke loose, it came through the 5"" port hole and hit injured in the face. It knocked the dust mask loose causing injured to breathe in a large amount of lime.

Fall from ladders

EE was 7' above ground with safety harness on. He secured beam w/rope to forklift to bring beam down. He used cut torch to release beam from building. Untied from the building and came down ladder. Forklift held beam in position. Beam made a noise as it settled on forklift arms and EE thought beam would fall on him so he jumped off ladder 5' above ground. Result: fractured tibia.

INJURED WAS REMOVING GUARD RAILING FROM AN ABANDONED SECTION OF CATWALK. WEEDS WERE GROWING UP THROUGH THE CATWALK GRATING. INJURED STEPPED INTO A HOLE IN THE CATWALK CAUSING A KNEE STRAIN WHEN HIS LEG & FOOT WENT THROUGH THE HOLE. REPORTED BECAUSE OF RESTRICTED DUTY.

INJURED WAS REACHING IN AN AWKWARD POSITION. HE EXPERIENCED A SHARP PAIN IN HIS BACK CAUSING HIM TO FALL, INJURING HIS ELBOW AND BACK. COULD POSSIBLY HAVE BEEN AN AGGRAVATION FROM AN OLD BACK INJURY OCCURRING 5/8/97. BECAME REPORTABLE ON 6/21/04 BECAUSE OF LOST TIME.
